Nous présentons une étude prospective qui vise à rechercher et caractériser une dysrégulation vasculaire au niveau de la choroïde rétro fovéolaire chez les patients porteurs d'un SAS.Matériel et Méthodes : Nous avons inclus des sujets chez qui le SAS venait d'être diagnostiqué. Nous avons apparié en âge, sexe et indice de masse corporelle (IMC) ces sujets avec des témoins sains. Chaque sujet a bénéficié d'un examen cardio vasculaire, neurologique et ophtalmologique complet. Une polysomnographie, une évaluation cardio vasculaire avec échographie cardiaque, échographie carotidienne, vitesse de l'onde de pouls, mesure ambulatoire de la pression artérielle. La mesure des paramètres de vascularisation choroïdienne - volume ChBVol ; vélocité ChBVel et flux ChBFlow - a été réalisée grâce au laser Doppler fluxmètre. La réactivité choroïdienne a été testée ors de l'hypercapnie et ors de l'hyperoxie. Résultats : Les patients SAS présentent la même réactivité choroïdienne lors de l'hypercapnie (augmentation du flux de 1,5% par mmHg de PCO2) et lors de l'hyperoxie (flux stable) que les sujets témoins. Discussion : Une réactivité moindre au CO2 était attendue chez les sujets SAS avec altération de la vasodilatation par anomalie du système du monoxyde d'azote, par hyperactivité sympathique. Cette absence de variation peut s'expliquer par la pureté de notre population indemne de pathologies intercurrentes.
