Treatment integrity is an important component of evidenced-based interventions and individuals who implement these interventions may encounter barriers to proper implementation. Performance feedback has been used as an effective way to improve treatment integrity for teachers, which has primarily used feedback regarding student outcomes and implementer adherence to intervention procedures. The purpose of the present studies was to consider the utility of home to school communication as a method of performance feedback regarding a student's response to a Daily Behavior Report Card (DRC) intervention targeting academic engagement in improving teacher treatment integrity levels. Results indicated that home-school communication in insolation might not be enough to promote high levels of teacher treatment integrity. Results also suggested that limited improvement in overall student academic engagement could be related to low to moderate levels of teacher treatment integrity, which provides additional support for the importance of implementation fidelity.

Book excerpt: Treatment integrity refers to the degree to which an intervention is implemented as intended. This book examines the adequacy of treatment integrity procedures in randomized controlled trials of psychosocial interventions and evaluates the associated factors. Results indicate that investigations that systematically implement treatment integrity procedures are virtually lacking in the literature. Skill-building treatments (e.g., cognitive-behavioral) as compared to non skill-building interventions (e.g., psychodynamic) were implemented with higher attention to integrity. Treatment integrity was addressed to a greater degree when interventions were evaluated using a treatment manual. Implications of the obtained results for psychotherapy research and practice, recommendations for improving how integrity is addressed, as well as guidelines on treatment integrity procedures are discussed. Further, this book includes a measure for assessing the adequacy of treatment integrity procedures and a rater manual.

Book excerpt: Objective: The relation between treatment integrity and youth client outcomes in psychosocial treatment research has implications for therapist training, study design, and study interpretation. Despite work done in this area, this relation remains unclear. The aim of the current study was to meta-analyze and describe characteristics of investigations of integrity and outcome in youth. Method and Analytic Plan: A total of N = 30 studies were identified. Conceptual and methodological factors were coded. Pearson's r represented the magnitude of the integrity-outcome correlation. Robust variance estimation was used to account for dependency of within-study effect sizes. Two models were run with identical methods, Model 1 did not impute/estimate effect sizes and Model 2 imputed/estimated missing effect sizes. Each model was built iteratively, including an unconditional model and moderator model. Tests of heterogeneity, publication bias, and sensitivity analyses were conducted. However, consultation is only effective if consultee's implement treatment plans with high levels of treatment integrity (Noell et al., 2005; Sanetti & Kratochwill, 2008). There are many factors that influence consultee's treatment integrity. The current study examines teacher respondents' answers to the Evaluating Factors that Influence Treatment Integrity During Consultation Survey. Results indicate the teacher consultees believe peer educational consultants to have more knowledge in the area of behavior. Further results indicate that there are up to 19 factors that teachers rate as important in the consultation process.

Book excerpt: The issue of treatment fidelity has been a concern in the field of evaluation research. Nearly three decades ago, Cook and Campbell (1975) outlined four types of validity that may influence treatment outcomes. Defining these validity measures has since prompted researchers to closely examine potential threats within the context of program evaluation. Related to Cook and Campbell's concept of construct validity, Sechrest and associates (1979) identified two complications that could further impact evaluation research. These complications involve the strength and integrity of treatment, and their associated implications on construct validity. The purpose of this study was to assess the delivery, receipt, and adherence of the CRI treatment implemented in the four Kanawha County schools. In creating a model for assessing treatment implementation, Lichstein, Riedel, and Grieve (1994) posit that there are three components in their model. Delivery assessment inspects how the treatment was delivered and the factors that enhanced or hindered delivery of the treatment. Receipt assessment examines whether the intended audience was able to understand the information that was provided to them. Adherence assessment examines whether the clients? receipt of the treatment will effect them; enactment looks at whether clients adhere to the directives and change behaviors. This study was designed to provide a viable test of the evaluation of an educational intervention employing a set of multiple components and multiple methods. These various components and methods for assessing the integrity of the intervention were suggested by Lichstein, Riedel, and Grove.

Book excerpt: Multi-tiered models of prevention such as Response-to-Intervention (RtI), Schoolwide Positive Behavior Support (SWPBS), and comprehensive, integrated models of academic and behavioral support (CI3T) are being implemented in schools across the country in an effort to provide early intervention and evidence-based practices to all students. To determine the quality of implementation and make data-based decisions about student responsiveness to these models, it is imperative to know if and how well the models are being implemented. Current research indicates the behavioral components of the primary plan of these models are most frequently assessed using the Schoolwide Evaluation Tool (SET). Although the SET evidences reliability and validity, it is used to measure school-level rather than classroom-level implementation. Thus, the purpose of this study was to examine initial evidence of reliability and validity of a tool designed to measure primary plan treatment integrity at the classroom level : teacher self-report over time (TSR). Specific research questions include: 1) What is the internal consistency, as measured by alpha coefficients, of the TSR?, and 2) What are the correlations between the TSR subscales and total scores with the SET subscales and total scores? According to our findings, the TSR demonstrated initial evidence of internal consistency as demonstrated by Cronbach's alpha levels ranging from .76 to .83. Further, we found the TSR and SET likely do not measure the same constructs as they were not significantly correlated. Limitations and suggestions for future research are provided.

Book excerpt: Frequently school psychologists are asked to develop treatments for teachers to remediate students' academic skills or social behaviors. When teachers implement them with high levels of treatment integrity they are beneficial to students. Treatment integrity has been increased by using direct training, performance feedback, and negative reinforcement procedures. Still, treatment integrity maintenance has not received the same amount of systematic inquiry. This study investigated treatment integrity maintenance over time after a systemic fading procedure with indiscriminable contingencies was implemented. The results showed that the teachers continued to implement treatment protocols at a high level after performance feedback was faded out. Students also fared better when treatment integrity was high.

Book excerpt: The primary purpose of this study was to examine whether consultant-consultee contracting could improve treatment integrity in an academic setting. Contracting has long since been utilized as a means to improve treatment integrity in the medical community as well as weight loss programs, drug addiction counseling, and for the improvement of study-skills. Treatment integrity is also an important research subject in the education community, thus making contracting an interesting extension of the current treatment integrity research. In addition, the researcher examined the effectiveness of parental involvement in cases where contracting alone was insufficient in achieving treatment integrity gains. The researcher was also interested in student improvement made from the academic interventions. A Small-N multiple baseline design with three participating teachers was utilized for this study. After an initial training phase, daily treatment integrity levels were monitored by the consultant. When integrity levels fell below a pre-established criterion, the contract was implemented. In cases where contracting alone was insufficient, parental involvement, which included a school-home note, was provided and the consultant continued to monitor treatment integrity levels. Two of the three teachers showed initial treatment integrity gains with the consultant-consultee contract, with one participant showing treatment integrity maintenance. One teacher fell below the pre-established criterion in the contracting phase and was provided additional support through the parental involvement phase. This teacher showed initial treatment integrity gains with parental involvement that were above levels in baseline and the contracting phase.
