Although therefore the rules laid down will generally sufice, yet you will occasionally meet with cases in which the accustomed signs are absent, or in which unusual symptoms are present. For instance, no complaint has more strongly marked signs than peritonitis the excessive and general pain of the abdomen, the great tenderness on pressure, the rapid, wiry pulse - and yet you may meet with fatal peritonitis with scarcely any pain, or with out tenderness, or with a pulse not above the normal standard. In order to obtain the necessary Skill in diagnosis, it will be requisite that you should practise the taking Of cases. You should record the symptoms and phy sical signs present in each case, the order in which the symptoms have been developed, the treatment adopted, the progress of the disease, and, if it terminate fatally, you should add the morbid appearances discovered after death. You will readily understand that unless some plan is adopted, there is great probability that you will either encumber your description Of the dis ease with a number Of unnecessary details, or overlook important facts. I have therefore added the following suggestions for a plan, which you may perhaps find useful until experience enables you to propose a better. Commence with the name and address of your patient, his age, and occupation. - The age is important, because many diseases, such as cancer, are chiefly found at cer tain periods of life. The nature of the occupation Often gives a clue to the complaint; as, for instance, painters and other workers in lead are especially liable to colic and paralysis. It is not intended as a treatise upon medical diagnosis. On that subject there is already a sufficiency of good works in existence. It aims rather at describing those methods of clinical investigation by the proper application of which a correct diagnosis can alone be arrived at. To every student when he first begins work in a medical ward the question presents itself: How shall I investigate this case? To that question the present work is intended to provide an answer. The first chapter deals, therefore, with the methods of case-taking in general, and includes a general scheme for the investigation of medical cases. The rest of the book is really an expansion of that scheme, each system being taken up separately, and the methods of investigating it described in detail. A special chapter has been devoted to the clinical methods of examining children, as these differ in many respects from those employed in the case of adults. He may adopt the method in vogue for many years, that of describing the clinical pictures of the various diseases, presenting details as to their etiology, symptomatology, diagnosis, prognosis and treatment. Or, he may approach the subject from the Opposite standpoint, describing the various symptoms which go to demonstrate that a person is ill, and give details as to their clinical relations and diagnostic significance. Inasmuch as the latter plan follows the lines of argument one employs when exam ining a case, it may be called the clinical method. The old or established system of teaching medicine has its advantages otherwise it would not have preserved its popularity even unto the present time. But it has its disadvantages, which become more and more obvious as we have forced upon us the truth of the axiom, We must treat the patient, not his disease. To label a given case of illness with the name of one or the other diseases described in the standard works on medicine is not making a diagnosis, because the standard clinical types as described in such works are the exception, and not the rule. We must, therefore, be able to proceed still further in our conclusion, and obtain an adequate conception of all the pathological changes taking place in the patient in whose case we happen to be interested, even though we are unable to pre sent our conclusion in the shape of some set title. All of these associated changes cannot be studied practically according to the old system of ex pounding the principles of medical practice. Approaching the subject from the reverse side, - the clinical, - the study of cases becomes interesting and easier. The problem is solved, not by matching the case in hand with the type as described in a standard text book, but by a process of reasoning. This method of study is, in reality, the only one possible in unravelling the mysteries of the majority of cases of chronic disease. The plan of the work, and the arrangement of diseases, have been made with special reference to Clinical Medicine. In the Introduction are considered topics of importance as bearing on medical study and practice. Conforming to the nosological method now generally adopted, diseases are divided into General and Local, and the latter are dis tributed into classes corresponding with the different physiological sys tems, namely, the Respiratory, Circulatory, Digestive, Urinary, and Nervous. Each of the six sections is prefaced by preliminary observa tions relating to Symptomatology, and other topics of practical importance as bearing on the clinical study and treatment of the diseases considered in that section. As far as practicable, diseases are arranged by grouping together those of which the diagnosis involves differentiation from each other. Regarded from a clinical standpoint, the advantage of this arrangement is more than compensatory for any apparent inconvenience in making hasty reference to particular diseases. Such reference will be found sufficiently facilitated by the conspicuous headings throughout the text, combined with an analytical table of contents and a full index. Of the several sections, the one devoted to Diseases of the Nervous System occupies the largest space.
