Significant quantities of both arginine and methionine are required for de novo creatine synthesis to satisfy whole body accretion requirements, even when creatine is provided in milk. Neonates that require intravenous feeding (parenteral nutrition, PN) are at risk for arginine deficiency because arginine is synthesized in the small intestine from dietary precursors; as such, arginine deficiency may limit creatine synthesis. L-arginine:glycine amidinotransferase (AGAT) is the first enzyme, and the rate limiting step, involved in creatine biosynthesis and catalyzes the conversion of arginine and glycine to guanidinoacetic acid (GAA) in the kidney. GAA is then methylated by methionine in the liver to produce creatine via guanidinoacetate N-methyltransferase. Our objective was to determine whether a parenteral diet supplemented with GAA or creatine could spare arginine and methionine for protein synthesis and growth. Piglets (6-10 d old, N = 31) were parenterally fed for 5 days one of five diets: 1) low arginine and low methionine (base), 2) base plus GAA, 3) base plus creatine, 4) high arginine and high methionine (Arg&Met), or 5) high methionine and low arginine plus GAA (Met&GAA). On day 6 of the experiment, piglets underwent a 6-hour constant infusion of stable isotopes of arginine, GAA and creatine to measure fractional conversion of arginine to GAA and creatine. In addition, continuous infusions of phenylalanine and tyrosine isotopes were used to measure whole body protein kinetics. Book excerpt: Methionine is an indispensable amino acid in the diet of neonates and has a metabolic role in the synthesis of creatine, phosphatidylcholine (PC) as well as protein synthesis and other transmethylation reactions. Creatine synthesis consumes significant amounts of arginine (a conditionally essential amino acid) and methionine, while PC requires methionine for its synthesis. Total parenteral nutrition (TPN) is typically marginal in these amino acids, and devoid in creatine. Therefore, our objective was to quantify the partitioning of methyl groups among transmethylation reactions and the sparing effect of dietary creatine and GAA on protein synthesis. Adequate supply of dietary methionine resulted in significantly greater hepatic fractional synthesis rate (FSR) of PC. Furthermore, GAA supplementation increased FSR of creatine by approximately 175%, but only when adequate dietary methionine was supplied. Moreover, piglet weight gain was similar to that of piglets receiving adequate arginine, suggesting arginine was spared for growth. Overall, supplemental GAA could be considered as a novel TPN ingredient to increase the synthesis of creatine and spare arginine for growth, but only when adequate methionine is provided.

Arginine transfers its amidino group to glycine to form guanidinoacetic acid (GAA) which is then transmethylated to creatine. Methionine is the primary methyl donor for transmethylation reactions via S-adenosylmethionine (SAM). SAM is demethylated to S-adenosylhomocysteine and transfers its methyl group to synthesize creatine, phosphatidylcholine (PC) and methylated DNA. In rapidly growing animals, the balance between requirements for growth versus those for maintenance becomes more critical as growth demands more amino acids for expansion of body protein mass as well as for the increased need for other critical metabolites. Therefore, our hypothesis is that dietary creatine and its precursor GAA can spare methionine and arginine. L-[methyl-3H] methionine was infused to measure the sparing effect of dietary creatine and GAA and to quantify the partitioning of transmethylation reactions. The fractional synthetic rate of creatine, PC, DNA and protein were measured using isotope kinetics. Creatine supplementation increased the availability of methyl groups for other transmethylation reactions by reducing the labile methyl groups needed for creatine synthesis. GAA supplementation increased hepatic creatine levels 5-fold, but only when methionine was not limited. Excess dietary methionine enhanced PC synthesis by double but not hepatic protein synthesis. Sparing methionine with creatine or supplementing methionine did not increase hepatic protein synthesis, suggesting that hepatic protein synthesis is conserved in neonates when methionine is limiting. Therefore, creatine levels in neonatal piglets can be maintained only when the supply of methyl groups is expanded in the diet.
