Each country chapter captures the historical background of public health services and the context in which they operate; sets out the main organizational structures; assesses the sources of public health financing and how it is allocated; explains the training and employment of the public health workforce; and analyses existing frameworks for quality and performance assessment. The study reveals a wide range of experience and variation across Europe and clearly illustrates two fundamentally different approaches to public health services: integration with curative health services (as in Slovenia or Sweden) or organization and provision through a separate parallel structure (Republic of Moldova). The case studies explore the context that explain this divergence and its implications. This study is the result of close collaboration between the European Observatory on Health Systems and Policies and the WHO Regional Office for Europe Division of Health Systems and Public Health. In setting global health priorities and carrying out initiatives, the WHO bureaucracy has faced the challenge of reconciling the preferences of a small minority of wealthy nations, who fund the organization, with the demands of poorer member countries, who hold the majority of votes. In The World Health Organization between North and South, Nitsan Chorev shows how the WHO bureaucracy has succeeded not only in avoiding having its agenda co-opted by either coalition of member states but also in reaching a consensus that fit the bureaucracy's own principles and interests. Chorev assesses the response of the WHO bureaucracy to member-state pressure in two particularly contentious moments: when during the 1970s and early 1980s developing countries forcefully called for a more equal international economic order, and when in the 1990s the United States and other wealthy countries demanded international organizations adopt neoliberal economic reforms. In analyzing these two periods, Chorev demonstrates how strategic maneuvering made it possible for a vulnerable bureaucracy to preserve a relatively autonomous agenda, promote a consistent set of values, and protect its interests in the face of challenges from developing and developed countries alike.

With the help of new sources, including the personal diaries and private papers of the late Sir Marrack Goulding--an Under-Secretary-General from 1988 to 1997 and the second highest-ranking UN official during the genocide--the book situates the Rwanda operation within the context of bureaucratic and power-political friction existing at UN Headquarters in the early 1990s. The book shows how this confrontation led to a lack of coordination between key UN departments on issues as diverse as reconnaissance, intelligence, and crisis management. Yet Dangerous Diplomacy goes beyond these institutional pathologies and identifies the conceptual origins of the Rwanda failure in the gray area that separates peacebuilding and peacekeeping. The difficulty of separating these two UN functions explains why six decades after the birth of the UN, it has still not been possible to demarcate the precise roles of some key UN departments.

There is certainly no plethora of serious studies on inter national organization in general, and almost none dealing with the administrative aspects of such organization. More precisely, the author is not aware of a single comprehensive treatment of the finances of any international political organization, past or present. Over the years, many former Secretariat members of the League, the United Nations, and their affiliated agencies have come forth with either memoirs or general commentaries on their organizations. And frequently these works have included revealing, but brief, passages dealing with budg etary questions, yet none has dealt with these questions in any detailed or thorough fashion. It is unfortunate that this is so. Not that the fate of the world rides on the United Nations budget, or that matters of peace and war will be determined by the dollars and cents of the Secretary-General's estimates. Yet questions of real importance to many of the world's citizens are decided in the budgetary struggle. Until the policy decisions of the various organs are translated into budget items, there is no visiting mission to encourageTogoland's movement toward eventual self-govern ment, no cease-fire observer in the Middle East, no rehabilitation com mission in South Korea, and no public administration advisor in San tiago.
