Book excerpt: The purpose of the present study was to determine whether distinct patterns of adolescent adjustment existed when four domains of functioning were considered. The study included a sample of 299 high-risk urban adolescents, predominantly African American, ages 9-16 and their maternal caregivers. Cluster analysis was used to identify patterns of adjustment. Logistic regression analyses were used to explore whether variations in levels of five theoretically and empirically supported protective factors predicted cluster membership. A four-cluster model was determined to best fit the data. Higher rates of goal directedness and anger regulation coping predicted membership within the highest functioning cluster over a cluster demonstrating high externalizing problem behaviors, and neighborhood cohesion predicted highest functioning cluster membership over a cluster demonstrating high internalizing symptoms. Findings suggest that within a high-risk population of adolescents, significant variability in functioning will exist. The presence or absence of specific protective factors predicts developmental outcomes.

Book excerpt: Within the field of developmental psychopathology, research has repeatedly demonstrated that there are multiple complex and dynamic pathways originating in childhood that may lead to the development of internalizing and externalizing problems among adolescents. However, additional research is needed that examines the unique and concurrent contributions among child-, parent-, and family-level risk and resilience factors during childhood that may be associated with internalizing and externalizing problems in adolescence. To address this gap, the current study utilized a person-centered approach to identify profiles of risk and resilience factors among youth in middle childhood (ages 10-12) characterized by the quality and quantity of (a) child-level factors (i.e., temperamental features, executive functioning abilities); (b) parent-level factors (i.e., parental acceptance, control, disciplinary style); and (c) family-level factors (i.e., family cohesion, conflict, organization) among a sample of 775 participants (Aim 1). The study also examined internalizing and externalizing symptom profiles in adolescence (age 16) by identifying subgroups of youth characterized by the quality and quantity of internalizing and externalizing problems within each of the identified childhood risk profiles (Aim 2). Lastly, the study investigated transitions from childhood risk profiles to adolescent symptom profiles (Aim 3). Results demonstrated that a four-class model best fit the data in regard to childhood risk profiles, with classes of youth most saliently characterized by (a) accepting parents, (b) controlling parents, (c) disengaged parents, and (d) chaotic homes. With regard to adolescent internalizing and externalizing symptom profiles, results indicated a three-class model best fit the data and included classes distinguished by the presence of (a) low symptoms, (b) moderate symptoms, and (c) high internalizing and moderate externalizing symptoms. Most youth from the four childhood risk profiles transitioned to the low symptom profile at age 16; however, youth from the chaotic home profile were more likely to transition into one of the two higher-level symptom profiles. Findings enhance our understanding of risk and resilience by identifying distinct childhood risk profiles and corresponding adolescent symptom profiles. These findings will have implications for both prevention and treatment efforts that target specific risk factors within each risk profile.

Book excerpt: The bulk of resilience research has focused on developmental outcomes in a single domain of functioning (e.g., academic achievement), overlooking the complex interplay of positive and negative developmental outcomes that may co-occur within an individual (Luthar, Cicchetti, & Becker, 2000). Resilience may strengthen over time and across domains (e.g., psychological well-being, physical health), thus a corresponding variation in functioning across domains of development is also likely (Wright, Fopma-Loy, & Fischer, 2005). A multidimensional approach to resilience is necessary to account for this variation in functioning. Therefore, the current research trend of examining a single outcome and then ascribing a unidimensional 'resilient vs. not resilient' label without specifying a domain of resilience may be masking the co-occurrence of positive and negative outcomes of at-risk individuals. To address this gap, this dissertation presents two studies exploring resilience as a multidimensional construct. Both studies examined data from 323 students who participated in the Chicago cluster randomized trial of the Positive Action (PA) program (Ji, DuBois, Flay, & Brechling, 2008). The first study identified distinct subcategories of resilience profiles in an at-risk group of adolescents. Latent class analysis results indicate that, in contrast to the unidimensional--i.e., resilient or not resilient--construct of resilience, there are multiple profiles of resilience among at-risk students. Four distinct groups of students with markedly different resilience patterns were identified: Consistently Resilient (35.5%)--high on positive and low on negative outcomes, Consistently Non-resilient (18%)--low on positive and high on negative outcomes, Socio-emotional Resilience with Academic Issues (29.6%)--high on Social-emotional and Character Development (SECD) and positive health behaviors, low/moderate on negative outcomes, but reporting very poor academic performance, and Academically Resilient with Mental Health Issues (16.9%)--high on positive outcomes (SECD was moderate), low/moderate on violence and substance use, but very high on anxiety and depression. The second study used multinomial logistic regression to examine individual-, family-, and community-level risk and protective factors at grade 3 that predict distinct resilience patterns among children at grade 5. Analyses predicted resilient group membership, indicating students with high levels of empathy, parental and teacher reports of responsibility, parental encouragement, and students' feelings of safety at grade 3 were more likely to be resilient across multiple domains at grade 5. Together, these studies empirically support a more integrative, comprehensive, and multidimensional conceptualization of resilience and its predictors that is better-suited to identifying hidden strengths and vulnerabilities in children and adolescents.

To address these critical unmet needs in the field of HRQOL assessment, and to advance scientific methods and clinical applications in this field, a conference was held at Case Western Reserve University. The conference set out to summarize current information concerning the development and implementation of measures of HRQOL assessment, to identify and consider key conceptual and methodological issues in research concerning the measurement of HRQOL, and to recommend priorities to advance the state-of-the-art in research and clinical applications of QOL assessment in children and adolescents with chronic health conditions.
