Book excerpt: Excerpt from The Dublin Journal of Medical Science, Vol. 130: July to December, 1910 The theme which I wish to-day to expound is based upon the knowledge that the work of the surgeon during the last ten years has conferred upon him the opportunity of dis covering that a very large proportion of the cases of dyspepsia owe their origin not to functional but to organic causes. What the proportion of the cases owning a structural origin may be it is not possible to say, but 1 am confident that it will not be long before, in any given case of inveterate dyspepsia, we shall require to be furnished with valid reasons for believing that the cause of the condition is anything other than a morbid change in some of the abdominal viscera. The tendency, that is to say, will surely be to regard all such cases as organic rather than functional, and to lay the burden of proof upon those who cling to the latter View. I often hear it said that the surgeon is encroaching upon the territory of the physician in his dealings with the serious forms of dyspepsia. This is a foolish way of viewing the question, for the distinction between medicine and surgery the divorce of brain from hand - always arbitrary, is here impossible. If the surgeon is able to demonstrate that manv of the forms of dyspepsia are veritably organic in their origin, it would be idle to deny that the treatment of the case must change with our changing knowledge. I will ask you to-dav to consider with me three forms of dyspepsia, the origin of which in organic lesions has now been conclusively demon strated. The chemical guess that this is due to certain hypothetical sub-salts of Ag has never been experimentally demonstrated and may be cast aside. The truth about the latent image is this. It represents adsorption complexes of colloid silver in different degrees of dispersion, with normal non-reduced silver haloids. This view is beautift demonstrated by the experiments of W. Reinders who has succeeded in producing these photo-haloids synthetically in the form of differently coloured crystals by allowing different silver haloids to crystallise in the presence of differently coloured colloid silvers. In deciding this important point the pylorus played the chief part. Indeed, a personality was actually supposed to be concerned with the performance of this function, and the name of pylorus signifies the guard or keeper of the entrance - the janitor or porter. For instance, Galen looked upon the pylorus as a regulator or governor of the entrance to the intestine, having sovereign authority and monarchical power over all-the other structures of the stomach. This governor was endowed with the faculty Of judgment and decision, by virtue of which he opened the portal only when he himself considered best. He did so at the end of digestion, not because he was urged thereto by the chyme, but because he had a knowledge of the events happening in the stomach. On the other hand, the pylorus closed the orifice and set up vomiting when he considered it would be injurious to the health to allow the stomach contents to pass further into the system.
