Book excerpt: From the early days of its recognized occurrence, AIDS has been per ceived as posing tremendous threats, burdens and challenges to human beings. Individuals, societies and, in a global point of view, mankind are affected by the effects of the HN infection, the nature and extent of which is still unclear in many ways. In the beginning only biomedical and epidemiological analyses of the problem were the top research priori ties, the former laden with great hopes that it may soon be possible to stop the spread of the disease and to overcome its physical impact. Yet it soon became clear that AIDS would be something to be reckoned and coped with on a long-term basis, making a thorough investigation of its impact absolutely mandatory. AIDS has serious economic consequences. Taken seriously, they can not be confined to predictions of costs intended to support the AIDS issue in the struggle for resources. Besides cost calculations - a method ologically tricky and wide-ranging topic in itself -and their application to cost-effectiveness and other analyses, economic issues include identify ing and assessing patterns of care, analyzing problems of financing, exploring impacts on markets other than health care, and modelling scenarios for future developments and strategies. At present, the eco nomic aspects of AIDS still constitute a very recent topic in European health economics and health systems research. Many projects are just about to start, and there must be a better exchange of information between research groups.

The book is unique in several ways: (1) it uses stochastic difference and differential equations to present the stochastic models of the HIV epidemic and HIV pathogenesis; in this sense, the deterministic models are considered as special cases when the numbers of different type of people or cells are very large (2) it provides, a critical analysis of deterministic and statistical models in the literature; (3) it develops state space models by combining stochastic models and statistical models; and (4) it provides a detailed discussion on the pros and cons of the different modeling approaches. This book is the first to introduce state space models for the HIV epidemic. It is also the first to develop stochastic models and state space models for the HIV pathogenesis in HIV-infected individuals.

The first eleven conferences addressed ecosystems, epidemiology, energy, environmental health, time series and ecological processes, energy and health, energy conversion and fluid mechanics, environmental epidemiology: risk assessment, atomic bomb survival data: utilization and analysis, modem statistical methods in chronic disease epidemiology and scientific issues in quantitative cancer risk assess ment. These Proceedings are a result of the twelfth conference on Statistical Methodology for Study of the AIDS Epidemic which was held in 1991 at the Mathematical Sciences Research Institute, Berkeley, California. For five days, 45 speakers and observers contributed their expertise in the relevant biology and statistics. The presentations were timely and the discussion was both enlightening and at times spirited. Members of the Program Committee for the Conference were Klaus Dietz (University of Tiibingen, Germany), Vernon T. Farewell (University of Waterloo, Ontario), and Nicholas P. Book excerpt: In March 1983 the Secretary of State for Welfare, Health and Cultural Affairs appointed the Steering Committee on Future Health Scenarios (STG). The task of the STG is to advise the Minister on the future of public health and health care in the long term in the Netherlands with a view to increasing the anticipatory capacity of government policy. Since 1983 the STG has set up a number of committees, including the Scenario Committee on AIDS, with a view to mapping out possible scenarios in their particular field of interest. The committees report in the form of scenario reports, which are of relevance not just to the national government but also to others involved in policy development and opinion formation in the field in question. What proportions will the HIV/AIDS epidemic assume and how many new HIV infections can be averted by HIV/AIDS prevention campaigns? How much care and support will people with HIV and AIDS need and what successes can be expected within the care sector? What will be the social consequences of AIDS and how can ostracism and the social isolation of people with HIV and AIDS be avoided? These and other related questions form the subject of this book, which examines the sociocultural and economic impact of AIDS in the Netherlands up to the year 2000. The book presents a study carried out by the Netherlands Institute of Mental Health (NcGv) and the National Institute of Public Health and Environmental Protection (RIVM), under the direction of the STG Scenario Committee on AIDS. The researchers look back at the epidemiological, sociocultural and economic impact of AIDS in the Netherlands during the first decade of the epidemic and, on the basis of this, use scenario analysis to project the impact of AIDS up to the year 2000. The future projections relate both to the impact of AIDS and the maximization of efforts to combat the disease. In addition to a scenario exploring the impact of AIDS if developments continue unchanged, alternative scenarios are also put forward. These focus on the impact of significant changes in such diverse fields as sexual behaviour, injecting drug use, the effectiveness and organization of HIV/AIDS prevention programmes, and the demand for delivery of care. The book concludes that the impact of AIDS will only become fully apparent in the second decade of the epidemic. The authors consider the implications of this conclusion for HIV/AIDS prevention programmes, research, care and the efforts to counter the socially undesirable impacts of AIDS.
