Book excerpt: Morbidity and mortality from breast cancer can be reduced by early detection through screening. Despite recommended guidelines for breast screening since 1988, participation rates have been suboptimal. The objectives of the current study were to describe: (1) changes in breast cancer screening knowledge, attitudes, decisional conflict, intentions and practices among women aged 50-69 years since initiation of a regional mass screening program in Ottawa-Carleton in 1991; and (2) breast cancer screening knowledge attitudes, intentions, and practices among women aged 40-49 years compared to women aged 50-69 years. Among women aged 50-69 years, the percentage ever having had a mammogram increased from 60% in 1991 to 83% in 1994. There were commensurate increases in the percentage reporting mammography within two years from 47% to 74%. There was an insignificant improvement in the annual professional breast examination (PBE) rate from 57% to 59%. A small, but statistically significant increase occurred in monthly breast self-examination (BSE) rate from 46% to 54%. Women in their forties continue to overutilize screening mammography; 63% reported ever having had a mammogram and 44% reported having had a mammogram within the past two years. Reported annual PBE and monthly BSE rates of women aged 40-49 years were comparable to the rates of women aged 50-69 years; 63% versus 59% and 48% versus 54% respectively. Intentions to have mammography every two years once they are 50 years and to have annual PBE were similar to those of women 50-69 years, however, they were more likely to accept an invitation to screening (71% versus 56%). Women 40-49 years were more knowledgeable than women 50-69 years while they had similar concerns about future mammography as the women over 50 years. (Abstract shortened by UMI.).

Book excerpt: Southeast Asian women, including Cambodians and Thais, experience higher rates of breast and cervical cancer incidence than any other racial/ethnic group in the U.S. Incidence of cervical cancer among Cambodian and Thai women is among the highest in the U.S. and incidence rate of breast cancer for these two groups is rapidly increasing, while rates are decreasing for other racial/ethnic groups. Cambodians and Thai women also experience higher rates of late-stage breast cancer than non-Hispanic White women, resulting in increased morbidity and mortality from the disease. Despite these staggering statistics, little is known about barriers and facilitators of screening for Cambodian and Thai women, particularly women in Southern California, which is home to the largest population of both groups. This dissertation sought to better understand breast and cervical cancer screening behavior among Cambodian and Thai women in California. Using a mixed methods approach, this study has the following aims: 1) identify individual and environmental level predictors of breast and cervical cancer screening among Cambodian and Thai women in Northern and Southern California, 2) examine the association between behavioral intention and mammogram adherence among Cambodian and Thai women in Southern California, 3) explore ethnic-specific, culturally-based norms and systems-related factors that influence the decision-making process for Cambodian and Thai women in Southern California, and 4) examine the role of community health navigators in assisting Cambodian and Thai women to overcome individual and environmental barriers to mammogram screening in Southern California. Results from this study contribute to breast and cervical cancer literature and expands the existing literature on Cambodian and Thai women. By stratifying data analysis, the quantitative portion of this study identified community-specific individual, social, and environmental level predictors of breast and cervical cancer.

Book excerpt: Screening programmes involve the systematic offer of testing for populations or groups of apparently healthy people to identify individuals who may be at future risk of a particular medical condition or disease, with the aim of offering intervention to reduce their risk. For many years, screening was practised without debate, and without evidence, but in the 1960s serious challenges were raised about many of the screening procedures then being practised. Benefits and harms of screening must be measured in high quality trials, and the benefits of screening must be weighed alongside the negative side-effects. Concerns were raised about potential and actual harm arising when people without a health problem received dangerous and unnecessary investigations and treatments as a result of routine screening tests. Controversy raged, and it took some 50 years to achieve widespread recognition that evidence-based and quality assured programme delivery was essential, coupled with provision of balanced informed to enable informed choice for potential participants. Commercially motivated provision of poor quality and non-evidence based screening tests is increasing and screening remains a highly contested topic that has relevance in all health systems including for the general public and media. Book excerpt: Cancer can be a life-threatening disease, and early screening measures are the cornerstone for decreasing the chances of advanced-stage presentation of breast cancer and eventually reducing mortality rates. Adherence to annual or biennial mammography screening is crucial to diminishing disease prevalence and eliminating breast cancer complications through early detection of the disease. Yet the rate of breast cancer screening has remained extremely low in Saudi Arabia. A clear understanding of Saudi women's perceptions of breast cancer and mammogram are key to understanding intentions to seek mammography screening among Saudi women. Therefore, the aim of this study is to identify the associations among the predisposing factors, including those related to mammogram use, such as perceived self-efficacy for, benefits of, and barriers to mammogram screening, as well as factors related to perceptions of breast cancer such as perceived threat, fear, and cultural beliefs about breast cancer. The predisposing factors were compared with enabling factors such as education level, annual household income, and previous mammography experience and need factors, including perceived health status, family history of breast cancer, and physician recommendations for mammography. All together, the proposed factors (predisposing, enabling, and need) were related to the outcome of this study, which is intention to seek mammogram screening among Saudi women at the recommended age for breast cancer screening (age 40 to 69 years). Theoretical frameworks used an integration of two of the most accepted behavioral models is used: The Andersen health care utilization model (Andersen, 1968) and the health belief model (Hochbaum, 1958; Rosenstock, 1960; Becker, 1974; Kirscht,1974). Method: A quantitative, descriptive and cross-sectional design employing a convenience, volunteer sampling of 194 Saudi women at the age recommended for breast cancer screening. The results indicated that perceived self-efficacy to, and benefits of mammography were the only two significant predictors in the final model. Researchers, health policy makers and health care providers must consider these two factors in their future research and interventions in order to achieve higher uptake of mammography in Saudi Arabia.

When coupled with appropriate treatment, early detection can reduce breast cancer mortality. At the request of Congress, the Food and Drug Administration (FDA) commissioned a study to examine the current practice of mammography and breast cancer detection, with a focus on the FDA's oversight via the Mammography Quality Standards Act (MQSA), to identify areas in need of improvement. Enacted in 1993, MQSA provides a general framework for ensuring national quality standards in facilities performing screening mammography, requires that each mammography facility be accredited and certified, and mandates that facilities will undergo annual inspections. This book recommends strategies for achieving continued progress in assuring mammography quality, including changes to MQSA regulation, as well as approaches that do not fall within the purview of MQSA. Specifically, this book provides recommendations aimed at improving mammography interpretation; revising MQSA regulations, inspections, and enforcement; ensuring an adequate workforce for breast cancer screening and diagnosis; and improving breast imaging quality beyond mammography.
