Book excerpt: On January 20, 2010, the Office of Inspector General initiated an investigation based on the review of a preliminary investigation conducted by the Ohio Bureau of Workers' Compensation ("BWC") Special Investigations Department. BWC identified evidence that Workers' Compensation Specialist Don P. Gooch of the Garfield Heights office was creating suspicious financial documents on state time using state equipment. Our investigation found that Gooch was bringing external storage devices to work, connecting them to his state computer, and accessing documents and images related to tax avoidance and debt elimination schemes. He created scores of documents and images and saved them on the external drives. One such document purported to be a $300 million bond that Gooch admitted to sending the United States Secretary of the Treasury. Gooch told us that he directed the Treasury to deposit the money into his "account" that he would use to eliminate his future personal debt. Gooch also created bogus money orders, one of which he sent to an attorney performing collections services for a credit card company. Additionally, we found that he used BWC printers to print out thousands of pages of records related to these schemes. Gooch spent hours of many of his state workdays engaged in these activities. Gooch, a registered notary public, used his notary status to attempt to legitimize documents that he created. When interviewed, Gooch attempted to minimize his conduct, but admitted to creating, modifying, and printing a variety of documents on state time. Gooch shared that he, and others for whom he created documents, believe that the federal government maintains an account for every U.S. citizen which can be accessed to discharge personal debt. A Federal Bureau of Investigation webpage titled, "Common Fraud Schemes," refers to this scheme as "Redemption," "Strawman," or "Acceptance for Value."

Book excerpt: In December 2008, the Ohio Inspector General's Office initiated an investigation to determine what, if any, corrective measures had been taken by the Ohio Bureau of Workers' Compensation ("BWC") as a result of their own May 2007 Internal Audit report which outlined various internal control weaknesses on the part of BWC management and its Pharmacy Benefit Manager vendor, ACS State Healthcare, LLC. After conducting a preliminary investigation, we learned that BWC had taken action to remedy most of the critical issues outlined in the report. However, we identified two areas of concern relating to the management of BWC's Pharmacy benefit program that warranted further investigation: BWC's failure to pursue available drug manufacturer rebates, and the agency's delay in terminating payments for three costly medications.
