The first objective was to evaluate the effects of an asthma education and self-management skills training program on asthma medication adherence. The second objective was to see if peak flow would increase as medication adherence increased. Five participants who were non-adherent to their medication regimen were exposed to the education and self-management skills training components using a multiple baseline design across participants. During asthma education participants were taught about asthma, how to manage it using medication as well as the effects of medication adherence. During the self-management skills training participants were taught how to take an active role in managing their asthma as well as how to utilize a peak flow meter to determine lung capacity. The results indicated that the asthma education and self-management skills training have the potential to increase medication adherence. Although, the results do not indicate a strong correlation between increased medication adherence and increased peak flow measures"--Document.

Book excerpt: OBJECTIVES: To synthesize the published literature on the effect of interventions designed to improve health care providers' adherence to asthma guidelines on: (1) health care process outcomes (Key Question 1); (2) clinical outcomes (Key Question 2); (3) health care processes that subsequently impact clinical outcomes (Key Question 3). DATA SOURCES: Reports of studies from MEDLINE(r), Embase(r), Cochrane Central Register of Controlled Trials (CENTRAL), Cumulative Index to Nursing and Allied Health Literature (CINAHL(r)), Educational Resources Information Center (ERICsm), PsycINFO(r), and Research and Development Resource Base in Continuing Medical Education (RDRB/CME), up to July 2012. REVIEW METHODS: Paired investigators independently reviewed each title, abstract, and full-text article to assess eligibility. Only comparative studies were eligible. Investigators abstracted data sequentially and independently graded the evidence. RESULTS: A total of 73 studies were eligible for review. A slight majority of studies were conducted in the U.S. (n=38). We classified studies as assessing eight types of interventions: decision support, organizational change, feedback and audit, clinical pharmacy support, education only, quality improvement (QI)/pay-for-performance, multicomponent, and information only. Half of the studies were randomized trials (n=34), 29 were pre-post, and the remaining 10 were a variety of nonrandomized study designs. The studies took place exclusively in primary care settings. The most frequently cited health care process outcome was prescription of asthma controller medication (n=41), followed by provision of an asthma action plan (n=18), prescription of a peak flow meter (n=17), and self-management education (n=12). Common clinical outcomes included emergency department (ED) visits (n=30) and hospitalizations (n=27), followed by use of short-acting ø2 agonists (n=9), missed school days (n=8), lung function tests (n=6), symptom days (n=6), quality of life (n=5), and urgent doctor visits (n=5). We identified 4 critical outcomes for which 68 studies provided information. There was moderate evidence for increased prescriptions of asthma controller medications using decision support, feedback and audit, and clinical pharmacy support interventions and low grade evidence for organizational change, multicomponent interventions. Moderate evidence supports the use of decision support and clinical pharmacy interventions to increase provision of patient self-education/asthma action plans; for the same outcome, low grade evidence supports the use of organizational change, feedback and audit, education only, quality improvement, and multicomponent interventions. Moderate grade evidence supports use of decision support tools to reduce ED visits/hospitalizations while low grade evidence suggests there is no benefit associated with organizational change, education only, and QI/pay-for-performance. Organizational change interventions provided no benefit for lost days of work/school. The evidence for the remainder of interventions was insufficient or low in strength. CONCLUSIONS: There is low to moderate evidence to support the use of decision support tools, feedback and audit, and clinical pharmacy support to improve the adherence of health care providers to asthma guidelines, as measured through health care process outcomes, and to improve clinical outcomes. There is a need to further evaluate health care provider-targeted interventions with a focus on standardized measures of outcomes and more rigorous study designs.

It also disproportionately affects low-income and racial and ethnic minorities. Although a wide range of interventions have been implemented to improve asthma-related outcomes among socioeconomically disadvantaged and minority children, disparities persist. This paper, which was commissioned by the JPB Foundation and conducted by MDRC in partnership with the National Academy for State Health Policy, standardizes and compares effects from rigorous evaluations across three main types of interventions: those that focus on improving education and self-management, those that focus on addressing remediation of the conditions that trigger asthma in the home environment, and those that focus on improving health care provider practice. While prior reviews have summarized findings for each intervention type, this review systematically compares findings across a range of outcomes and a spectrum of different approaches to addressing childhood asthma, drawing from about 30 independent studies and earlier published reviews. In addition, the paper includes case studies of various local programs and state-level policy initiatives to illuminate current efforts to address childhood asthma in low-income communities and barriers faced in sustaining asthma programs. Key findings include: (1) Providing education on how to manage asthma is fundamental to improving outcomes, but it may not be enough to alleviate disparities in asthma management; (2) Some local programs are clearly bridging informational gaps among low-income families and are helping them maintain a safe home environment for their children with asthma; (3) The health care benefits of interventions that go beyond education are unclear; (4) Barriers to medication management are complex and medication adherence is difficult to influence; and (5) The role of the state, and the Medicaid program in particular, is relevant for the financing and sustainability of asthma programs for low-income children. Although the long-run sustainability of some of the programs highlighted in this report is uncertain, what is clear is that combating the disparities in the prevalence of asthma among different populations will require more than an agreement on standard asthma management practices within the medical community. Book excerpt: This Review sets out to propose a structure for the funding arrangements for the whole spectrum of health research, with the objective of obtaining the maximum benefit from research success and, where possible, eliminating duplication of effort. The Review found, however, that the UK is at risk of failing to reap the full economic, health and social benefits that the UK's public investment in health research should generate. There is no overarching UK health research strategy to ensure UK health priorities are considered through all types of research and there are two key gaps in the translation of health research: (i) translating ideas from basic and clinical research into the development of new products and approaches to treatment of disease and illness; (ii) implementing those new products and approaches into clinical practice.The Review also found that the wider funding arrangements for supporting translation of ideas from conception to practice could be more coherent or comprehensive and, where arrangements exist, they do not function well. The Review identified cultural, institutional and financial barriers to translating research into practice in the publicly funded research arena. But it also found that, in the private sector, the pharmaceuticals industry is facing increasing challenges in translating research into health and economic benefit. The Review has sought to make recommendations that will increase the translation of R&D into health and economic benefit for the UK, both in the public and private sectors. The Review recommends that the Government should seek to achieve better coordination of health research and more coherent funding arrangements to support translation by establishing an Office for Strategic Coordination of Health Research (OSCHR).

Book excerpt: The impact of asthma self-management programs on medical resource utilization has been studied repeatedly. The impact of these programs on a child's perceived quality of life, however, has not been addressed. This descriptive, correlational study compared the perceived quality of life and number of hospitalizations, emergency room and clinic visits in asthmatic children ages 7 - 11 years who adhered to a self-management plan with those who did not adhere to a plan. Quality of life was measured using the Childhood Asthma Questionnaire, a valid and reliable quality of life measurement tool for children of different ages. There were no significant differences between the Adherence and Non-adherence groups relative to quality of life or medical resource utilization. Findings did support a significant decrease in medical resource utilization for both groups since the establishment of their self- management plan. These findings confirm the role of self-management education as a worthwhile and cost effective investment for children.

Asthma self-management education programs aim to improve asthma treatment, management, and control in the United States and help patients to develop the knowledge and skills about asthma. Previous studies have shown that asthma self-management education programs have improved quality of care for children and reduced medical care costs. However, most of these studies had small sample sizes or focused on children from one certain hospital or a selected region for analysis. To fill this gap, this study analyzes data from the 2006-2012 Behavioral Risk Factor Surveillance Survey (BRFSS) Asthma Call-back Survey (ACBS). ACBS is a nationally representative survey of children with asthma in the United States, which increases the generalizability of the findings. Multinomial logistic regression models are used to analyze the association between participation in asthma self-management education programs and health outcomes and healthcare utilization for children with asthma. The results indicate that receiving more asthma education is associated with fewer routine care visits, medical visits for asthma episodes, hospitalizations, emergency department visits, and urgent care visits, and better control of asthma symptoms. The benefits of asthma education on hospitalizations and emergency department visits did not differ by financial hardship or race/ethnicity. This study provides evidence that asthma self-management education improves health outcomes for children and reduces healthcare utilization.
