Book excerpt: Results with 9,566 children who participated for four years in a 10-site, school-based preventive care program indicated that dental health lessons, brushing and flossing, fluoride tablets and mouthrinsing, and professionally applied topical fluorides were not especially effective in reducing a clinically significant amount of dental decay. This was true when the procedures were used singly or in combination. The only study procedure that consistently reduced decay involved applying occlusal sealants to the chewing surfaces of the posterior teeth. Although this measure prevented 23 to 65 percent of all new carious lesions, this amounted to only one to two carious surfaces in four years. Children who were especially susceptible to decay did not benefit appreciably more from any of the preventive measures than did children in general. The direct costs of sealants or prophy/gel treatments were about $23 per child per year (in 1981 dollars). The cost of adding a fluoride mouthrinse program to an existing supervised health education program was about $3.29 per child per year. Communal water fluoridation was reaffirmed as the most cost effective measure for reducing dental decay in children.

Dental caries is preventable, yet it is the most common chronic disease among children and adolescents in the US [2]-[6]. US dental care systems have fallen short of preventing avoidable oral diseases and their consequences, despite enormous dental expenditures. Self-care through oral hygiene, a key health behavior for caries risk reduction, is not practiced nearly as consistently or accurately as dentists and health organizations would like [7],[8]. Evidence-based and cost-effective preventive dental treatments exist but are underutilized. Value-based care (VBC) is an effort to strengthen the quality of dental care, decrease unnecessary expenditure and increase cost-effectiveness through an emphasis on prevention. This paper has three aims. Aim #1 is to understand the current dental VBC landscape through a narrative review. It synthesizes the available literature on dental value-based models and their challenges.

Book excerpt: The purpose of this study was to provide a cost analysis of the Miles of Smiles Program, a collaboration between the University of Missouri-Kansas City (UMKC) School of Dentistry and the Olathe School District. This preventive program was implemented to address the access to oral health care issues that affect low income children within the school district. The analysis of the program utilized an inventory list and an existing de-identified database to determine the costs associated with operating the program throughout the 2008-2009 school term. Costs related to equipment, supplies, and personnel were included. The results of the analysis revealed that the cost of operating the program during 2008-2009 was $107,515.74. The program received Medicaid reimbursement for approximately 1.5% of the total cost of operating the program and approximately 6.3% of the amount produced through billable services; however, challenges with submitting and billing Medicaid claims for the first time contributed to this low percentage of reimbursement. It was determined that for the program to be sustainable, continuous external sources of funding or a change in the program design would be necessary.
