Book excerpt: This report presents the results of an FAO Expert Working Group Meeting “Scoping exercise to increase the understanding of risks of antimicrobial resistance (AMR) in aquaculture”. The meeting was attended by 14 experts from nine countries, representing intergovernmental organizations, academia, research institutions and the private sector. A risk profiling exercise was conducted on two bacterial pathogen groups (Streptococcus spp. and Vibrio parahaemolyticus) selected based on their importance to fish health and public health. Both bacterial agents affect tilapia, the second largest species group produced in aquaculture globally, which contributes significantly to global food and nutrition security. The risk profiling exercise for the two bacterial pathogens revealed that in both cases, the AMR risks posed by these pathogens were likely to be low and thus conducting a full risk assessment was not recommended. The risk profiling outlined in Codex Alimentarius was used as guidance, but it was recommended to review and adapt it as appropriate for aquatic AMR risk assessment. The Expert Group agreed to develop a project proposal to contribute to a multisectoral project "Towards reducing aquaculture-based AMR through a cross-sectoral approach". The project concept note will include investigation on two bacterial agents important to both animal and human health, namely: Streptococcus spp. and mesophilic aeromonads.

The objectives of this project were to develop and/or enhance the knowledge, skills and capacity of the participating Competent Authorities on fisheries and aquaculture, as well as to assist them in the development and implementation of policies and national action plans (NAPs) on the prudent and responsible use of antimicrobials. The project enhanced the capacities of national Competent Authority (technical specialists, inspection and laboratory staff) to enable productive engagement with other lead agencies (e.g. the World Health Organization [WHO], national agriculture, food safety and animal health authorities), particularly with respect to their aquaculture and fish food safety component contributions to the NAP and the integration of the aquatic sector within the One Health framework. The report also briefly summarizes the recent actions and activities taken by FAO related to AMR in aquaculture since the completion of this project, including awareness raising targeting policymakers and aquaculture stakeholders, relevant publications, candidate reference centers and other ongoing projects to date.

Book excerpt: Antimicrobial resistance (AMR) is the ability of microorganisms to fight antimicrobial compounds, reducing the efficacy of treating diseases in humans, animals, and plants. AMR risk is outpacing human population growth, owing to misuse of antimicrobials in large quantities in food systems, and is a serious threat to food security and sustainable development. FAO, with the World Health Organization (WHO), the World Organization for Animal Health (OIE), and the United Nations Environment Programme (UNEP), is supporting countries in developing and implementing their One Health National Action Plans on AMR. The eventual aim is to ensure sustainable use of antimicrobials to minimize AMR risks, in alignment with the Global Action Plan on AMR. The scope of the evaluation covers FAO’s entire work on AMR up to early 2020 and its role in the global AMR architecture. It examines FAO’s organizational and institutional set-up for AMR work. FAO has a strong mandate to work on AMR, implementing activities in 45 countries and providing far-reaching support on AMR National Action Plans (NAPs). FAO’s technical expertise is a key comparative advantage in its work on AMR. It is underpinned by the strong scientific grounding of FAO’s work, engendered in its AMR working groups and supported by its collaboration with research centers, universities, and the Tripartite organizations. Nevertheless, the work is relatively recent and, given the long impact pathways, it has had limited results. A comprehensive strategic and programmatic approach would increase the likelihood of achieving results in combating AMR. FAO should prioritize its work in a long-term strategy on AMR that recognizes the seriousness of the threat and is fully integrated into the Organization’s Strategic Framework. The strategy should set out FAO’s long-term role in combating AMR and that of its divisions and offices, as well as its approach at the country and regional level. FAO should consolidate its work on AMR through a strong programmatic approach with a central coordination and management structure that links with the Regional Offices and is supported by dedicated core funding.

Increasing temperatures, ocean warming and acidification, severe droughts, wildfires, altered precipitation patterns, melting glaciers, rising sea levels and amplification of extreme weather events have direct implications for our food systems. While the impacts of such environmental factors on food security are well known, the effects on food safety receive less attention. The purpose of Climate change: Unpacking the burden on food safety is to identify and attempt to quantify some current and anticipated food safety issues that are associated with climate change. The food safety hazards considered in the publication are foodborne pathogens and parasites, harmful algal blooms, pesticides, mycotoxins and heavy metals with emphasis on methylmercury. There is also, a dedicated section on the benefits of forward-looking approaches such as horizon scanning and foresight, which will not only aid in anticipating future challenges in a shifting global food safety landscape, but also help build resilient food systems that can be continually updated as more knowledge is assimilated. By building a more widespread and better understanding of the consequences climate change has on food safety, it is hoped that this document will aid in fostering stronger international cooperation in making our food safer by reducing the global burden of these concerns.

Finally, needs and challenges are assessed in the context of the findings from the data collected from the countries. Accordingly, retail and foodservice establishments, as well as food producers at all levels of the food production chain, have a growing responsibility to ensure that proper food safety and sanitation practices are followed, thereby, safeguarding the health of their guests and customers. Achieving food safety success in this changing environment requires going beyond traditional training, testing, and inspectional approaches to managing risks. It requires a better understanding of organizational culture and the human dimensions of food safety. To improve the food safety performance of a retail or foodservice establishment, an organization with thousands of employees, or a local community, you must change the way people do things. You must change their behavior. In fact, simply put, food safety equals behavior. When viewed from these lenses, one of the most common contributing causes of food borne disease is unsafe behavior (such as improper hand washing, cross-contamination, or undercooking food). Thus, to improve food safety, we need to better integrate food science with behavioral science and use a systems-based approach to managing food safety risk. The importance of organizational culture, human behavior, and systems thinking is well documented in the occupational safety and health fields. However, significant contributions to the scientific literature on these topics are noticeably absent in the field of food safety.

The sector is recognised as creator of jobs and an important source of foreign export earnings for both developed and developing countries. However, the expanded global movements of live aquatic animals have been accompanied by the transboundary spread of a wide variety of pathogens necessitating the routine use of veterinary medicines to prevent and treat disease outbreaks, assure healthy stocks and maximise production. The use of appropriate antimicrobial treatments is one of the most effective management responses to such emergencies, but their inappropriate use can lead to increased frequency of bacterial resistance, potential transfer of resistance genes in bacteria from the aquatic environment to other bacteria, and the occurrence of bacterial residues in aquaculture products resulting in bans by importing countries. The FAO/AAHRI Expert Workshop on Improving Biosecurity through Prudent and Responsible Use of Veterinary Medicines in Aquatic Food Production was convened in Bangkok, Thailand, from 15 to 18 December 2009, to understand the current status of the use of antimicrobials in aquaculture, and to discuss the concerns and impacts of their irresponsible use on human health, the aquatic environment and trade. The discussions became the basis for drafting recommendations targeted at government and private sectors, and for developing guiding principles on the responsible use of antimicrobials in aquaculture to be considered as part of future FAO Code of Conduct for Responsible Fisheries (CCRF) Technical Guidelines for Responsible Fisheries on Prudent and Responsible Use of Veterinary Medicines in Aquaculture. Part 1 of this publication contains 15 technical background papers presented during the expert workshop, contributed by 29 specialists. Part 2 contains the highlights of the expert workshop. Because aquaculture is expected to continue to increase its contribution to the world's production of aquatic food, offer opportunities to alleviate poverty, increase employment and community development, and reduce overexploitation of natural aquatic resources, appropriate guidance to aquaculture stakeholders on the responsible use of veterinary medicines has become essential. Safe and effective veterinary medicines need to be available for efficient aquaculture production, and their use should be in line with established principles on prudent use to safeguard public and animal health. The use of such medicines should be part of national and on-farm biosecurity plans and in accordance with an overall national policy for sustainable aquaculture.
