Book excerpt: Cette étude présente les résultats fonctionnels de 65 prothèses totales de genou SURFAX°, auprès d'une cohorte de 61 patients, avec un recul moyen de 22 mois (12 à 48). Il s'agit d'une prothèse semi-contrainte, caractérisée par l'absence de conservation des ligaments croisés, un ancrage par des tiges vissées et un pivot rotatoire. Elle permet une rotation axiale et indépendante de la flexion-extension. L'évaluation clinique préopératoire et postopératoire a permis de suivre l'évolution des scores de l'International Knee Society et d'obtenir un score moyen de 162.5+/-28 (sur 200 points), représentant 64% de bons et très bons résultats. Ces résultats sont comparables à ceux de la littérature. Il est apparu dans cette série que la reprise d'une prothèse, une patellectomie préalable, un nombre d'antécédents chirurgicaux locaux supérieur à quatre, l'obésité et, à un moindre degrés, un antécédent d'ostéotomie tibiale de valgisation, représentent des facteurs défavorables des résultats fonctionnels de cet implant. L'âge n'a pas d'influence sur ces résultats. L'intérêt de cet implant est mis en évidence dans le cadre des reprises de prothèse, des grandes déformations axiales et des destructions ostéocartilagineuses et ligamentaires importantes. Une évolution de l'implant pourrait s'envisager en restituant l'asymétrie des condyles pour restaurer une rotation automatique des condyles. Il conviendrait aussi de mettre au point un ancillaire spécifique pour les reprises de prothèse.
