Book excerpt: Excerpt from The Medical Record, Vol. 5: A Semi-Monthly Journal of Medicine and Surgery; March 1, 1870-January 15, 1871 The disease has found its victims chiefly among the destitute. It has been called the famine fever. Dr. Bigbie concurs with Dr. Murchison in the opinion that rela sing fever is the result of destitution, while typhus is produced by destitution and over-crowding combined, and thinks that it will stand the test of further observation and renewed careful investigation. Dr. Murchison found, on examining the records of the London Fever Hospital, that from 1847 to the time of search, 430, or per cent. Of all the relapsing fever patients were pers, whose expenses were paid by parishes. A arge proportion of them for some time previous to their attack, had been literally starving. In a special treatise the matter must be thoroughly gone into from the bottom upward, as many readers will approach the subject in simple ignorance of it. But here it is assumed that the reader already possesses a fair text-book acquaintance with the heart and its various affections. With this ex planation no mistake ought to occur. This book was released on 1997-10-28 with total page 257 pages. Available in PDF, EPUB and Kindle. Book excerpt: Most industries have plunged into data automation, but health care organizations have lagged in moving patients' medical records from paper to computers. In its first edition, this book presented a blueprint for introducing the computer-based patient record (CPR). The revised edition adds new information to the original book. One section describes recent developments, including the creation of a computer-based patient record institute. An international chapter highlights what is new in this still-emerging technology. An expert committee explores the potential of machine-readable CPRs to improve diagnostic and care decisions, provide a database for policymaking, and much more, addressing these key questions: Who uses patient records? What technology is available and what further research is necessary to meet users' needs? What should government, medical organizations, and others do to make the transition to CPRs? The volume also explores such issues as privacy and confidentiality, costs, the need for training, legal barriers to CPRs, and other key topics.

Book excerpt: Excerpt from The Medical Record, Vol. 13: A Weekly Journal of Medicine and Surgery; January 5, 1878-June 29, 1878 With these facts before us, we shall understand that the indications of treatment differ considerably in the case of the adult and of the child. Surgical writers have not sufficiently considered these differences, but they have generally said, or left it to be inferred, that the same plan of treatment was applicable to both, and that the prognosis did not differ essentially. I do not think so. The indications are, in fact. In some sense reversed. For while in adults the first and most difficult indication is to overcome the Shortening occasioned by the obliquity of the fracture and the powerful action of the fully developed muscles, and the second is to keep the limb in line - here, in the case of children, the first and most difficult indication is to keep the limb in line, and the second is to over come the action of the muscles, or this second indica tion may not be present at all. Let us look at the usual modes of treatment of broken thighs in children, and see whether they are suited to these changed conditions. Book excerpt: How the hidden trade in our sensitive medical information became a multibillion-dollar business, but has done little to improve our health-care outcomes Hidden to consumers, patient medical data has become a multibillion-dollar worldwide trade industry between our health-care providers, drug companies, and a complex web of middlemen. This great medical-data bazaar sells copies of the prescription you recently filled, your hospital records, insurance claims, blood-test results, and more, stripped of your name but possibly with identifiers such as year of birth, gender, and doctor. As computing grows ever more sophisticated, patient dossiers become increasingly vulnerable to reidentification and the possibility of being targeted by identity thieves or hackers. Paradoxically, comprehensive electronic files for patient treatment—the reason medical data exists in the first place—remain an elusive goal. Even today, patients or their doctors rarely have easy access to comprehensive records that could improve care. In the evolution of medical data, the instinct for profit has outstripped patient needs. This book tells the human, behind-the-scenes story of how such a system evolved internationally. It begins with New York advertising man Ludwig Wolfgang Frohlich, who founded IMS Health, the world’s dominant health-data miner, in the 1950s. IMS Health now gathers patient medical data from more than 45 billion transactions annually from 780,000 data feeds in more than 100 countries. Our Bodies, Our Data uncovers some of Frohlich’s hidden past and follows the story of what happened in the following decades. This is both a story about medicine and medical practice, and about big business and maximizing profits, and the places these meet, places most patients would like to believe are off-limits. Our Bodies, Our Data seeks to spark debate on how we can best balance the promise big data offers to advance medicine and improve lives while preserving the rights and interests of every patient. We, the public, deserve a say in this discussion. After all, it’s our data.

Book excerpt: Excerpt from Medical Record, Vol. 93: A Weekly Journal of Medicine and Surgery; January 5, 1918-June 29, 1918 In order to diagnose these cases correctly it is. Necessary for the physician to see the man as a whole, not alone his disturbed physical functions. The factor primarily important in the treatment is to regulate not so much what the patient shall eat or drink, but what he thinks. The whole man must be treated, his whole personality worked upon, and when the cause of the mental disorder has been removed the physical disturbances will of themselves disappear. Want of time forbids me even to mention the many and varied forms of stomach and intestinal disorders directly due to mental and emotional disturbances. Book excerpt: Excerpt from Medical Record, Vol. 71: A Weekly Journal of Medicine and Surgery; January 5, 1907-June 29, 1907 Case IX. - While the following appended case of pneumonia did not occur in the neighborhood of Messer, it may have some bearing upon the ques tion of susceptibility of certain families in their relation to pneumonia. The patient was the son of the patient in Case VIII., the nephew of Cases No. IV. And VI., and a cousin of Case V. Or in other words, of this series of nine cases of pneu monia, four occurred in one family. This patient's aunt and cousin had died of pneumonia during this epidemic of grippe. The patient was a boy, aged three years, who lived with his father, twelve miles distant from Messer. The child had not been away from home. And had not been exposed to any case of croupous pneumonia. For a few days previous to March 29 he had suffered from a cough and cold. The patient lived in the same room with his father, who developed pneumonia on March 28. The patient developed croupous pneumonia on the fol lowing day, or one dav later than the father. The lower lobe of the right lung was consolidated, and recovery occurred at the end of two or three weeks.
