Available in PDF, EPUB and Kindle. Book excerpt: Tuberculosis (TB) infects over 9 million people annually and is responsible for approximately 1.5 million deaths each year. Despite high treatment success rates, declines in incidence have averaged only 1.5% per year over the past decade. The newly adopted End TB Strategy sets a progressive agenda that moves beyond the medical sphere and includes a focus on upstream social determinants aimed at prevention. The strategy specifically calls for stronger poverty alleviation and social protection programs and policies. This dissertation sets out to support the need for social protection programs to combat TB, as well as attempting to answer the questions of who should be the recipients of such programs and in what form should they take. Paper one uses country-level data to show that spending on social protection is associated with decreased TB prevalence, incidence, and mortality. This work has a global purview and predicts a drop of 18 per 100,000 persons in TB prevalence rate from 1% increase in social protection spending. This is true even after adjusting for factors associated with TB rates such as level of economic development, the strength of the health system, and HIV burden. The second paper examines the relationship between household poverty and TB disease. Using household asset and characteristic data, this work creates household socioeconomic quintiles and attempts to show the relationship between this measure and individual active TB disease. The work utilized eight national TB prevalence surveys; large household surveys with a rigorous diagnostic TB algorithm. Although this approach found lower risk of TB disease for individuals in the poorest quintiles in four countries, a dose-response relationship was not observed. This paper also created an absolute wealth estimate, a US dollar-based measure of household wealth, which allowed for comparability across settings and also pooled country models. This measure of household socio-economic level did not have a clear association with individual TB risk. This work suggests novel ways of assessing the relationship between poverty and TB at the individual level that have the potential to be more efficient and to further the field of the social determinants of TB. The final paper of this dissertation focuses on a TB patient cost survey in Myanmar. This nationally representative survey includes 966 TB patients across the country and gathers information about their income and costs while seeking care. This survey is the first step in measuring the percentage of TB-affected households experiencing catastrophic costs due to the disease, defined as costs exceeding 20% of annual household income. This metric is one of the high level indicators in the End TB Strategy and will be measured by the World Health Organization and its partners in the majority of high burden countries in the coming years. The Myanmar survey found 65% of households experiencing catastrophic costs due to TB, with major cost drivers being patient's time and additional food and/or nutritional supplements required because of the disease. Together these three papers support the need for social protection, alongside appropriate and timely medical care, in order to reach the ambitious targets set forth by the End TB Strategy.

These are the region ́s chronically poor, who have remained so despite unprecedented inroads against poverty in Latin America and the Caribbean since the turn of the century. Left Behind: Chronic Poverty in Latin America and the Caribbean takes a closer look at the region’s entrenched poor, who and where they are, and how existing policies need to change in order to effectively assist them. The book shows significant variations of rates of chronic poverty both across and within countries. Within a single country, some regions show incidence rates up to eight times higher than the lowest. Despite the higher rates of chronic poverty in rural areas, chronic poverty is as much an urban as a rural issue. In fact, considering absolute numbers, urban areas in many countries, including Chile, Brazil, Mexico, Colombia and the Dominican Republic, have more chronic poor than rural areas. Undoubtedly the region has come a long way during the decade in terms of poverty reduction, guided by a mix of sustained growth and increased levels in amounts and quality of public spending and programs targeted directly or indirectly to the chronic poor. While improving endowments and the context where the chronic poor live is a necessary condition going forward, the decade’s experience suggests that it may not be enough to reach the chronic poor. The book posits that refinements to the existing policy toolkit †“ as opposed to more programs †“ may come a long way in helping the remaining poor. These refinements include intensifying efforts to improve coordination between different social and economic programs, which can boost the income generation process and deal with the intergenerational transmission of chronic poverty by investing in early childhood development. Equally important though, there is an urgent need to adapt programs to directly address the psychological toll of chronic poverty on people’s mindset and aspirations, which currently undermines the effectiveness of the existing policy efforts.

The proportion of households living below the official poverty line has declined slowly and unevenly in the past four decades, and poverty reduction has been much slower than in neighboring countries such as the People's Republic of China, Indonesia, Thailand, and Viet Nam. Economic growth has gone through boom and bust cycles, and recent episodes of moderate economic expansion have had limited impact on the poor. Great inequality across income brackets, regions, and sectors, as well as unmanaged population growth, are considered some of the key factors constraining poverty reduction efforts. This publication analyzes the causes of poverty and recommends ways to accelerate poverty reduction and achieve more inclusive growth. it also provides an overview of current government responses, strategies, and achievements in the fight against poverty and identifies and prioritizes future needs and interventions. The analysis is based on current literature and the latest available data, including the 2006 Family Income and Expenditure Survey.

Part of the agenda in recent years to tackle poverty in Africa has been the launching of social safety nets programs. All countries have now deployed safety net interventions as part of their core development programs. The number of programs has skyrocketed since the mid-2000s though many programs remain limited in size. This shift in social policy reflects the progressive evolution in the understanding of the role that social safety nets can play in the fight against poverty and vulnerability, and more generally in the human capital and growth agenda. Evidence on their impacts on equity, resilience, and opportunity is growing, and makes a foundational case for investments in safety nets as a major component of national development plans. For this potential to be realized, however, safety net programs need to be significantly scaled-up. Such scaling up will involve a series of technical considerations to identify the parameters, tools, and processes that can deliver maximum benefits to the poor and vulnerable. However, in addition to technical considerations, and at least as importantly, this report argues that a series of decisive shifts need to occur in three other critical spheres: political, institutional, and fiscal. First, the political processes that shape the extent and nature of social policy need to be recognized, by stimulating political appetite for safety nets, choosing politically smart parameters, and harnessing the political impacts of safety nets to promote their sustainability. Second, the anchoring of safety net programs in institutional arrangements †“ related to the overarching policy framework for safety nets, the functions of policy and coordination, as well as program management and implementation †“ is particularly important as programs expand and are increasingly implemented through national channels. And third, in most countries, the level and predictability of resources devoted to the sector needs to increase for safety nets to reach the desired scale, through increased efficiency, increased volumes and new sources of financing, and greater ability to effectively respond to shocks. This report highlights the implications which political, institutional, and fiscal aspects have for the choice and design of programs. Fundamentally, it argues that these considerations are critical to ensure the successful scaling-up of social safety nets in Africa, and that ignoring them could lead to technically-sound, but practically impossible, choices and designs.

Book excerpt: There have been calls to revisit the experiences of TB screening campaigns that were widely applied in Europe and North America in the mid-20th century, as well as more recent experiences with TB screening in countries with a high burden of the disease, and to assess their possible relevance for TB care and prevention in the 21st century. In response, WHO has developed guidelines on screening for active TB. An extensive review of the evidence has been undertaken. The review suggests that screening, if done in the right way and targeting the right people, may reduce suffering and death, but the review also highlights several reasons to be cautious. As discussed in detail in this book, there is a need to balance potential benefits against the risks and costs of screening; this conclusion is mirrored by the history of TB screening. This publication presents the first comprehensive assessment by WHO of the appropriateness of screening for active TB since the recommendations made in 1974 by the Expert Committee. However, the relative effectiveness and cost effectiveness of screening remain uncertain, a point that is underscored by the systematic reviews presented in this guideline. Evidence suggests that some risk groups should always be screened, whereas the prioritization of other risk groups as well as the choice of screening approach depend on the epidemiology, the health-system context, and the resources available. This book sets out basic principles for prioritizing risk groups and choosing a screening approach; it also emphasizes the importance of assessing the epidemiological situation, adapting approaches to local situations, integrating TB screening into other health-promotion activities, minimizing the risk of harm to individuals, and engaging in continual monitoring and evaluation. It calls for more and better research to assess the impact of screening and to develop and evaluate new screening tests and approaches.

In addition, health professionals - including doctors, nurses, and educators working both in government services and nongovernmental organizations, such as technical agencies that are treating patients and organizing treatment services - will find these guidelines to be useful.
