Book excerpt: Excerpt from The American Journal of the Medical Sciences, 1902, Vol. 123 She was in such a miserable condition that I really thought that it would have been better for her if she had not survived. Two months later she re-entered the hospital, and I Operated for repair of the fistula. It was found that the uretero-vesical anastomosis on the left side had been entirely successful. Fortunately, the end of the right ureter had become adherent at the edge of the fistula. A probe was introduced into it, and it was dissected off with the vesical mucosa, and thus turned into the bladder. The edges of the fistula were then split so that the everted mucosa could be rolled in ward. The vaginal edge was denuded, the cervix uteri being utilized to fill in the upper part Of the gap. Sutures were introduced in the usual manner, parallel with the long axis of the vagina, care being exercised not to include the end of the ureter. A catheter was left in situ for three days, and the patient subsequently passed her urine every four hours, retaining from four to six ounces. On the sixth day the nurse allowed her to go all night, so that twelve ounces accumulated in the bladder, with the inevitable result. Book excerpt: Excerpt from The American Journal of the Medical Sciences, Vol. 111 In 1836 Donne found a previously undescribed parasite in the vaginal secretions of women. These were described as roundish bodies about twice the size of pus corpuscles and difficult to distinguish from the latter except by their independent motion. The motion was produced by certain organs, viz., a flagelliform appendage at the anterior end which worked with great rapidity, and a row of short cilia, at one side and near the flagellum, which kept up a sort of rotary motion. At the posterior end was a tail-like appendage of variable shape (Fig. 18). On account of the fact that the parasite resembled monads in having a flagellum, and "trichodines" (ciliata) in having cilia, Donne gave it the name of trichomonas, with the specific name vaginale on account of the habitat. Although there were errors in Donne's description, it was remarkably good considering the methods of observation and the knowledge of micro-parasites then available. Certain details in the description will be mentioned later. Notwithstanding Donne's growing reputation as a microscopist, and the fact that his observations were controlled by such men as Dujardin, Milne-Edwards, Ricord, and others, many microscopists (Vogel, Siebold, Valentin) denied the parasitic nature of the bodies, and held them to be altered epithelial cells from the uterus. Later, however, Scanzoni and Kolliker, Hennig, Haussmann, and Gasser re-examined the organisms, mostly from the side of clinical microscopy. Trephining and exploration of brain, with great improvement, lasting nine months; relapse. Second operation; marked and increasing improvement. - May M., aet. twenty-eight, American by birth and parentage, and of good education; of rather slight physique but always in fair health. There is no family taint of insanity or personal history of neurotic tendency or mental instability. In April, 1884, she became deranged, the mental disability continuously increasing. In July of the same year she was admitted to the Illinois Hospital for the Insane, at Elgin, in the service of Dr. Church, and entered on the records as a case of subacute mania. The father, a very well educated and intelligent man, subsequently stated that the only assignable cause for his daughter's alienation was a knock she had given her head in coming up a cellar stairway. She was stunned by the blow and delirium lasting a few hours resulted. The details of this accident, coming through third parties, are not entirely reliable, and the portion of the head struck could only be inferred. A few days afterward a lady whom she was visiting thought her "out of her head." Gradually she grew worse, and finally was so unmanageable, though never violent, as to require hospital treatment and restraint. Book excerpt: Excerpt from The American Journal of the Medical Sciences, Vol. 99 Among the many acute diseases of bones, probably none has attracted more attention of late years than osteomyelitis. This is largely due to the fact that with our advanced knowledge of bacteriology many points in its etiology and causation have been cleared up, and at the same time our methods of treatment are more scientific and accurate. Acute arthritis of infants is a disease which may properly be described under the acute infectious processes of bone. The term was first used by Jules Teilhard La Terrisse in his thesis presented to the Faculty of Medicine of Paris for the degree of doctor of medicine, in 183. He reports three eases in children of three, six, and nine days respectively, and on autopsy in each case found pus in several of the joints. In one case there were also several abscesses in the lung. He believes the joint collections of pus were metastatic, and says that M. Dance, in an article on phlebitis, has cited similar cases. In 1874, Mr. T. Smith, surgeon to St. Bartholomew's Hospital, describes the affection and report twenty-one cases, under the name "Acute Arthritis of Infants."
