The diseases caused by trematodes have been neglected for years, especially as compared with other parasitic diseases. However, the geographical limits and the populations at risk are currently expanding and changing in relation to factors such as growing international markets, improved transportation systems, and demographic changes. This has led to a growing international interest in trematode infections, although factors such as the difficulties entailed in the diagnosis, the complexity of human and agricultural practices, the lack of assessments of the economic costs or the limited number of effective drugs are preventing the development of control measures of these diseases in humans and livestock. In-depth studies are needed to clarify the current epidemiology of these helminth infections and to identify new and specific targets for both effective diagnosis and treatments. The main goal of this book is to present the major trematodes and their corresponding diseases in the framework of modern parasitology, considering matters such as the application of novel techniques and analysis of data in the context of host-parasite interactions and to show applications of new techniques and concepts for the studies on digenetic trematodes. This book replaces a previous volume Schistosomiasis: Epidemiology, Treatment and Control (Heinemann, 1982) by Jordan and Webbe. All chapters have been rewritten by internationally renowned workers. Ultrasound, expected to aid identification of early disease in the field and increase our understanding of its evolution, is discussed in a new chapter. Others, each with an extensive bibliography, review the parasites and their snail intermediate hosts, epidemiology, clinical manifestations and pathology, diagnosis, immunology, drugs and patient management and control. These infections were known to the ancient Egyptians and their transmission shows little evidence of slowing down, globally. The good news is that field applicable, and increasingly affordable, chemotherapy has been available for almost 25 years. Using chemotherapy and other means of control, some countries have decreased transmission and made excellent headway against morbidity. The bad news is that the public health problems caused by schistosomiasis are still with us, with the estimated number of cases of schistosomiasis, while shifting geographically, remaining approximately 200 million for the last 30 years. In fact, with the development of field usable ultrasound technology and meta-analyses performed on existing data, there is a new appreciation for the extent of non-lethal morbidity associated with these infections. While the percentage of individuals with severe hepatosplenic disease remains below 10%, recent reassessments of morbidity associated with schistosomiasis indicate that the prevelance of symptoms and the cost in diability-adjusted life years is much greater than was previously, commonly appreciated (Van der Werf, M. J. , et al. 2003, Acta Tropica 86:125-139; Charles H. King, personnel communication). Strong impetus for addressing these issues is provided by the World Health Assembly’s recently passed Resolution 54. 19, which calls for efforts to reduce morbidity caused by schistosomiasis and soil-transmitted helminths in school-aged children, largely through chemotherapy campaigns.

For some decades, the Western hemi sphere's interest in animal parasites has been eclipsed by concern for bacteria and viruses as agents of transmissible disease. Only very recently, initiatives like the Tropical Disease Research programs of WHO-World Bank-UNDP, and the Great Neglected Disease networks of the Rockefeller and MacArthur Foundations have begun to compensate for the neglect of these problems by United States federal health research agencies. Throughout that period, how ever, the Rockefeller Institute (later University) has given high priority to the challenges of parasitism, corresponding during a formidable period with Dr. Trager's own career. The present work then, is a distillation of the insight collected by our principal doyen of parasite biology, informed but by no means confined to his own research. It is addressed to the reader of broad biological interest and training, not to the specialist. The disarmingly unpretentious style makes the work readily accessible to college undergraduates or even to gifted high school students; but do not be deceived thereby, as it has an enormous range of factual information and theoretical insight, familiar to few, but potentially important to most biologists.
