Book excerpt: It is with great pleasure that I write this Foreword to the Proceedings of the International Conference on Behçet’s Disease which was held in Berlin in June 2002. This was the first International Conference held under the auspices of the International Society for Behçet’s Disease which was founded in 2000 in Seoul. First, I congratulate our colleagues in Berlin, led by Professor Christos Zouboulis of the Department of Dermatology at the Free University of Berlin, for having organised a most successful conference and for having compiled these proceedings so rapidly. It will be realised immediately on scanning the contents of this book that the conference was truly international with 210 participants from 26 countries, as Professor Zouboulis has noted in his preface. These included basic scientists, epidemiologists, pathologists, clinicians and, importantly, representatives from patient organisations. The latter held their own conference alongside the scientific-medical conference to mutual benefit. The combined session of patients and doctors (abstracts on pp 601 – 626) gave the opportunity for an exchange of information and fruitful discussion. The wide ranging scope of the communications is evident from the index and it was most encouraging to see their origin – from all parts of the world, from senior and junior colleagues and, from many different disciplines. Many communications may be regarded as preliminary reports of research in progress and we look forward to seeing the definitive publications in appropriate journals in due course.

Each year, thousands of women die needlessly from the consequences of these infections, including cervical cancer, ectopic pregnancy, acute and chronic infections of the uterus and the fallopian tubes, and puerperal infections. For many women, this happens because they receive medical attention too late, if at all. The terrible irony of this tragedy is that early diagnosis of and treatment for many RTis do not require high-technology health care. For the hundreds of millions of women with chronic RTis acquired from their sexual partners, life can become a living hell. Infection is a major cause of infertility, and it leads to scorn and rejection in many countries. These women may experience constant pain, have festering lesions of the genital tract, be at enhanced risk of second ary diseases, and endure social ostracism. The problems associated with RT!s have grown even greater in the past decade with the emergence of human immunodeficiency virus (HIV) and AIDS. Preexisting sexually transmitted disease, particularly when associated with genital tract ulcers, raises women's vulnerability to the transmission of HIV 3-5 fold.

Many become pregnant due to negligent care that deprives them of the right to make independent decisions about their health and lives. Others are forced to carry life-threatening pregnancies to term because medical providers refuse to provide abortions services that, in these circumstances, are legal. Some choose to seek alternative and at times highly unsafe abortions from unlicensed providers. Others forgo care entirely, and some even die. In 2008, according to Argentina's national health ministry, over 20 percent of deaths recorded due to obstetric emergencies were caused by unsafe abortions. Over the past 10 years, Argentina has accumulated an impressive artillery of reproductive and sexual health related policies. Though they ignore key constituencies such as women with disabilities, these policies would, if implemented, go a long way to overcoming the suffering documented in this report and elsewhere. But the laws and policies intended to benefit women and girls--such as the legal exceptions to the general criminalization of abortion--often go unimplemented. Moreover, the absence of oversight and accountability for this failure indicates that few in authority seem to care. For the state, the resulting and pressing public health concerns such as preventable maternal mortality, unsafe abortion, and unwanted pregnancies, are also costly.
