Does the migration of highly skilled professionals from developing and underdeveloped countries to developed countries harm or hurt their country of origin?In Emigration, Brain Drain, and Development: The Case of Sub-Saharan Africa, Arno Tanner questions the emerging literature that stresses the positive aspects of labor migration. He finds that while emigration certainly cannot be stopped, and may be beneficial in some cases, unhindered high-skilled emigration —particularly in the case of sub-Saharan Africa —can have disastrous consequences. In examining the cases of Malawi, Tanzania, and Zimbabwe, Tanner finds striking trends. For instance, the outflow of physicians from Malawi may severely hurt AIDS prevention. Furthermore, sub-Saharan Africans tend not to return; remittances are erratic, have dwindled over time, and do not offset the costs of emigration. Tanner recommends specific policies where carefully targeted development measures could be used to mitigate the negative consequences of brain drain.

Book excerpt: Concerns about the negative impact of the "brain drain" of health professionals from Africa have led to a dominant narrative in which those who migrate are a permanent, and costly, loss to the country of origin and a permanent, and valuable, gain for the country of destination. In the new world of transnationalism, a global skills market, and greatly increased mobility by health professionals, it is unlikely that the traditional permanent-exodus model of the brain drain narrative adequately captures all forms of migration by South African doctors. This report first examines the temporary employment opportunities for South African doctors in countries such as the United Kingdom, Ireland, Canada, and Australia. This report draws two major conclusions: first, the dominant brain drain narrative overlooks the complex nature of South African physician migration and ignores the fact that a significant number of doctors have temporary employment experience outside the country. Second, it suggests that temporary employment overseas increases the chances of permanent emigration later.

Meredeth Turshen considers the implications of introducing private insurance in countries with growing unemployment, a shrinking formal job sector, and a lack of social security programs or other safety nets. She debates the pros and cons of shifting the delivery of health services to the nongovernmental sector in the context of new concepts of the role of the state. Many of the schemes to privatize the purchase and sale of pharmaceuticals reverse decades of United Nations work challenging the power of the multinational drug industry. Turshen weighs these policy changes in light of the World Bank's eclipse of the World Health Organization as the premier UN health policy agency. Until now, no book has disputed the World Bank's plans to privatize health care in Africa. This is the first book-length analysis of policy changes in light of monetarism and globalization. Throughout the book, Turshen examines the implications of privatization for gender equity. She also provides a case study of Zimbabwe and comparative material from Malawi, Mozambique, and Zambia. First, by offering options to those affected by conflict and crises in a region that has limited formal disaster management and social protection systems. Second, by mitigating shortcomings and distortions in regional labour markets. Third, by providing support to struggling rural economies and ever expanding urban areas in terms of livelihoods and social capital transfers. Chapter One consists of a study of the country’s historical experience of migration and, in particular, analyses the changes in official attitudes throughout the twentieth century, indicating the roots of contemporary ideas and policy dilemmas. Chapters Two, Three, Four and Five complement this analysis of the South African State’s capacity to reform and manage the South African migration situation by looking at often neglected dimensions: the first explores the question of skilled labour, a crucial question given the unbalanced structure of the South African labour market; the second examines the impact of migration on local government in South African cities and specifically implications for urban planning, service delivery, health, security, and political accountability; the third analyses the nature of undocumented migration to South Africa and the challenges it raises to both State and non-State actors; The book concludes with an examination of health as a critical issue when examining the relationship between migration and development in South Africa, in light of recent empirical data.

They have traditionally been viewed as repositories of information and wisdom, and are critical pillars of the community but as the HIV/AIDS pandemic destroys family systems, the elderly increasingly have to deal with the loss of their own support while absorbing the additional responsibilities of caring for their orphaned grandchildren. Aging in Sub-Saharan Africa explores ways to promote U.S. research interests and to augment the sub-Saharan governments' capacity to address the many challenges posed by population aging. Five major themes are explored in the book such as the need for a basic definition of "older person," the need for national governments to invest more in basic research and the coordination of data collection across countries, and the need for improved dialogue between local researchers and policy makers. This book makes three major recommendations: 1) the development of a research agenda 2) enhancing research opportunity and implementation and 3) the translation of research findings.

Book excerpt: Migration from and within Africa, just like migration elsewhere in the world, often generates anti-immigrant sentiment and ignites heated public debate about the migration policies of the destination countries. These countries include South Africa as well as others outside the continent. The countries of origin are also keen to minimize losses through "brain drain" and to capture resources such as remittances. Increasingly, international organizations and human rights advocates have stressed the need to protect the interests of migrants themselves. However, while the UNDP's 2009 Human Development Report talks of "win-win-win" solutions, in practice it is the perceived interests of destination countries that enjoy the greatest attention, while the rights of migrants themselves are afforded the least. Yet migration is not just an issue in itself: it also points to structural inequalities between countries and regions. Managing migration and protecting migrants is too limited an agenda. Activists and policymakers must also address these inequalities directly to ensure that people can pursue their fundamental human rights whether they move or stay. It is not enough to measure development only in terms of progress at the national level: development must also be measured in terms of reductions in the gross levels of inequality that now determine differential rights on the basis of accident of birth.
