Hence, the significance of effectively preparing senior nursing students (SNSs) through the development of clinical reasoning, clinical judgement and critical thinking skills in order to face the challenges of their new career. Therefore, the aim of this evidenced-based practice (EBP) change project was to build stronger critical thinking skills in SNSs; thereby, effectively preparing them for transition from college into the health care system. The population consisted of SNSs in a rigorous 24-month rigorous Associate Degree Nursing (ADN) program in an urban Pacific Northwest community. Nine SNSs participated in this EBP change project. This practice change implemented scenario based simulation centered learning experience (SCSBLE) followed with debriefing for meaningful learning (DML) to increase critical thinking skills in SNSs. The results of these learning experiences were very positive, with an increase from pre-testing to post-testing scores by 7.22% demonstrating an increase in critical thinking skills in addition to students verbally expressing better assessment skills post SCSBLE/DML. Concluding, through SCSBLE/DML experiences with SNSs critical thinking skills and confidence were built. Subsequent implementations must be administered with larger numbers of participants to solidify the evidence. Available in PDF, EPUB and Kindle. Book excerpt: Background: In rural healthcare, to adequately staff intensive care units (ICU) often new graduate nurses are hired directly into the ICU setting. Factors contributing to this hiring practice include the retirement of experienced nurses, and movement of potential staff to larger cities, both of which result in a limited number of experienced ICU nurses in the geographical area. This employment process places new graduate nurses into positions that require mature critical thinking skills. Typically new graduates have fundemental skills due to limited exposure to advanced protocols. EBP Framework: Utilizing Kurt Lewin's Model, the project established a means to introduce a new protocol to the existing curriculum. The Star Model of Evidence-Based Practice guided the evidence gathering and the steps of implementation of the new protocol. The model's ease of use fostered the adoption of evidence-based protocols into an established Associate Degree Nursing program. This project has become the protype for further curricula changes to incorportate additional evidence-based protocols that promote critical thinking techniques for students in clinical training. Methods: In attempting to enhance the critical thinking skills of the student nurses, this project introduced a new advanced protocol into the clinical curriculum of an Associate Degree Nursing program (ADN). Clinical training using the evidence-based practice (EBP) ventilator associated pneumonia (VAP) protocol exposed the students to the assessment and technical skillset associated with performing the VAP prevention protocol in an ICU setting. This process was designed to increase the students' confidence and critical thinking abilities. Additionally, use of the protocol provided a formal educational structure employed between the staff nurses and the student nurses. This interaction enhanced the experienced nurses' communication and mentorship of the students. The goals of the project were: to enhance the students' skillset on the Centers for Disease Control (CDC) protocol for VAP prevention; to allow the new teaching initiative into the school's future curriculum; and to enhance future students' knowledge base on the CDC VAP bundle. Formal adoption of an evidence-based protocol by the ADN program's curriculum commitee served as the project's outcome measure. Finding/Results: Based upon the performance on the task checklist, the students demonstrated their ability to transfer the academic training into executable steps as verified by their receptors. The students' performance revealed the evidence-based VAP protocol teachings were effective and the protocol was successfully submitted to the curriculum committee for adoption into the ADN program's clinical curriculum with the support of the College of Nursing's Director. Furthermore, the ADN faculty plan to use more evidence-based protocols to update and improvement the school's clinical curriculum.

Book excerpt: This study explicated critical thinking teaching strategies being utilized by California Associate Degree Nursing (ADN) faculty, and their perceived effectiveness, to serve as a model for faculties in nursing and other disciplines. Questions addressed by the study included not only teaching strategies in use and their perceived effectiveness, but also exploration of common threads among nursing educators in their definitions of critical thinking, and common themes of "especially effective" techniques. The study involved a pilot of the survey instrument, utilizing one sample school's nursing faculty, with subsequent distribution to all 69 other ADN program senior nursing course lead faculty. The instrument included demographic school and personal faculty data; closed-form, Likert-scale questions on critical thinking teaching strategies and their perceived effectiveness; and an open-form question requesting a narrative description of an "especially effective" teaching strategy that had been implemented. Respondents were also asked to articulate a definition of critical thinking. A 70% return rate was obtained, utilizing follow-up letters and phone calls. The typical respondent was female, 41 to 50 years of age, with 11 to 20 years of experience in teaching nursing. Study results showed that a common definition of critical thinking did not exist among sample respondents, although five major threads could be identified. Respondents defined the concept of critical thinking through one or more of the following lenses: (1) problem-solving (nursing process); (2) Bloom's taxonomy of higher-order thinking skills; (3) reasoning (or informal logic); (4) reflection, imagining alternatives, challenging beliefs, and creativity; and (5) thinking about thinking, interpreting meaning, and metacognition. Critical thinking teaching strategies used most often, and perceived as very effective by faculty, included brainstorming, use of case studies, class or small group discussion, inductive reasoning techniques, scenarios or role play, written reports, self-study exercises, and teacher role modeling. Narrative descriptions from respondents' teaching experience fell generally into the categories of case studies, class or small group discussion, scenarios or role play, written reports, inductive reasoning techniques, brainstorming, and panel discussions. The dominant theme that emerged from the data was the use of case studies. Study results display 22 exemplars of actual teaching strategies that have been implemented by nurse educator respondents, including their commentaries. Discussion and implications of research findings and recommendations for further study are provided.
