Book excerpt: To maintain good health, patients with Type 2 diabetes (T2DM) must perform self-care activities (SCA), many of which require health literacy – the ability to obtain, find, and use healthcare information to make decisions about health. In this dissertation, using the information-motivation-behavioral skills theoretical framework, I assess and examine health literacy in relation to SCA among underserved people with T2DM. This descriptive correlational study is a secondary analysis of data from a larger study of participants from six federally qualified health centers in an urban county in South Central Texas. The sample comprised 388 patients with T2DM (261 English-speaking, 127 Spanish-speaking; 53±10.33 years old), with the majority reporting an annual income of less than $10,000. Functional health literacy was measured with the Newest Vital Sign screening tool; diabetes-specific oral health literacy, with the DM-REALM – a modification of the original REALM instrument. Only 18.3% of participants exhibited adequate functional health literacy; 10.4%, adequate oral health literacy. In bivariate analysis, functional health literacy was related to age, gender, language, acculturation, income, education, and marital status. Oral health literacy was related to gender, employment status, and type of insurance. Significant predictors of functional health literacy included gender, education, and type of insurance. Significant predictors of oral health literacy included gender, employment status, and years with T2DM. In bivariate analysis, diabetes self-efficacy and health information-seeking behaviors were related to diabetes SCA. In hierarchical multiple regression, functional health literacy, health information-seeking behaviors, and diabetes self-efficacy were significant predictors of diabetes SCA. Although health literacy did not exhibit a significant direct effect on diabetes SCA, health information-seeking behaviors mediated the relationship between oral health literacy and diabetes SCA. These findings add to previous findings showing the high prevalence of limited health literacy among underserved people and the negative influence of limited health literacy on SCA among people with T2DM. Further studies of the impact of limited health literacy on diabetes SCA in diverse, underserved populations of people with T2DM are warranted. Future health literacy interventions should incorporate individual- and systems-level approaches in addressing promotion of diabetes SCA

Abstract: Physical inactivity has been widely regarded as a leading cause of non-communicable chronic diseases such as type 2 diabetes mellitus (T2DM) and hypertension. Physical activity (PA) has been shown to be the most effective self-care behaviour in alleviating T2DM, an incurable chronic disease. Previous behavioural modification programs intended to encourage and maintain PA behaviour among middle-aged and older patients with T2DM have had inconsistent results. PA has remained as a recommended diabetes self-care behaviour with the lowest compliance rate in many regions of the world. This thesis proposes a program focusing on health literacy (HL) as a means to nurture T2DM patients' ability to navigate, read, understand, process, comprehend and act on health information, and thereby modify PA behaviour. This approach is proposed for three reasons: (1) the flood of available health information of varying quality affects patients' decision making with regard to self-care behaviour; (2) the high prevalence of low HL in the US, Europe, and Asia affects the comprehension of self-care behaviour programs; (3) because 50% of older T2DM patients are impaired cognitively due to the impact of aging and hence have difficulty in carrying out suitable self-care behaviour without proper guidance. All of these factors can influence the effectiveness of PA behavioural modification programs designed to help middle-aged and older T2DM patients make self-care decisions based on sound health information according to their level of HL. This thesis reviews existing HL-oriented programs and models and validates the instrument used to examine the effects of a culture-, language-, disease- and age-specific, theory-based, HL-oriented program on PA behaviour among middle-aged and older patients with T2DM. In the study, 324 middle-aged and older Chinese T2DM patients recruited from two hospital diabetes centres in Hong Kong were divided randomly into intervention and control groups. The intervention group participated in an HL-oriented program on PA behaviour. The program significantly improved the mean of PA behaviour of patients in the intervention group from a sedentary level (under 1,000 average Metabolic Equivalents/minutes/week; 4463 mean number of steps per week) to an internationally acceptable standard (over 2,000 average METs-minutes/week; 7459 mean number of steps per week), and this improvement was maintained at three- and six-month follow-ups. The PA behaviour of the control group actually subsided. This study showed that a program addressing and nurturing patients' ability to explore, understand and manage health information on T2DM and PA was effective in improving the PA behaviour of middle-aged and older adults with T2DM. This thesis is the first study to use objective measurements to evaluate the effect of a culture-, language-, disease- and age-specific, theory-based, HL-oriented program on PA behaviour. It extends the generalizability of culture and language-sensitive HL oriented programming from the United States (where previous HL studies were conducted) to China. The study can serve as a model for future investigations of self-care behaviour among patients with different chronic illnesses in various regions of China. Book excerpt: Type 2 diabetes mellitus is a significant health issue, as it requires patients to perform daily self-management activities. One in every three Americans with Type 2 diabetes has limited or low health literacy skills. Limited or low health literacy skills can impact a patient’s overall health outcome; therefore, a critical element for disease management and patient adherence is health literacy. The purpose of this three-month scholarly project was to determine if the utilization of an educational intervention specific to one’s level of health literacy, combined with a short message service reinforcement tool, could improve glycemic control and self-efficacy in Type 2 diabetes mellitus patients with limited and low HL. A sample of four participants was recruited for this scholarly project. Participants completed pre and post self-efficacy and health literacy questionnaires. During the three-month intervention phase, participants interacted with the project leader utilizing the text messaging reinforcement tool. After the three-month intervention phase, participants had an overall 9% increase in self-efficacy confidence, a 42% overall increase in health literacy knowledge, and an overall HbA1c mean value reduction of 0.5%. This combination therapy intervention is easy and convenient for health care providers and Type 2 diabetes mellitus patients with limited or low health literacy. This intervention is an efficient and effective method to improve patients self-efficacy and diabetes knowledge; furthermore, this method improves patient self-management skills, glycemic control, and patient longevity.

To successful management diabetes and avoid complications, one must have proficient health literacy ability to understand and follow recommend management techniques efficiently and effectively (Liang & Brach, 2017). Evidence-based diabetes self-management programs that use low health literacy tools have proven to lower the glycated hemoglobin (A1C) levels and improve diabetic knowledge and practices of participants. In this capstone project, a multimodal, low health literacy, diabetes self-management toolkit was implemented in an inner-city clinic among persons with diabetes and various health literacy levels. From the clinic, twelve patients with diabetes were recruited, consented, and enrolled into a tiered diabetes education intervention. Participants were mostly female (n=7), mean age 52 years old, all high school graduates and most having high health literacy. Participant's health literacy level was measured using the Rapid Estimate of Adult Literacy in Medicine (DM-REALM) survey and knowledge was assessed via the Brief Diabetes Knowledge survey pre-intervention and post-intervention. Compliance was measured by self-report and pre-intervention and post-intervention A1C levels. Participants improved medication adherence and endorsed eating less carbohydrates pre to post intervention. The group mean A1C declined pre-intervention to post-intervention while knowledge scores increased across all literacy levels. This three-month evidence-based, diabetes education intervention showed that patients with diabetes could successfully manage their diabetes when provided with low health-literacy tools. More research is needed to explore the impact of diabetes toolkits on diabetes knowledge and A1C's longitudinally in this population. Book excerpt: The field of health literacy has evolved from one focused on individuals to one that recognizes that health literacy is multidimensional. While communicating in a health literate manner is important for everyone, it is particularly important when communicating with those with limited health literacy who also experience more serious medication errors, higher rates of hospitalization and use of the emergency room, poor health outcomes, and increased mortality. Over the past decade, research has shown that health literacy interventions can significantly impact various areas including health care costs, outcomes, and health disparities. To understand the extent to which health literacy has been shown to be effective at contributing to the Quadruple Aim of improving the health of communities, providing better care, providing affordable care, and improving the experience of the health care team, the National Academies of Sciences, Engineering, and Medicine convened a public workshop on building the case for health literacy. This publication summarizes the presentations and discussions from the workshop, and highlights important lessons about the role of health literacy in meeting the Quadruple Aim, case studies of organizations that have adopted health literacy, and discussions among the different stakeholders involved in making the case for health literacy.

Diabetes self-care involves seven essential behaviors, including: (1) eating a healthy diet, (2) being physically active, (3) being compliant with medication, (4) monitoring blood glucose, (5) having good coping skills, (6) being able to problem-solve, and (7) engaging in health risk reduction behaviors. Inadequate health literacy and poor social support have been associated with poor diabetes self-care. The link between diabetes self-care and health literacy among people of Arabic descent living in the United States remains unclear. No studies have been identified as featuring investigations of the level of health literacy among this group, nor have they clarified the links between health literacy and diabetes self-care behaviors. Such behaviors0́9 association with social support also merits further investigation. The overarching aim of this study is to investigate the level of health literacy among this population and to examine the relationship between perceived social support and health literacy, and diabetes self-care behaviors in individuals of Arabic descent who have diabetes (type 1 and type 2). For convenience, the study will be restricted to those living in Michigan. Method: Non-experimental descriptive correlational design comprised a convenience sample of 83 individuals of Arabic descent living in Michigan who have diabetes. Participants were older than 18, may be male or female.

Book excerpt: Background and Significance: The annual cost of diagnosed type 2 diabetes mellitus (DM2) in the United States is $327 billion, with individuals with DM2 spending 2.3 times more on health care than individuals without DM2. Education, such as Diabetes Self-Management Education and Support (DSMES) programs, and self-care support, utilizing tools such as the Summary of Diabetes Self-Care Activities (SDSCA) Measure, are the cornerstones of improving care and outcomes for patients with DM2. Purpose: The purpose of this quality improvement project was to implement a standardized self-care assessment and educational intervention for a mid-Michigan internal medicine clinic's adult patient population over 18 years of age with DM2 and hemoglobin A1c (HbA1c) greater than 9% and improve patient self-care. Methods: The Plan, Do, Act, Study (PDSA) Cycle and the Chronic Care Model were used as a framework to guide the project. Eligible participants were identified by clinic staff and received self-care education. Evaluation: The validated SDSCA tool was utilized to assess self-care pre- and post- educational intervention. A two-tailed paired t-test was then performed to compare pre- and post-intervention scores. Outcomes: Of 25 eligible patients in the clinic, 13 patients participated in the initial phase with 3 patients lost to follow-up, leaving 10 patients included in data analysis. Book excerpt: Controlling type 2 diabetes mellitus requires the individual to perform specific self-care behaviors. The current study investigated the impact of anxiety and depression on these behaviors. In addition, the study investigated the role of sociodemographic variables, sleep and pain in the performance of specific self-care behaviors. Using data collected from primary care clinics providing integrated care to individuals with type 2 diabetics, the Chi-Square Test of Independence was conducted to investigate relationships. This was followed by a series of logistic regressions to identify variables that could predict changes in self-care. Depression and anxiety was measured by scores on the PHQ-9 and GAD-7. Levels of self-care were measured by information collected from patients completing the Summary of Diabetes Self-Care Activities. Findings from this study suggest that self-care behaviors can be influenced by numerous factors: anxiety, age, ethnicity, gender, marital status, and sleep difficulty. Both a correlation and predictive relationship between anxiety and general diet scores were found, but anxiety did not have a statistically significant impact on any other individual self-care behavior. The current study makes a meaningful contribution to the literature by examining the influence of anxiety, depression, sociodemographic factors, sleep and pain on improvements in self-care behaviors.

Available in PDF, EPUB and Kindle. Book excerpt: Introduction: Type 2 Diabetes (T2D) is a chronic metabolic disease characterized by high blood glucose levels in the blood. It is associated with microvascular and macrovascular complications which can lead to potential threats such as to amputations and even death. The irony of the disease is that these complications are preventable with appropriate treatment and self-management. The Emergency Medicine Department (ED) at University of Southwestern Medical center conducted this study to assess health literacy Parkland Memorial Hospital patients with T2D. The objective for the research study was to assess the for association of health literacy with management of T2D. Methods: This was a prospective study with collection of personal health information (PHI) and 30 day-follow up for ED recidivism. Eligibility was assessed by pre-screening via EPIC (Electronic Medical Record System for Parkland). The tool for measuring health literacy was the Short Assessment of Health Literacy (SAHL) and data was collected. Results: The total number of subjects enrolled was 23 with ages 18 or above male and females both with Spanish or English speaking only with T2D. Mean age of the subjects was ~50 years with standard deviation of 10 years, males were over half than women. About 74% were white hispanic males. According to the data collected, 30% of the patients demonstrated inadequate health literacy based on SAHL score survey. Since the study could not reach adequate power due to low enrollment, no significant associations could be made from this small sample size. Total number of subjects required to have adequate power was 400. Conclusions: Due to low enrollment period at this time the recommendation would be to continue collecting data to have a larger sample size to afford the observation of statistically relevant associations. If any, statistically significant associations are found, then future studies will focus on improving diabetes outcomes through the development of educational tools at the individual patient’s appropriate literacy level. There are many reasons to improve diabetes care and explore all possible factors that contribute to poor outcomes. Millions of people are living with uncontrolled diabetes and the burden is not only on the patient but also on the community as a whole. Quality care should aim for improved benchmarks for patients with diabetes and their knowledge about the disease, such as 1) obtaining HbA1c levels below 8%, 2) blood pressure in the normal range, 3) having regular foot exams to keep a check on any developing signs of pressure sores and 4) most importantly having dilated eye exam on a regular basis.
