The present article, then, is not intended to laud the action of caustics as superior or equal to that of the scalpel under all circumstances in these diseases, but only to indicate, in a measure, the conditions under which they should be employed in preference to all other means, or as accessory to these, the selection of the particular caustic for the given purpose, and, in a word, to assist in rescuing from the hands of ignorant quacks and pre tenders such precious remedies; for it can not be denied that in this country, at least, they have been abandoned to a large extent to such persons, whose ignorant and in discriminate uses of them have tended to bring them into further disrepute. T., a tall, spare woman of nervous temperament, when about twenty-seven years of age, suffered a severe disappointment in a love affair. Soon afterward she began to present symptoms of impaired nutrition, loss of color, emaciation, debility, etc. She became much more highly nervous and excitable. At the same time the action of the heart became rapid and excited, and the thyroid gland enlarged. There was a marked irregularity of menstruation. There was a marked anaemic bellows murmur over the base of the heart. The carotid arteries pulsated violently, and were the seat of a strong thrill and aneurismal bruit. There was also pulsation of the thyroid gland, with thrill and murmur. Under the use of iron, digitalis and tonics, she improved so much that she discontinued treatment. Before long she suffered from a return of all the above symptoms in an aggravated form, and, in addition, there was prominence of the eyeballs; but again she found relief, and during the next five years she had several similar relapses, though each time the symptoms were more advanced. The degree of enlargement of the thyroid varied considerably, being extremely marked at times. Both lobes were equally affected. The prominence of the eyeballs also varied much from time to time; during several of the attacks, being so extreme that the eyelids would not close over them. On several occasions there was slight conjunctivitis, always easily relieved. The attacks were always marked by a great increase in her debility and nervous excitability. The hearts action never regained its normal state, but continued rapid and easily excited by slight causes. At times the pulse was exceedingly rapid, 130 to 150 in the minute. After several such attacks her general health became permanently impaired. The bellows murmur became harsher in character. The area of cardiac dullness increased, and the sounds of the heart became sharp and weak, indicating the development of dilatation with slight mitral insufficiency. dema of the feet and other evidences of impaired circulation now appeared. In the spring of 1864 she again came under my observation, and in far worse condition than at any previous time. Book excerpt: Excerpt from Cincinnati Medical News, Vol. 9 Many attempts have been made to exhibit phosphorus in an uncombined state, which has long been a desideratum with the profession. It has been proposed to dissolve it in ether, but a more dangerous advice could hardly have been suggested. Cod-Liver oil, too, has been tried to some extent as a vehicle, but the phosphorus is readily oxidized unless protected by a long and difficult process, and the nauseous taste of the oil practically removes this agent from consideration. We have for many years been supplying this element to the profession in the form of pills, which seem to obviate the objections to the other forms of administration now in use. They are absolutely protected from change by the sugar-coating, itself a powerful deoxidizing agent. They are easy and pleasant to administer, prompt in their action, and probably introduce the element in the best form for speedy absorption. While pills of phosphorus, properly prepared, are a safe and efficient method of administering this remedy, physicians will do well to avoid all brands which are not known to be worthy of confidence. To properly prepare these pills requires expensive and complicated apparatus, and the highest skill that can be procured. We ask only a careful comparison of our products with those of other manufacturers, believing that such a test is the best advertisement we can desire. Book excerpt: Excerpt from The Cincinnati Medical News, 1880, Vol. 13: Vol. IX, New Series The above may sound tenable, but the fault lies in one of the premises which will not bear analyzing. The statement that the pus is in the ear and should be al lowed to escape, is correct in one sense of the term, and incorrect in another. It leads a person not acquainted with the nature of the disease to imagine that there is an abscess or cavity located somewhere at the internal extremity of the canal; that this cavity is filled withpus; that the perforation spoken of is the outlet, and that when all of the pus has escaped from the cavity, that nature will assert her influence and close the perforation, as well as the cavity. Theoretically this sounds very well, and to the uneducated apparently explains the na ture of the disease in a very satisfactory manner. But we will examine it a little more closely. Sometimes we see a person who has had an otorrhoea for ten or fifteen years, the discharge being continuous. How large would the cavity have to be to contain that amount of pus? Not in the ear, for it is not large enough. Certainly not in the brain, for death Would soon claim the victim. Then as we can not find a place to locate the cavity, we must explain the cause of the discharge in some other way. In order for us to have a proper understanding of the cause, it will be necessary for us to look at the anatomy of this part of the organ. When cold is applied to certain definite regions of the skin in pigeons, they exhibit retrograde movements alter nating with fits of stupor. The agent, so far, has been rhigolene, to produce the necessary cold. Ether, when vaporized, was totally unable to produce any effect. The region to which the cold must be applied is the skin of the back of the neck. These phenomena were first oh served by Dr. S. Weir Mitchell. I have lately been study ing the effect of irritants on the skin of pigeons, and the seat of the phenomena produced. I found that ether, alcohol, chloroform and nitrite of amyl were powerless to produce the effects seen after the application of rhigolene. If, however, bisulphide of carbon was dropped on the skin of the back of the neck, then all the phenomena produced by rhigolene ensued in a marked manner. Thus a single drop of the bisulphide of carbon applied to the back of the neck of the pigeon caused him to retrograde and to pass into states of quietude. When bisulphide of carbon is applied to the skin of the neck in pigeons, the birds run forward as if no agent was acting on them, but sud denly they commence to run backwards, it being quite evidently against their will, as they seek to overcome the tendency. During the period ot'quietude, the body sinks down somewhat, the head being drawn in close to the body and bent towards the ground to a considerable ex. Tent. That these phenomena are due to a simple irrita tion of the nerves, is proved by an experiment where I attached a small bulldog forceps to the skin on the back of the neck, when similar retrograde movements ensued, followed by a period 01 quietude. As remarked by Dr. Mitchell, these phenomena are re ex in their nature. Here the mechanical irritation of the sensory nerves is re ected94 retrograde and lateral movements with hypnotism. So hazardous have those cases been to surgeons, that our best authors on the subject concede as much as an inch or two shortening in oblique fractures of the long bones, especially of the lower extremities. This is an act of magnanimity on their part for which we feel truly grateful. To be experts in this branch of surgery, we must be correct diagnosticians, and last, not least, render a fitting prognosis. It is certainly better to predict a result com patible with the nature of the case and its surroundings, than to promise a good result when reason and good judgment would insure the reverse.
