Book excerpt: L'objectif de notre travail rétrospectif a été de mettre en évidence les profils évolutifs des critères orthophoniques et les facteurs d'influence des très bons résultats après implantation cochléaire. Nous avons étudié les profils évolutifs de la perception, de la production verbale, de l'intelligibilité, du mode de communication des enfants sourds congénitaux ou pré et péri-linguaux implantés dans notre service de 1993 à 1999 et répondant aux critères de meilleurs résultats sur un délai de 5 ans. D'autre part, nous avons étudié l'influence de facteurs socioprofessionnels et familiaux et les caractères de la surdité. Une comparaison des mêmes critères avec un groupe d'enfants aux résultats moyens a été réalisée. Il ressort de notre étude que la prise de repères linguistiques rapides dans un délai de deux ans post-implantation grâce à une très bonne perception est un facteur de bon pronostic d'excellente évolution. On observe la mise en place rapide du langage oral (au moins juxtapositions de mots) à deux ans est associée aux très bons scores de perception (>70% en liste ouverte) et d'intelligibilité (score > 3 au SIR). Nous avons également mis en évidence que la stimulation orthophonique intensive (environ 3 heures par semaine de rééducation individuelle) et précoce dès la période initiale et dans les deux premières années est un facteur important pour obtenir de très bons résultats, de même la scolarité en intégration et le mode de communication oral initial sont des facteurs qui influencent les bons résultats. Les autres facteurs doivent être discutés sur une plus grande population. En conclusion, l'évolution des critères orthophoniques à deux ans, le mode de communication initial tourné vers l'oral, le mode de scolarisation en milieu normalisé et le temps d'orthophonie passé dans les deux premières années après l'implantation cochléaire favorisent l'émergence des très bons résultats et semblent être des facteurs de développement linguistique précoce.

Book excerpt: Introduction : le développement de l'implant cochléaire a permis à une proportion croissante d'enfants sourds d'entrer dans le monde de la communication orale. Mais la grande variabilité des résultats reste une préoccupation importante. L'objectif de cette étude est d'évaluer l'impact de certains facteurs sociaux tels que l'implication familiale et le bilinguisme sur l'évolution des scores orthophoniques après implantation cochléaire chez l'enfant. Matériels et méthodes : il s'agit d'une étude rétrospective, monocentrique, sur 270 enfants ayant bénéficié d'une implantation cochléaire entre 1998 et 2014 et d'un suivi d'au minimum 4 ans. Les résultats post-opératoires étaient évalués annuellement à l'aide du score orthophonique APCEI. L'influence des différents facteurs pronostiques, et notamment de l'implication familiale, du bilinguisme et des troubles neuropsychologiques, a été analysée. Résultats : quatre ans après l'implantation, les enfants ayant une faible implication familiale et un bilinguisme ont un score d'intelligibilité moins bon que les enfants non impactés par ces facteurs sociaux. 19,1% des enfants avec faible implication familiale sont considérés comme mauvais répondeurs, contre 5,4% des enfants bénéficiant d'une forte implication parentale. 19,3% des enfants vivant dans un milieu bilingue sont considérés comme mauvais répondeurs, contre 6,6% des enfants monolingues. La présence de troubles neuropsychologiques associés est responsable de capacités linguistiques moindres. Les résultats suggèrent que les malformations de l'oreille interne et l'âge tardif d'implantation sont négativement associés aux scores d'intelligibilité quatre ans après l'implantation. Conclusion : l'évaluation des facteurs pronostiques est un élément clé pour la prise en charge des enfants implantés. Cette étude a confirmé l'impact des facteurs sociaux dans le développement des capacités linguistiques après implantation cochléaire. Comprendre ces causes de variation peut aider les professionnels de santé et les parents à créer les meilleures conditions possibles de réhabilitation après implantation. Pour améliorer les recherches ultérieures, il est nécessaire de développer des échelles sociales standardisées et d'évaluer l'impact non seulement sur les résultats linguistiques mais également sur la qualité de vie.
