Book excerpt: The first generation of surgical robots are already being installed in a number of operating rooms around the world. Robotics is being introduced to medicine because it allows for unprecedented control and precision of surgical instruments in minimally invasive procedures. So far, robots have been used to position an endoscope, perform gallbladder surgery and correct gastroesophogeal reflux and heartburn. The ultimate goal of the robotic surgery field is to design a robot that can be used to perform closed-chest, beating-heart surgery. The use of robotics in surgery will expand over the next decades without any doubt. Minimally Invasive Surgery (MIS) is a revolutionary approach in surgery. In MIS, the operation is performed with instruments and viewing equipment inserted into the body through small incisions created by the surgeon, in contrast to open surgery with large incisions. This minimizes surgical trauma and damage to healthy tissue, resulting in shorter patient recovery time. The aim of this book is to provide an overview of the state-of-art, to present new ideas, original results and practical experiences in this expanding area. Nevertheless, many chapters in the book concern advanced research on this growing area. The book provides critical analysis of clinical trials, assessment of the benefits and risks of the application of these technologies. This book is certainly a small sample of the research activity on Medical Robotics going on around the globe as you read it, but it surely covers a good deal of what has been done in the field recently, and as such it works as a valuable source for researchers interested in the involved subjects, whether they are currently “medical roboticists” or not.

Available in PDF, EPUB and Kindle. Book excerpt: The introduction of the surgical robot resulted in increasing technical difficulty of these robot assisted surgeries (RAS). The qualification and certification of RAS skills are still in a preliminary phase within all surgical specialties, also in urology. New methods of training for novice surgeons (residents and fellows) in these highly specialised techniques should be implemented to guarantee patient safety. The challenge for novice surgeons is how to learn new surgical procedures and once the procedure is learned and they become experts how to analyse past performances and subsequently use this as a lesson for the future in order to improve postoperative outcome. In the first part of this these we explored the best methods to educate surgeons in robotic surgery. By using different modalities of training, (i.e., hands-on training combined with theoretical information of learning) we discovered (chapter 2) novice robot surgeons were too positive in the self-assessment of their own surgical skills after a training in RAS. To prevent overconfidence biases the novice surgeons should be provided with feedback (proctoring) to inform them about their results to enhance learning and inform them of their competence levels. Although results (chapter 3) show there is no significant difference in the acquisition of surgical skills between different types of proctoring provided to novice surgeons the participant satisfaction seems to be higher in the “human proctoring” group. Which could result in additional motivation for the participant to continue training in RAS. Additional research (chapter 4) shows residents in urology are allowed to perform RAS during their residency, but criteria for starting RAS differ significantly among the teaching hospitals. This shows there is a need for the implementation of a (multi-step) training and certification program in the Dutch urology residency curriculum.

Specifically, the dissertation aims to (1) develop a human-centric analytical approach to objectively evaluate robot-assisted teleoperation, (2) develop machine learning based approaches for automated surgical skill assessment in basic surgical training tasks while improving the interpretability of the assessment to support efficient acquisition of skills, and (3) develop methods for an transferrable and task independent assessment of the operative difficulty demand and surgical skills by extending analysis to broader scenarios of robotic surgical teleoperation. Based on novel techniques of computational intelligence and human-centric analysis, this PhD research highlights the potential to improve surgical training and intuitive design of teleoperation systems for high performing robot-assisted surgery.

Additionally, surgical training is undergoing a paradigm shift with more emphasis on the development of technical skills earlier in training. Thus metrics for the skills, especially objective metrics, become crucial. One field of surgery where such techniques can be developed is robotic surgery, as here all movements are already digitalized and therefore easily susceptible to analysis. Robotic surgery requires surgeons to perform a much longer and difficult training process which create numerous new challenges for surgical training. Hence, a new method of surgical skill assessment is required to ensure that surgeons have adequate skill level to be allowed to operate freely on patients. Among many possible approaches, those that provide noninvasive monitoring of expert surgeon and have the ability to automatically evaluate surgeon's skill are of increased interest. Therefore, in this dissertation we develop a predictive framework for surgical skill assessment to automatically evaluate performance of surgeon in RMIS. Our classification framework is based on the Global Movement Features (GMFs) which extracted from kinematic movement data. The proposed method addresses some of the limitations in previous work and gives more insight about underlying patterns of surgical skill levels.
