Book excerpt: Twenty years of mortality follow-up were used to study the effect of physical activity on cardiovascular disease (CVD) mortality in white male US Railroad workers (n = 3043). Caloric intake, leisure time activity (LTA), and working pulse rate were used as indices of physical activity. The interrelationships among these physical activity indices, other measures of physical fitness, and the CVD risk factors of obesity, serum cholesterol, blood pressure, and smoking were studied. These physical activity indices were significantly related to each other, to physical fitness variables, and to CVD risk factors. Caloric intake and LTA were significantly inversely related to CVD mortality after adjusting for age. Point estimates of risk were 1.3 and 1.4 respectively. Logistic regression analyses in the CVD free (n = 2578) showed that caloric intake was attenuated by SBP, however serum cholesterol and smoking improved the caloric intake/CVD mortality relationship. LTA remained a significant independent predictor of CVD with age, SBP, serum cholesterol, and smoking in the model. The age adjusted risk estimate of working pulse rate on CVD mortality was 1.5. It remained significantly related to CVD deaths with age, serum cholesterol, and smoking in the model, however, addition of SBP to the model resulted in working pulse rate no longer being an independent predictor of CVD mortality. In general, it may be concluded that these results support the hypothesis that physical activity protects against CVD mortality. Furthermore, physical activity appears to be directly as well as indirectly related to CVD mortality.

A potential limitation of available risk calculators are they generally estimate risk based on traditional risk factors, which is problematic since cardiorespiratory fitness (CRF)/physical activity are independent predictors of CVD. To address the limitations, Wickeramasinghe et al. developed a calculator to assess 30-year CVD risk that includes CRF as a variable, along with traditional risk factors, but few studies have evaluated the impact of aerobic training on this risk score. The purpose of the present study was to evaluate the effect of aerobic exercise training on 30-year CVD risk. METHODS: The study included adults with elevated CRP levels who were randomized to an aerobic exercise group or a control group. Baseline and follow-up data (age, total cholesterol, systolic blood pressure, diabetes, body mass index, fitness level, and smoking status) were entered into the Wickeramasinghe et al. [1] calculator to calculate 30-year CVD mortality risk. The aerobic exercise intervention lasted 4 months and individuals expended approximately 16 kcal/kg per week (KKW). The control group was asked to maintain their sedentary status over the course of the intervention. RESULTS: A significant reduction in 30- year CVD risk mortality was observed between the exercise and the control group following the intervention (-0.5[CI: -1.3,0.4] versus -2.0[CI: -2.9, -1.1]. Furthermore, CT and reduced physical activity (PA) contribute to a sustained reduction in cardiopulmonary fitness (CPF), an independent predictor of CV mortality. Recent studies have explored the potential of aerobic exercise as a cardioprotective strategy, and many have reported CPF as an outcome. The main purpose of this review was to synthesize evidence from the existing childhood cancer survivor (CCS) studies examining the effect of aerobic exercise on CPF. The specific objectives were to investigate how certain clinical characteristics (e.g., treatment status) and exercise parameters (e.g., program length) change the effect of exercise on CPF. Pooled evidence from the nine included studies suggests that aerobic exercise has a statistically and clinically significant positive effect on CPF. Findings on the moderating effect of clinical characteristics and exercise parameters were insignificant.
