About 40% of vehicle-miles traveled occur on rural roads; however, these rural trips account for 54% of all traffic fatalities. In 2010, a total of 18,026 lives were lost in crashes on rural roadways. Several factors contribute to these higher injury and fatality rates, including that rural crashes are more likely to involve higher vehicle speeds, a lower rate of seat belt use, and less availability of timely emergency medical care. Rural crashes present unique challenges for emergency medical service (EMS) systems. Compared with urban areas, a greater percentage of rural crashes result in multiple fatalities and higher rates of head-on collisions, roll overs, and ejected crash occupants. In addition, rural EMS systems often rely on a volunteer force, and tend to have less financial resources for staffing, equipment, and training. Response times in rural areas are longer owing to the greater travel distances required to reach the scene of a crash. Additionally, some rural EMS systems operate in areas with limited telecommunication options. This synthesis study explores the state of the practice for a broad cross-section of EMS system characteristics. It identifies factors that affect the timely provision of effective medical care in rural areas. In addition, it examines broader issues such as personnel, data records, and interaction with other agencies"--Summary.

Book excerpt: The motivation for this study is to reduce the adverse impacts of trauma caused by motor vehicle crash (MVC), including rural regions, where crashes account for a high percentage of trauma injury and death. One key aspect of reducing adverse effects of MVCs is to improve emergency medical services (EMS)--that is, to increase the efficiency and effectiveness of emergency and transportation services to respond, clear the scene of the crash, provide patient care, and take a patient to the correct hospital. The study centered on a 6-month demonstration of a multi-media (voice, video, pictures, data) EMS application ("CrashHelp") in the greater Boise area. For the pilot test period, 1,513 CrashHelp reports, 306 digital images, and 1,121 voice recordings were sent by 81 paramedics to 8 hospital emergency departments (EDs). EMS and ED staff generally thought the system was easy to use, captured useful information, and could play a role in enhanced clinical decision-making.

Book excerpt: The focus of this exploratory study is emergency medical response (EMS) for motor vehicle crashes (MVCs) in American Indian reservations and communities. Tribal transportation professionals have raised questions about the role of EMS in the high MVC fatality rate – 656 annually – in these areas. We conducted a national survey (n=189) of tribal governments, first responders, and state-tribe transportation liaisons. They assessed factors in the quality of EMS response in their areas (e.g., 911 access, dispatch, accessibility of MVC locations, responders’ training and equipment, distance to hospital; and inter-jurisdictional coordination). We recommend continuing research on MVCs specifically in American Indian reservations and communities. Specifically, more research is needed on dispatch issues (e.g., cell phone coverage and dispatchers’ ability to pinpoint MVC sites) because this is study respondents’ single highest area of concern, and the whole EMS response hinges on successfully placing a call for help. And, examples of productive inter-jurisdictional coordination need to be identified since tribes report vastly different experiences with this aspect of EMS system functioning. Two statistically significant findings merit further study, namely: a) heightened concern about response times and hospital access among responders from the Pacific Northwest and Alaska (compared with other regions); and b) higher optimism on all aspects of EMS response from study participants who work for tribal governments versus participants without that affiliation. Finally, it is important for researchers to communicate clearly about the geographic areas covered under "American Indian reservations and communities” in order to improve characterization of road safety and EMS issues.

Cardiac arrest is the third leading cause of death in the United States, following cancer and heart disease. Four out of five cardiac arrests occur in the home, and more than 90 percent of individuals with cardiac arrest die before reaching the hospital. First and foremost, cardiac arrest treatment is a community issue - local resources and personnel must provide appropriate, high-quality care to save the life of a community member. Time between onset of arrest and provision of care is fundamental, and shortening this time is one of the best ways to reduce the risk of death and disability from cardiac arrest. Specific actions can be implemented now to decrease this time, and recent advances in science could lead to new discoveries in the causes of, and treatments for, cardiac arrest. However, specific barriers must first be addressed. Strategies to Improve Cardiac Arrest Survival examines the complete system of response to cardiac arrest in the United States and identifies opportunities within existing and new treatments, strategies, and research that promise to improve the survival and recovery of patients. The recommendations of Strategies to Improve Cardiac Arrest Survival provide high-priority actions to advance the field as a whole. This report will help citizens, government agencies, and private industry to improve health outcomes from sudden cardiac arrest across the United States.

Book excerpt: Deteriorating traffic conditions and resulting safety problems on I-81 have long been a topic of concern. This, coupled with increasing traffic congestion along this largely four-lane highway, has resulted in increased crash rates. Emergency medical service (EMS) responders summoned to motor vehicle crashes along I-81 are trained volunteers with experience in dealing with motor vehicle crashes but do not have critical care treatment abilities. In serious crashes additional expertise is needed at the crash site to prevent disability or death. In these cases, a medical transport helicopter is summoned to the site to bring critical level care to victims and transport them rapidly to the nearest trauma center. A significant number of motor vehicle crash victims along the I-81 corridor are eventually transferred to the University of Virginia Hospital, the closest level 1 trauma center, by the Pegasus medical evacuation helicopter staffed by specially trained critical care providers. Since critical care trained providers, doctors, and nurses cannot be present at each crash site, the next best situation would be if such personnel could see the crash site remotely through ground-to-air video and be able to give treatment advice to EMS responders. To this end, companies with "off-the shelf" technology that could potentially work for this project were identified and contacted, and they subsequently provided information and/or demonstrations of their products. The project director made a determination as to whether the equipment could perform in a technically acceptable fashion and whether the company was willing to make any modifications to ensure proper configuration of the equipment. A public demonstration of the identified and customized equipment was held at the Charlottesville-Albemarle Airport in July 2001 and was attended by representative stakeholders, end-users, and members of the media. The demonstrated equipment performed in accordance with all expectations. Thus acceptable technology exists for the deployment phase of this study to proceed

This study explored police and EMS response times and hospital distances in occupational motor vehicle crash fatalities--the leading cause of occupational fatalities in Wyoming. Objectives. Identify police and EMS travel distances and response times correlated with victim survival for transport to a medical facility versus death on site. Methods. A database was created from WYDOT 2003-2007 crash reports previously identified in occupational fatality surveillance (Anderson, et al. 2010). Estimated time of incident, police and EMS notification, and scene arrival times were used to analyze response times in relation to event outcome. ArcGIS was used to analyze distances from crash sites to hospitals. Statistical tests included descriptive, Chi-Square's and ANOVAs. Results. Significant difference was found between EMS notification time and on-scene arrival in relation to event outcome. Significance was found for travel distances between 21-40 miles of crash site to nearest hospital. Significance did not exist between police response times and population demographics including vehicle type, ejection/entrapment, and state of victim residence. Implications. The results of this study have the potential to impact policy and emergency healthcare services in Wyoming. A map was created portraying the 21-40 mile radius surrounding Wyoming hospitals indicating distances in which event outcome may be influenced. Regional Trauma Centers were only third in the trauma level designation of hospitals nearest crash sites indicating the majority of accident victims were not nearest to the highest level of trauma care providers in the state.
