L’extraction de l’incisive mandibulaire est rarement envisagée, mais elle constitue une option intéressante dans des cas bien définis. Les indications de ce type de traitement regroupent les impératifs orthodontiques visant à traiter une dysharmonie dento-maxillaire, une dysharmonie dento-dentaire par excès mandibulaire et d’autres cas particuliers, ainsi que des nécessités plus générales, selon l’état de la dent et de son environnement parodontal. L’étude détaillée de ces indications permet de mieux cibler l’indication de traitement. Les contre-indications existent également et sont importantes à connaitre. Le but est de pouvoir tirer partie de tous les avantages du recours à une telle méthode en termes de simplification et de gain de temps. Ces avantages sont particulièrement intéressants aux yeux de la patientèle adulte dont la consultation est fréquente pour des encombrements antérieurs mandibulaires. Cette technique présente malgré tout des inconvénients qu’il convient de connaître. La mise en oeuvre de cette technique est détaillée dans les étapes de la conduite à tenir. L’importance d’un diagnostic soigneux est mise en avant, tout comme le choix de l’incisive à extraire. Les finitions sont très importantes pour garantir le résultat et la stabilité du traitement. Face à un encombrement mandibulaire antérieur, il faudra tenir compte de plusieurs paramètres tels que l'anatomie dentaire, la valeur de l'encombrement, la dysharmonie dento-dentaire et l'hygiène. L'extraction de l'incisive mandibulaire et la réduction amélaire inter-proximale sont des thérapeutiques offrant une bonne alternative en circonscrivant les mouvements au niveau antérieur et de ce fait diminuent le temps de traitement par rapport à un traitement avec extraction de prémolaires. Dans ce travail seront abordés les avantages, inconvénients et indications de ces deux procédures orthodontiques, ainsi que le déroulement du plan de traitement qui en découle et enfin des notions primordiales de récidive et stabilité. Ce cheminement nous permettra d'effectuer une revue de littérature en 2019 sur la comparaison et l'intérêt de ces deux procédures.
