Currently, as a means of improving the health and healthcare availability in the Western Addition, Engage San Francisco and the USF School of Nursing and Health Professions is in the process of developing a integrated care clinic serving the health needs of the Western Addition. This paper will detail the 300-hour fieldwork experience working with both agencies to conduct a health needs assessment that will lay the foundation of the services that will be provided at the integrated care clinic.
