Book excerpt: Adolescents must enter adult health care services at age 18, yet most are not ready and have not mastered daily self management of their type 1 diabetes. The purpose of this study was to determine the feasibility of a transition planning intervention, focused on educating adolescent/parent dyads about diabetes self management in preparation for the transition from pediatric to adult health care, using a newly developed transition plan and framework. The setting was a diabetes endocrinology clinic where 95% of young type 1 diabetics in the metropolitan area are seen. The intervention included supporting the parent in the relinquishing of diabetes management responsibilities to the adolescent. This study measured the transition readiness and self management practices of the adolescent as well as the parents' ability to promote autonomy in their adolescents' transition to self management. In addition, this study sought to gain information from the dyads on the usefulness of this intervention. The intervention consisted of four weekly sessions emphasizing a review of type 1 diabetes pathophysiology, daily living with type 1 diabetes, leaving the parental home, and reproductive health. A non-experimental, pre-post feasibility design was used and 11 adolescent/parent dyads consented to participate. Seven dyads completed the intervention. Using descriptive statistics, mean scores improved for transition readiness, diabetes care activities, problem solving, communication, and goals setting. Post intervention evaluations completed by participants were favorable. Most agreed or strongly agreed that the transition intervention was helpful in getting adolescents ready for transition and taking care of diabetes as a young adult. They would recommend this intervention to others. Based on the findings of this study and evidence found in the literature, the intervention will soon be packaged into brief 15-minute teaching sessions presented during quarterly clinic visits for all adolescents age 15-18 at the study site. If any adolescent remains in the clinic until age 19 or later, the intervention will continue quarterly until the adolescent transitions to adult health care. In addition, based on the responses given by the adolescent/parent dyads at completion of the sessions, this intervention contains useful information relevant to the adolescents' transition to adult health care.

Book excerpt: Background: Care transitions, defined as hospital discharge or movement from one healthcare setting to another, are currently a major concern for healthcare providers and policy makers alike. These care transitions are occurring in the context of increasingly fragmented care and have been known to result in hospital readmissions, adverse events, and medication errors. The adolescents' transition between pediatric and the adult care system is not only fragmented but occurs at a time when they are most vulnerable and ill equipped for change. Objectives: The purpose of this research was to conduct a feasibility study to examine if the use of a diabetes transition program, SMILE (Self-Management In Living Everyday), improves glycemic control in adolescents with type I diabetes mellitus (TIDM). The aims of the study were to examine 1) pre and post scores in diabetes knowledge, diabetes distress, and glycemic among a sample of adolescents with TIDM participating in the SMILE program, 2) relationships between diabetes knowledge, diabetes distress, and glycemic control among a sample of adolescents with TIDM participating in the SMILE program, and 3) the feasibility and acceptability of an individualized transition program utilizing technology, health care advocates, and home education visits among a sample of adolescents with TIDM participating in the SMILE program. Methods: A descriptive, correlational, repeated measures design was used for this feasibility study. Both quantitative and qualitative data were collected from a purposive sample of 14 adolescents recruited from patients who received care in the endocrine clinic at a large academic children's hospital located in Southern California. Data were collected during the spring of 2012 and participants' total time in the program was three months. Study measures included The Diabetes Knowledge Test (Fitzgerald, et al., 1998), Diabetes Distress Scale (DDS17), (Polonsky et al., 2005), Hemoglobin Al c, Frequency of Self Monitored Blood Glucose Testing (SMBG), and selected demographics. Statistical analysis included descriptive and inferential statistics. Results: A statistically significant difference was found in the pre mean = 12.57 (SD = 1.61) and post mean = 10.97 (SD=1.62) intervention HbAlc, t (13) = 3.94, p =.002; the pre mean, M = 72.4 (SD = 5.2) and post mean 87.5 (SD = 3.8) diabetes knowledge scores t (13) = - 6.47, p

Complications arising from inadequate follow up and treatment not only represent a disruption to care but also result in significant increases in the morbidity and mortality of such patients. One particular challenge faced by pediatric healthcare practitioners is the inevitable need to transfer care from the pediatric to the adult healthcare setting. Unsuccessful patient transfer results in the loss of appropriate care and an increase in the risk for adverse health outcomes. The main obstacles to the timely transfer of care are a lack of adequate patient preparedness for transfer of care and inadequate inter-physician networking/sharing of information in a culturally competent manner. Definition/Goals : Given these obstacles, this study stipulates that the provision of educational and networking resources to patients and adult care practitioners during the transition process will enable more effective transitioning from pediatric to adult care in T1DM patients. The main goal of this project is to create culturally appropriate transition tools and secondarily to identify and address obstacles to the process of educating patients and the establishment of networks among providers to the transitioning population. Methods/Materials : English and Spanish transition tools including patient and parent questionnaires, practitioner health summary tools, and transition brochure will be created. Materials will be reviewed by transition providers and clinic staff for effectiveness. Outcomes : These tools will then be made available to patients and practitioners through the Pediatric Endocrine clinic for immediate use in transitioning patients. The effectiveness of the transition tools will be assessed in follow up studies determining the percentage of transition teens scheduled and seen by their new providers and by feedback from both the transitioning population and their new providers.

This survey provided information on what registered nurses and dieticians perceived as barriers to successful transition from pediatric to adult care and whether a dedicated facilitator would improve the overall health and glycemic control of young adult diabetics. Factors that affect the successful transition, identified from the research, are reviewed in this research paper. Suggestions for implementing guidelines for transition and follow-up care by a nurse practitioner are indicated. Guidance in transition by a familiar nurse practitioner, liaising between the pediatric and adult practices, will aid in reducing "dropping out" of medical care, and the costly complications of Type 1 diabetes.

This is especially so for children and teenagers with type 1 diabetes. In addition to the general challenges experienced in adolescence, young people with type 1 diabetes are also taking increased responsibility for their diabetes management as well as transitioning into the adult health care system. This transition period is known to impact both parent and child, with both experiencing impaired emotional wellbeing including moderate to severe anxiety[1]. Providing support to both young people with diabetes and their parents to help them navigate this transition period is vital to ensure optimal diabetes management. The Diabetes MILES Youth Study 2014 survey provides a national snapshot of the emotional well-being, general health and psychosocial issues facing young people with type 1 or type 2 diabetes and their parents.This report found that parents of children with diabetes are at increased risk of anxiety and depression. Parents major concerns were around hypoglycaemia at night and that parents worried about their child developing complications and feeling like they were the u201cdiabetes policeu201d with their child.The transition from childhood to adolescence can be a challenging time for any child and their parents. This is especially so for children and teenagers with type 1 diabetes.It is recognised that children with diabetes often start to rebel against the diabetes management routine at this time. This could be for many reasons, including that they do not want to appear u2018differentu2019 from their friends. They may want to be more independent of their own lives and not have their parents so involved.Body image concerns may arise at this time and may impact on diabetes management. Diabetes may also contribute to self-confidence issues.During the transition period, the responsibility for diabetes management will move from the parents to the teenager. This responsibility may put pressure on the teenager, especially as they experience frustrations relating to unpredictable or changing blood glucose levels (BGLs), despite their best efforts. Anecdotal reports to Diabetes Queensland and information obtained through research demonstrates that parents are often concerned about their teenageru2019s health during this transition period, including relating to diabetes management, mental health. Parents often feel that they are being distanced and do not know how to approach their child to discuss their concerns.AimsThe One on One program aims to improve confidence among both young people with diabetes and their parents to navigate the transition into adulthood and help to establish supportive relationships both between parent and child as well as among peers. Increased awareness of diabetes and diabetes management for parents of type 1 children whilst transitioning into adulthood.The parents who attend the session benefit from the peer support offered to feel more connected and less isolated in their experiences relating to their child with diabetesThis program is designed to engage a Psychologist who facilitates discussion and provides parents with information and strategies to assist them and their child through this transition time. The program also provides a peer support opportunity for the type 1 child, allowing them to meet others in the same situation, to share experiences and to feel less isolated.MethodsYoung people with type 1 diabetes aged from 9 to 16, and their parents, attend a tailored education session delivered in collaboration with the local health care team who provide services to young people with type 1 diabetes. The session also includes presentations from local young adults with type 1 diabetes who share their experiences and insights in their own transition into adulthood with the condition. To allow for the development of peer relationships and the delivery of targeted information, parents and children are separated for part of the session with children partaking in engaging relationship-building activities and parents having an opportunity to discuss relevant issues with other parents in the presence of a psychologist and or an endocrinologist and a credentialled diabetes educator. Opportunity for open discussion of parents concerns, identifying similarities which assists with the feeling of community and support. Open discussion between the parents and the health team to look at strategies to assist with the transition period. The children whilst away from the parents are engaged in building a support network with other children of similar age who also have type 1 diabetes.Having the local health team involved is important as they hear the concerns of the parents and the strategies discussed so when going back to clinical practice they can assist in the implementation to support transition to adulthood [2].ResultsThe program has been successful in increasing confidence among both parents and young people in navigating the transition into adulthood with type 1 diabetes. The sessions have also facilitated on going peer support relationships and mechanisms both between young people and between parents. Positive feedback for the program with over 90% of respondents recommending this evening to other parents of a child with type 1 diabetesAn overwhelming majority of participants completing the evaluation were satisfied with the program they attended, with more than 95% of NDSS registrants overall agreeing the sessions they attended met their needs in terms of what they were hoping to learn or achieve.An overwhelming majority of participants completing the evaluation felt the program increased their awareness of the NDSS, with more than 89% of respondents overall agreeing with this statement. Attendees at events are asked to rate the speakers in terms of ease of understanding and usefulness of information. These are useful measures of whether the seminar was adequately aimed at the target group and whether it has been successful in meeting set objectives. Overall, for NDSS registrant sessions, more than 95% of responses indicated speakers were easy to understand and over 95% of responses that participants were provided with information they can use [3].Feedback from previous events as well as other Diabetes Queensland programs and research, indicates that parents of children with type 1 find it useful to hear from older adolescents or young adults about their diabetes experience through the teenage years. DiscussionSupport to both parents and young people navigating the transition into adulthood, with the added pressures of managing a complex condition like type 1 diabetes, is an important strategy to improve emotional wellbeing and diabetes management outcomes.
