This review summarizes HSPA domains and indicators used by Member States in their HSPA or health system-related reports. Thirty Member States published in the English language and from their latest documents, 1485 distinct indicators were extracted. The number of indicators reported per Member State ranged from 9 to 146, with a mean of 50. Among the 14 domains of the WHO 2007 framework, service delivery and improved health were covered by virtually all Member States analysed (30 and 29, respectively), but coverage varied for the other 12 domains, with health workforce and financing having good coverage (25 and 26, respectively) but others, such as safety, efficiency, coverage or responsiveness, covered in only 20-30% of documents. Further refinement of frameworks, both in clarity on scope and function and in the conceptual robustness of domains, is warranted and further standardization of generic sets of indicators should be sought.

With European health systems facing a plethora of challenges that can be addressed through public health interventions there is renewed interest in strengthening public health services. Yet there are enormous gaps in our knowledge. How many people work in public health? How much money is spent on public health? What does it actually achieve? None of these questions can be answered easily. This volume brings together current knowledge on the organization and financing of public health services in Europe. It is based on country reports on the organization and financing of public health services in nine European countries and an in-depth analysis of the involvement of public health services in addressing three contemporary public health challenges (alcohol obesity and antimicrobial resistance). The focus is on four core dimensions of public health services: organization financing the public health workforce and quality assurance. The questions the volume seeks to answer are: o How are public health services in Europe organized? Are there good practices that can be emulated? What policy options are available? o How much is spent on public health services? Where do resources come from? And what was the impact of the economic crisis? o What do we know about the public health workforce? How can it be strengthened? o How is the quality of public health services being assured? Evidence was obtained by a scoping review of academic literature in English, Dutch and German and of grey literature in English, Dutch, German and Italian, supported by a Region-wide expert enquiry. Emerging findings were presented to representatives from 19 Member States of the Region to check for accuracy and omissions. The report highlights much good health literacy policy-related activity, mostly in the health and education sectors, and proposes areas for future development. Policy considerations to facilitate the sharing of good health literacy policy practice, the development of policy aims and activities across all societal areas, and the development of robust health literacy metrics to identify the need for and monitor effectiveness are presented.

Lessons learned from across the Region on action taken by the WHO Regional Office for Europe and Member States to improve the health and well-being of their populations are presented. The report also addresses the new public health challenges that have emerged in recent years. To respond effectively to these challenges new forms of evidence are essential to measure health and well-being in different cultural and subjective contexts. This is particularly important in the context of the 2030 Agenda for Sustainable Development and the Sustainable Development Goals whose health indicators overlap significantly with those for Health 2020. The report will be a useful source of information for policy-makers throughout the Region helping them identify areas that need further assessment and policy action at the national level. It should inspire Member States and other stakeholders to contribute to the work under the umbrella of the WHO European Health Information Initiative: a collaboration between the Regional Office European institutions and Member States aimed at improving the information that underpins policy. Only through broad international cooperation and bold strides in the way evidence is used in the 21st century will evidence fully inform health policy-making for the benefit of all.

This report synthesizes the global evidence on the role of the arts in improving health and well-being, with a specific focus on the WHO European Region. Results from over 3000 studies identified a major role for the arts in the prevention of ill health, promotion of health, and management and treatment of illness across the lifespan. The reviewed evidence included study designs such as uncontrolled pilot studies, case studies, small-scale cross-sectional surveys, nationally representative longitudinal cohort studies, community-wide ethnographies and randomized controlled trials from diverse disciplines. The beneficial impact of the arts could be furthered through acknowledging and acting on the growing evidence base; promoting arts engagement at the individual, local and national levels; and supporting cross-sectoral collaboration.

In the context of increasing noncommunicable disease (NCD) burden and unhealthy diets, there is a need to continue identifying optimal, evidence-informed diets and interventions for the prevention and control of NCDs. The MD and ND have been identified as region-specific healthy diets. To support decision-makers in shaping context-specific diet and nutrition policies, this review provides a summary of the NCD burden and activities in the Region; outlines the NCD-related health benefits of the MD and ND; describes interventions and policies in 15 countries; reviews four identified studies into the effectiveness of MD and ND policies on NCD outcomes; and discusses policy implications and options. In the context of MD and ND interventions for NCDs, there remains a Region-wide need to increase translation of evidence into action, monitor and evaluate the impact of existing policies on NCD outcomes and share activities through public platforms to support information sharing.

The study reveals a wide range of experience and variation across Europe and clearly illustrates two fundamentally different approaches to public health services: integration with curative health services (as in Slovenia or Sweden) or organization and provision through a separate parallel structure (Republic of Moldova). The case studies explore the context that explain this divergence and its implications. Book excerpt: The increasing number of refugees, asylum seekers, and irregular migrants poses a challenge for mental health services in Europe. This review found that these groups are exposed to risk factors for mental disorders before, during, and after migration. The prevalence of psychotic, mood, and substance-use disorders in these groups varies but overall resembles that in the host populations. Refugees and asylum seekers, however, have higher rates of post-traumatic stress disorder. Poor socioeconomic conditions are associated with increased rates of depression five years after resettlement. Refugees, asylum seekers, and irregular migrants encounter barriers to accessing mental health care. Good practice for mental health care includes promoting social integration, developing outreach services, coordinating health care, providing information on entitlements and available services, and training professionals to work with these groups. These actions require resources and organizational flexibility.
