In our western civilization, the bulk of attention has focused on those indi viduals who specifically seek treatment or those who have become so disabled by these problems that they require treatment. These indi viduals usually qualify for a psychiatric diagnosis of alcohol or other substance abuse. However, just as it has been recognized that primary substance abuse is frequently associated with other diagnosable psychi atric disorders, such as sociopathy or attention deficit disorder (residual type) and that the origins of substance abuse are multivariate, we have also begun to become aware that many other individuals in our society with psychiatric or other problems also suffer, to varying degrees, from substance abuse. These problems may be considered secondary by vari ous specialists or treatment personnel; but nevertheless, they are prob lems, and what disorder is primary or secondary in a given individual may often be very difficult to determine in a meaningful fashion. Thus, within the past decade, research studies have reported significant inci dences of substance abuse/or misuse in high school and college-aged populations, in medical populations, and in individuals with other psy chiatric disorders such as schizophrenia, depression, and the anxiety and personality disorders. Yet to date little has been done to bring together and systematize this widely scattered data that describes the presence of substance abuse problems in various populations.

A ‘substance’ is a chemical compound that alters behaviour or mood when ingested. This group includes alcohol, nicotine, caffeine, heroin and cocaine along with chocolate and soft drinks. The vast majority of users of the ‘safe’ drugs are not perceived as addicts, they can be equally as addictive and harmful to one’s health. There is a clear distinction between a user and an abuser of drugs. ‘Substance use’ suggests that the individual ingests the drug in moderate amounts that have no significant effect on their social, educational or occupational functionality. The drug has the ability, based on the quantity ingested to create impaired judgement, mood changes and lowered motor ability; this result is referred to as intoxication. ‘Substance abuse’ as an extension of ‘use’ is really defined on how the use of the drug affects the individual’s relationships, his work or education and whether it causes him to place himself or others in physically dangerous situations (Barlow & Durand: 2005). ‘Drug dependence’ is often described as addiction. There is however some disagreement in how we best define substance dependence (Woody & Cacciola: 1997). One definition would explain how the individual requires systematically greater quantities of the drug to experience the same level of intoxication (tolerance), and will behave negatively if the drug is not ingested (withdrawal) (Franklin & Frances: 1999). An alternate view on substance dependence pertains to the behaviour that focuses on finding the required drug as a dependence indicator. A different view of substance dependence focuses on the drug seeking behaviour itself as an indication of dependence. The repetitive ingestion of the drug, an increased need for more of the drug and the likelihood that re-use of the drug will occur after a period of abstinence; are all behaviours that help to define the extent of the drug dependence. These behaviours have a psychological component and these in conjunction with physiological components of tolerance and withdrawal make for a better understanding of substance dependence. Here, Louis Berger contests both the orthodox view of substance abuse as a "disease" explicable within the medical model, and the fashionable dissenting view that substance abuse is a habit controllable through the "willpower" fostered by superficial treatment approaches. According to Berger, substance abuse is first and foremost a symptom. He argues that it is only by grasping this fact that we can understand why standard approaches to treatment and prevention have failed. Berger invokes a wide spectrum of recent analytic insights about infant and child development, the psychology of narcissism, and primitive character disorders in making the case that substance abuse masks serious preoedipal (or "midrange") psychopathology. Such psychopathology, operating at both cultural and person levels, explains why certain individuals become dependent on illicit drugs; it is equally revelatory of why the substance abuse "establishment" -- and society at large -- continues to misconstrue the nature of the problem and to proffer ill-conceived and ineffective remedies. In 1993, the first version of these guidelines, titled: ‘An outline for the management of alcohol problems: Quality assurance in the treatment of drug dependence project’ was published (Mattick & Jarvis 1993). The Australian Government commissioned an update a decade later (Shand et al. 2003) and a further edition in 2009 to integrate the Guidelines with the Australian Guidelines to Reduce Health Risks from Drinking Alcohol (National Health and Medical Research Council, NHMRC 2009; Haber et al., 2009). The present version of the Guidelines was also commissioned by the Commonwealth of Australia to remain current and integrated with the updated NHMRC consumption guidelines (2020). In order to ensure that guidelines remain relevant, the next set of guidelines should be updated in 2025, consistent with NHMRC recommendation that guidelines be updated every five years. These guidelines aim to provide up-to-date, evidence-based information to clinicians on available treatments for people with alcohol problems and are largely directed towards individual clinicians in practice, such as primary care physicians (general practitioners, nursing staff), specialist medical practitioners, psychologists and other counsellors, and other health professionals. Some chapters highlight service or system level issues that impact on clinicians and their patients. These include recommendations concerning Aboriginal and Torres Strait Islander peoples, culturally and linguistically diverse groups, stigma, and discrimination. Elsewhere, organisation capacity is implied, such as medical resources for withdrawal management where recommendations indicate use of medications. As all forms of treatment will not be readily available or suitable for all populations or settings, these guidelines may require interpretation and adaptation.
