Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

Book excerpt: As one of the nations with the most efficient and reliable healthcare systems, the United States has mainly focused on the acquisition of healthcare insurance plans by citizens during the current century. However, evidence shows that some children are not eligible for health insurance. Some parents get their insurance through an employer which doesn't cover children or parents or guardians with Medicare and can't have dependent coverage. Today, there are three main healthcare insurance plans that children in California can apply for: Children Health Insurance Program (CHIP), the County Children's Health Initiative Program (CCHIP), and Medi-Cal. However, each health insurance program is significantly different. Undertaking a policy analysis with social justice lens, this study proposes to examine all three California children's insurance options--barriers and the eligibility criteria and will try to describe the significant number of benefits of high-quality patient care and the distribution for children in California. This graduate project proposes to use the social justice policy framework adapted by Jillian Jimenez (2015) to examine the state's three healthcare insurance programs. The results will offer clarity and suggestions to both government policy and healthcare practitioners.

Over the past two decades, the majority of states have outsourced the provision of social health insurance through Medicaid Managed Care (MMC) programs. These programs have been extensively studied in the literature - with little evidence of large positive or negative effects. However, most states initially allowed older and sicker enrollees to remain enrolled in the government run fee for service (FFS) programs. It is possible that these more fragile enrollees could have a different experience in managed care. In this paper we study California's mandatory enrollment of the senior and persons with disabilities (SPD) population in MMC. We find this mandatory enrollment caused an increased use of the emergency department and transfers between hospitals. This was not simply a hassle cost for enrollees - we also estimate an increase in mortality for the affected population. These effects were strongest for the sickest enrollees - the types of enrollees that might be expected to have a different experience with managed care. Our results suggest the adverse impact of MMC varies by the enrollee health, which should inform the optimal outsourcing decision for governments.

Book excerpt: Excerpt from Report of the Social Insurance Commission of the State of California: March, 1919 The Social Insurance Commission appointed by Governor Hiram W. Johnson in 1915 reported to the legislature at its forty-second session, in accordance with the creating act. The report, based upon a general survey of the social and industrial conditions of California in relation to the destitution problem, included the recommendation that California establish a social health insurance system. Pursuant to this recommendation, the legislature, by a two-thirds vote of both houses, proposed to the people of the state of California a health insurance enabling amendment to remove technical constitutional Obstacles standing in the way of health insurance legislation. In addition, the legislature re-created by chapter 57, section 22, of the statutes of 1917 the Social Insurance Commission as an unsalaried body, similar to the commission appointed in 1915. The commission was required to report to the forty-third session of the legislature the details of any social insurance measure which it desired to recommend. Pursuant to the terms of the act, Governor William D. Stephens appointed the following members: Miss Katharine C. Felton, Mrs. Frances N. Noel, George H. Dunlop, Dr. Flora W. Smith, Daniel C. Murphy, Ansley K. Salz, and Chester H. Rowell. These appointees met for the formal reorganization of the commission in August, 1917. At this meeting Mr. Chester H. Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Book excerpt: California's unprecedented reduction on welfare caseload has potentially serious consequences for health-insurance coverage. Although all welfare recipients are automatically covered by Medi-Cal (the state version of Medicaid), only some former recipients are eligible. This research examines health insurance coverage among families who have left welfare, using data on approximately 3,000 current and former welfare recipients from the California Health and Social Services Survey. The authors found that a substantial proportion of people in former welfare families are uninsured--30 percent of female family caretakers; 23 percent of children; and 43 percent of partners and spouses of female caretakers. Hispanics are most likely to be uninsured. Many of these uninsured individuals many be eligible for, but not participating in, Medi-Cal or Healthy Families, California's State Children1s Health Insurance Program. Medi-Cal coverage declines as time away from welfare increases. However, this trend is countered by changes in employee-sponsored insurance coverage, which is higher among those off welfare longer. Of former welfare recipients who had some form of employment in the month prior to being interviewed, slightly more than half were offered insurance. Complementary strategies to improve the likelihood that such insurance is offered would improve health insurance outcomes.
