Ces effets peuvent être périphérique : mydriase, bouche sèche, nausées, constipation, rétention urinaire, tachycardie ; et/ou centraux : confusion, désorientation, hallucinations, agitation, irritabilité, troubles mnésiques. La prise de médicament atropinique chez le patient âgé et notamment chez le patient atteint de la Maladie d'Alzheimer peut aggraver l'état du patient (hallucinations, troubles mnésiques), ou même déclencher une démence. De plus la co-prescription de médicaments atropiniques et de médicaments de la Maladie d'Alzheimer mène à une interaction médicamenteuse, en effet les mécanismes d'action de ces médicaments sont radicalement opposés. L'association de plusieurs médicaments atropiniques entraine l'accumulation des effets indésirables. Il existe différentes échelles permettant de mesurer la charge atropinique des médicaments, elles se basent sur des données biologiques ou bibliographiques. Ainsi il est possible de calculer la charge atropinique globale chez un patient en additionnant les poids de chacun de ses médicaments. Notre étude s'appuie sur deux échelles atropiniques, la liste ADS (Anticholinergic Drug Scale) et la liste de Duran qui a utilisé sept échelles existantes et les données bibliographique pour en établir une seule. Nous avons étudié les déclarations de la Base Régionale de Pharmacovigilance Midi-Pyrénées contenant un médicament de la Maladie d'Alzheimer entre 1999 et 2013 afin de quantifier la charge atropinique de chaque déclaration. Il ressort de cette étude que 59,37% des cas contiennent un médicament atropinique et que 18,3% des cas ont un poids atropinique supérieur ou égal à 3 selon la liste ADS. Selon la liste de Duran, 45,05% des cas contiennent un médicament atropinique et 11% ont un poids atropinique supérieur ou égal à 3, c'est-à-dire considéré comme élevé. Ainsi, notre étude, à son échelle, présente l'avantage de donner une image de la prescription de médicaments atropiniques chez le patient atteint de la Maladie d'Alzheimer. Il serait nécessaire que les prescripteurs soient informés des effets de ces médicaments atropiniques afin de limiter leur prescription et de rechercher une alternative thérapeutique.

Cette thèse se base sur les résultats d'une étude menée au sein du pôle gériatrie du CHU de Toulouse, dont l'objectif était de dresser un bilan des prescriptions de psychotropes les plus prescrits sont, par ordre d'importance : les anti-Alzheimer, les anxiolytiques, les antidépresseurs, les antipsychotiques et les hypnotiques. On constate dans la majorité des cas un effort de la part des praticiens pour prescrire la molécule ayant le meilleur rapport bénéfice/risque pour le patient. La prochaine étape souhaitable consiste maintenant à formaliser ces pratiques sous forme de protocoles, afin de garantir une prise en charge organisée et la moins délétère possible pour ces patients.
