Currently, diagnosis is based primarily on the symptoms reported by the individual rather than on objective diagnostic markers, and there is little empirical evidence for the optimal degree and duration of physical rest needed to promote recovery or the best timing and approach for returning to full physical activity. Sports-Related Concussions in Youth: Improving the Science, Changing the Culture reviews the science of sports-related concussions in youth from elementary school through young adulthood, as well as in military personnel and their dependents. This report recommends actions that can be taken by a range of audiences - including research funding agencies, legislatures, state and school superintendents and athletic directors, military organizations, and equipment manufacturers, as well as youth who participate in sports and their parents - to improve what is known about concussions and to reduce their occurrence. According to Sports-Related Concussions in Youth, if the youth sports community can adopt the belief that concussions are serious injuries and emphasize care for players with concussions until they are fully recovered, then the culture in which these athletes perform and compete will become much safer. Improving understanding of the extent, causes, effects, and prevention of sports-related concussions is vitally important for the health and well-being of youth athletes. The findings and recommendations in this report set a direction for research to reach this goal.

All of us were impressed by the wealth of empirical data that was being generated by investigators interested in brain damage and recovery from both behavioral and biological orientations. Nevertheless, we were concerned about the relative paucity of attempts to evaluate the data provided by new technologies in more than a narrow context or to present new theories or reexamine time-honored ideas in the light of new findings. We recognized that science is guided by new technologies, by hard data, and by theories and ideas. Yet we were forced to conclude that, although investi gators were often anxious to publicize new methods and empirical fmdings, the same could not be said about broad hypotheses, underlying concepts, or in ferences and speculations that extended beyond the empirical data. Not only were many scientists not formally discussing the broad implications of their data, but, when stimulating ideas were presented, they were more likely to be heard in the halls or over a meal than in organized sessions at scientific meetings.

Nowhere has the surprise traditionally been greater than in the field of psychology. College students anticipate that their psychology courses will illuminate their personal problems and their friends' per sonalities; they are nonplussed to discover that the perception of geometric forms and the running ofT-mazes dominates the textbooks. The situation is comparable in the domain of linguistics. Nonprofessional observers assume that linguists study exotic languages, that when they choose to focus on their own language, they will examine the meanings of utterances and the uses to which language is put. Such onlookers are taken aback to learn that the learning of remote languages is a marginal activity for most linguists; they are equally amazed to discover that the lion's share of work in the discipline focuses on issues of syntax and phonol ogy, which are virtually invisible to the speaker of a language. Science moves in its own, often mysterious ways, and there are perfectly good reasons why experimental psychologists prefer to look at mazes rather than at madness, and why linguists study syntax rather than Sanskrit. Nonetheless, it is a happy event for all concerned when the interests of professionals and non specialists begin to move toward one another and a field of study comes to address the "big questions" as well as the experimentally most tractable ones. Discourse Ability and Brain Damage reflects this trend in scientific research.

Long term effects, and in particular neurodegenerative proteinopathy subsequent to TBI, remain theoretical, notwithstanding the poorly understood ‘punch drunk’ syndrome of the early and mid-20th century, and the 21st century concept of chronic traumatic encephalopathy. This book, the Handbook of Traumatic Brain Injury and Neurodegeneration, has as its theme the marriage between neurodegenerative disease and neurotrauma through TBI surrogates such as sport, military service, and experimental models, and the legitimacy of that marriage. In the 32 contributions included here, this handbook not only explores the deleterious effects of genuine TBI, but also, and more importantly, the relationship between TBI and neurodegeneration. Controversy notwithstanding, there is much to be learned about the biological effects of TBI, substrates for long-term sequelae, the relationship between TBI and diverse neuropsychiatric disorders, and targets for therapy. The overall message to the neuroscience community from these papers may be a cautionary tale.
