Book excerpt: Critical care units are environments of elevated stress and high acuity that require educated, knowledgeable and skilled practitioners. To function in such settings, nurses must be reflective, life-long learners who are ready to adjust to the rapidly changing landscape of critical care medicine. The purpose of this qualitative research study was to investigate the reported experiences of undergraduate baccalaureate nursing students' in their application of learned theory to clinical practice following a rotation in the critical care setting through Schön's (1983) theoretical framework of reflective practice and more particularly will examine reflection-on-action. Baccalaureate nursing students were questioned regarding their use of reflective practice which was categorized as metacognitive knowledge (self-knowledge), metacognitive experiences (task-knowledge) as well as metacognitive strategies (planning-knowledge). These students reported that their reflective practice was related to the transition of classroom theory to a practiced skill based understanding obtained through the development of their critical thinking skills. Students also reported the development of a holistic understanding of the professional nurse role. This examination supports changes in baccalaureate nursing curricula that could aide students in the assimilation of theoretical knowledge into clinical practice, thereby increasing experiential learning through the reflective process. Nurse educators should provide experiences that promote critical thinking skills. In addition, recommendations for research could consider what undergraduate baccalaureate nursing faculty report regarding how to promote student experiences in collaborative communication in a multi-disciplinary environment that examines faculty at public and private institutions.

Book excerpt: The purpose of this project was to determine if early simulation would increase the clinical confidence of novice nursing students. A convenience sample of 20 junior nursing students in their first semester of a baccalaureate nursing program within a small, rural university participated in the project prior to their first clinical experience. The students were administered the Confidence Scale as a pre-test prior to the early simulation experience which consisted of a scenario comparable to what the students would experience in the clinical setting. After the simulation, the primary investigator facilitated a debriefing exercise and then administered the Confidence Scale again as a post-test, as well as the Student Satisfaction and Self-Confidence in Learning Instrument to determine confidence levels after the simulation. A paired samples t test was performed to evaluate the change in confidence levels after the early simulation intervention. The results indicated that there was a statistically significant improvement in confidence scores after the simulation for each of the five questions on the Confidence Scale. The Student Satisfaction and Self-Confidence in Learning instrument results also demonstrated high levels of satisfaction and confidence after the early simulation experience. Linear regression was implemented to determine relationships between the demographic information and the changes in the pre-test and post-test confidence levels. A statistically significant relationship was found between the Confidence Scale question related to confidence in portraying competence in front of an observer and employment as a home health CNA. Another statistically significant relationship was found between the Confidence Scale question related to confidence in task performance and employment as a long term care CNA.

Book excerpt: The clinical learning environment (CLE) provides students with an opportunity to build confidence and competence in the provision of patient care (Benner, 2010). The aim of this qualitative descriptive research was to explore and describe student perceptions of developing confidence through their clinical learning experiences, discovering what features of the CLE support their learning and the development of confidence. Ten students were recruited through purposive sampling and participated in one-on-one semi-structured interviews. Self-efficacy theory (Bandura, 1997) was used as a theoretical framework to guide this research and for the interpretation of the content analysis. The analysis revealed five socio-structural themes that support student learning and development of confidence. These findings elaborate Bandura's (1997) theory. The most influential, through verbal persuasion was the clinical nursing instructor (CNI) followed by self, the buddy nurse, peers, and the staff/unit environment. Students perceived their development of confidence through a bi-directional interaction between their cognitive/affective processes, behaviour and the CLE. Understanding student perceptions of their confidence development, signals the need for informed pedagogical strategies to support student learning and development of confidence.

Book excerpt: Teaching nursing students how to make effective clinical decisions is imperative to facilitate safe practice, however, nursing education may not be adequately developing clinical decision-making (CDM) skills. Little is known about graduating students’ perceptions of their affective states that may interfere with CDM abilities. The purpose of this study was to describe the level of self-confidence and anxiety with CDM in senior accelerated baccalaureate nursing students at the completion of their clinical education at a large private university in northern New Jersey. Fifteen participants completed the Nursing Anxiety and Self-Confidence with Clinical Decision Making Scale at the end of the program. Higher scores suggest higher levels of self-confidence and anxiety with CDM. Mean self-confidence scores during CDM were M = 133.33, SD = 15.57 and mean anxiety scores during CDM were M = 63.13, SD = 27.81. Mean item scores indicated the “Almost Totally” response for self-confidence and the “Just a Little” for anxiety. A high level of self-confidence and low level of anxiety during the process of CDM was reported. Understanding the affective states that influence CDM, may help to gauge readiness to make clinical decisions in practice.

Book excerpt: Baccalaureate nursing students must develop clinical reasoning skills in order to make sound clinical judgments. How students understand clinical reasoning is of interest to nurse educators. In a qualitative study, eight third-year nursing students were interviewed about their perceptions of clinical reasoning on a Clinical Judgment Exercise (CJE). An overarching theme of Over Time emerged from the data along with two themes: Understanding of Clinical Reasoning and Making Sense of the Assignment. The sub-themes that emerged were the same for each theme and were not knowing, knowing, applying knowing and valuing knowing. Conclusions were that student participants perceived: 1) understanding of clinical reasoning developed over time, 2) understanding of the patient's problem deepened over the time of writing the assignment, 3) they were challenged by the complexity of the patient, 4) they were able to apply learning from the CJE to nursing practice and 5) writing the CJE was stressful.
