Book excerpt: WHO published in 2020 a comprehensive protocol for measuring and monitoring industrially produced trans-fatty acids (TFA), intended to serve as a reference method that is globally applicable for surveillance and monitoring of TFA content for different types of foods. Although this WHO reference protocol was successfully implemented in several laboratories worldwide, some laboratories, especially those operating on a tight budget, had difficulties adopting it. To address the above challenges, WHO held a two-day, virtual WHO expert consultation meeting on 27 and 30 June 2022 to discuss simplified procedures and other suitable alternatives that enable laboratories operating with limited resources to conduct fatty acid methyl ester (FAME) analysis. Based on this expert consultation, the WHO Laboratory Protocol – Simplified protocol for measuring trans-fatty acids content as a percentage of total fatty acids in food products was developed. This fit-for-purpose protocol provides the data that are required for governments’ surveillance and monitoring activities to check the trend of TFA content in food products on the market and ensure that food products comply with regulations for TFA elimination.

Book excerpt: The current guideline for the consumption of trans fatty acids (trans FA) is that they should not exceed 2% of food energy. Due to health concerns, the Food Standards Agency was asked to consider the case for a reduction in this recommendation to less than 1% and the Scientific Advisory Committee on Nutrition has thus tasked with examining recent scientific evidence on the health effects of trans FA. It concludes that there is consistent evidence to support a moderate effect of trans FA on increasing the risk of coronary heart disease but evidence related to other diseases is limited and no reliable risk assessment can be made. There is however no firm scientific basis for a change in the current recommendation of not exceeding 2% of food energy. There is also insufficient evidence to justify the differentiation of trans FA from vegetable oil and animal sources.

The oxidation of unsaturated fatty acids is one of the most fundamental reactions in lipid chemistry. When unsaturated lipids are exposed to air, the complex, volatile oxidation compounds that are formed cause rancidity. This decreases the quality of foods that contain natural lipid components as well as foods in which oils are used as ingredients. Furthermore, products of lipid oxidation have been implicated in many vital biological reactions, and evidence has accumulated to show that free radicals and reactive oxygen species participate in tissue injuries and in degenerative disease. Although there have been many significant advances in this challenging field, many important problems remain unsolved. The most frequent underlying pathology, namely atherosclerosis, and its clinical sequelae, namely coronary heart d- ease, cerebrovascular disease and peripheral artery disease, remain common although for a long time we have been made aware of avoidable or mo- ?able etiological factors such as smoking, fat-rich diet or lack of exercise, and although these adverse lifestyle factors have been extensively addressed by population-wide primary prevention programs. Cardiovascular morbidity and mortality also remain high despite successful anti-hypertensive and lipid lowering drug therapies which help to reduce cardiovascular morbidity and mortality by about 30% in both secondary and tertiary prevention settings. This can partly be explained by the increasing life expectancy and growing p- portionofelderly people,especiallyinEuropeandNorthAmerica. Inaddition, the World Health Organization makes the alarming prediction that probably in response to the spreading of western dietary behavior and lack of exercise resulting in an increasing prevalence of diabetes, dyslipidemia and hypert- sion, cardiovascular diseases rather than infectious diseases will become the most frequent cause of death worldwide. This volume of the Handbook of Experimental Pharmacology entitled “Atherosclerosis” is divided into four parts and intends to give an overview on the pathogenesis of atherosclerosis, established treatment and prevention regimen, and of perspectives for the development of new treatment modalities.
