Book excerpt: Angiogenesis is the growth of new blood vessels and is a key process which occurs during pathological disease progression. Excessive and damaging angiogenesis occurs in diseases such as cancer, diabetic retinopathies, age-related macular degeneration and atherosclerosis. In other diseases such as stroke and myocardial infarction, insufficient or improper angiogenesis results in tissue loss and ultimately higher morbidity and mortality. In this book we will begin by providing the reader with an overview of the process of angiogenesis including normal embryological development of blood vessels. The following chapters will each focus on a key angiogenic disease incorporating current scientific knowledge concerning the causes of activation of the “angiogenic switch”, pathological consequences, current treatment options and future perspectives. Where appropriate, results from pre-clinical trials, novel imaging modalities and nanotechnological approaches will be incorporated into these sections. Finally, since it is now believed that the process of angiogenesis operated via different signalling mechanisms in different vascular beds, we will discuss our current understanding of this phenomenon. The target audience for this book would include researchers in all the basic sciences; post-graduate students at Universities and Institutes; pharmaceutical industries; clinicians working in vascular biology or tissue imaging; pathologists; neurologists; tumour biologists; ophthalmologists and cardiologists.

Book excerpt: Over the past decade, rapid progress had been made in elucidating the cellular and molecular mechanisms of new blood vessel formation in both health and disease. Lack of sufficient angiogenesis (new capillary development) and collateral formation (remodeling of existing blood vessels) in adult tissues (e. g. , heart, brain, skeletal muscle) during criti cal ischemia has prompted novel therapeutic strategies to stimulate/fa cilitate angiogenesis and collateral formation in ischemic conditions ("therapeutic angiogenesis"). Therapeutic angiogenesis describes a strategy of inducing new col lateral vessels that can bypass vascular obstructions caused by atherosclerotic disease and stimulating new capillaries that can en hance tissue oxygen exchange. Naturally occurring growth factors (e. g. , FGFs, VEGFs) have been identified and demonstrated to induce new blood vessel formation in the adult organism. The therapeutic ap plication of these growth factors has been demonstrated to enhance these natural processes and have been shown in a number of animal models to increase blood flow and oxygen exchange to ischemic tis sues. Therapeutic angiogenesis therefore represents a new medical principle that is anticipated to be used as an adjunct or alternative to angioplasty and/or bypass surgery. A number of strategies have been proposed and tested to deliver growth factors. One strategy is the direct administration of the growth factor protein by intramuscular, intraarte rial or intravenous delivery.

Last but not least, two-photon intravital microscopy is presented as suitable tool to assess plaque angiogenesis in atherosclerotic lesions.

Stem cell therapy has emerged as a promising therapeutic strategy. Stem cells synthesize and secrete cytokines that promote cell recruitment, immunomodulation, extracellular matrix remodeling, angiogenesis, and neuroregeneration, all of which promote regeneration. Besides that, stem cells are also capable of differentiating in various cell types, being employed in tissue engineering. Preclinical and clinical investigations have reported efficacy of stem cell therapy for various vascular diseases. Even though results are encouraging, the studies demonstrate variation in stem cell type and origin, route and protocol for administration, and concomitant use of other treatment strategies, impairing easy interpretation of results and clinical application. The purpose of this book is to compile and comprise the current state of the evidence regarding stem cell therapy for each vascular disease, elucidating possible clinical applications. More than an objective guide for readers on the use of this novel treatment strategy, this publication will advocate for stem cell therapy use and development and will be of significant interest to physicians in a wide range of disciplines as well as researchers.

Therefore, it is frequently referred to in relation to therapeutic angiogenesis. Despite the big clinical potential and the many promising clinical trials on arteriogenesis and therapeutic angiogenesis, the exact molecular mechanisms involved in the multifactorial processes of arteriogenesis are still not completely understood. A better understanding is needed in order to define successful clinical therapies. In this Special Issue, multiple aspects of arteriogenesis and therapeutic angiogenesis will be addressed, ranging from the role of inflammatory processes and immune cells, to growth factors, microRNAs and environmental factors like hypoxia. Therapeutic angiogenesis will also be discussed in relation to the atherosclerosis and intraplaque angiogenesis in hypoxic lesions, as well as specific forms of arteriogenesis in relation to spinal cord blood supply and aorta surgery. The effects of exercise, a frequently prescribed therapy for PAD patients, on arteriogenesis are also discussed. PAD is typically a disease of older individuals, and the incidence of PAD continues to rise due to an increase in cardiometabolic disease and an aging population. Importantly, “all cause” and cardiovascular morbidity and mortality are significantly increased in PAD patients. PAD diagnosis remains a significant challenge, as a large number of patients are asymptomatic. Moreover, PAD results in a significant financial and societal burden with underutilized diagnostics and limited effective therapies. Here we discuss PAD signs and symptoms, pathophysiological mechanisms, current management, and future disease targets and possible therapeutic treatments for PAD.

The field of angiogenesis has grown enormously in the past 30 years, with only 40 papers published in 1980 and nearly 6000 in 2010. Why has there been this explosive growth in angiogenesis research? Angiogenic therapies provide a potential to conquer cancer, heart diseases, and more than 70 of life's most threatening medical conditions. The lives of at least 1 billion people worldwide could be improved with angiogenic therapy, according to the Angiogenesis Foundation. In this little book, we provide a simple approach to understand the essential elements of the angiogenic process, we critique the most powerful angiogenesis assays that are used to discover proangiogenic and antiangiogenic substances, and we provide an in-depth physiological perspective on how angiogenesis is regulated in normal, healthy tissues of the human body. All tissues of the body require a continuous supply of oxygen to burn metabolic substrates that are needed for energy. Oxygen is conducted to these tissues by blood capillaries: more capillaries can improve tissue oxygenation and thus enhance energy production; fewer capillaries can lead to hypoxia and even anoxia in the tissues. This means that angiogenic therapies designed to control the growth and regression of blood capillaries can be used to improve the survival of poorly perfused tissues that are essential to the body (heart, brain, skeletal muscle, etc.) and to rid the body of unwanted tissues (tumors).
