Book excerpt: Introduction : Les anticoagulants oraux directs (AOD) tendent à remplacer les antivitamines K (AVK) depuis leur mise sur le marché. Cependant, l'incidence des hémorragies majeures et mineures est plus élevée chez les femmes comparativement aux hommes ; cette différence est potentiellement en lien avec les hémorragies génitales. Les médecins généralistes sont les premiers médecins consultés dans le cadre du parcours de soin coordonné et il existe peu de données de la littérature concernant les hémorragies génitales chez les femmes jeunes sous anticoagulant. L'objectif de cette étude était d'évaluer les pratiques et la perception des médecins généralistes lors de l'utilisation d'un traitement anticoagulant oral chez la femme en âge de procréer. Matériel et méthodes : Il s'agissait d'une étude descriptive observationnelle transversale, monocentrique. Un questionnaire comprenant 11 questions a été envoyé à 900 médecins généralistes des Bouches-du-Rhône, du Var et du Vaucluse randomisés par tirage au sort aléatoire via les Pages Jaunes®. Ce questionnaire évaluait le type de patientèle, l'anticoagulant le plus prescrit, la prise en charge des hémorragies génitales et l'évaluation de la qualité de vie des patientes sous anticoagulant. Résultats : Les AOD étaient les anticoagulants les plus prescrits selon les médecins généralistes interrogés Les hémorragies génitales étaient la 2e cause d'hémorragie mineure relatée. La plupart des médecins généralistes (60.6%) estimaient que les méno-métrorragies étaient imputables aux AVK. Près d'un quart des médecins rapportaient une altération de la qualité de vie des patientes anticoagulées alors que seuls 47.5% d'entre eux abordaient systématiquement le sujet des hémorragies génitales en consultation. Discussion : D'après les données de la littérature, les hémorragies génitales sont le plus souvent imputables aux AOD contrairement à nos résultats ; ces résultats sont probablement liés à une extrapolation, par les médecins généralistes, des hémorragies génitales aux hémorragies tout site confondu liées aux AVK. Actuellement, il n'existe pas de recommandation concernant la prise en charge des hémorragies génitales chez ces femmes jeunes. Au terme de notre étude, un algorithme est proposé pour la prise en charge de ces méno-métrorragies. Les hémorragies génitales sont trop peu fréquemment évoquées en consultation et probablement sous diagnostiquées. Conclusion : Notre étude suggère qu'il existe une altération de la qualité de vie chez les patientes en âge de procréer traitées par anticoagulant présentant des hémorragies génitales ; cette dernière est sous-estimée car il s'agit d'un sujet tabou, rarement évoqué en consultation. Une information médicale devrait être diffusée à large échelle afin qu'une conduite à tenir adaptée puisse être adoptée par les médecins.

Book excerpt: Les anticoagulants oraux direct (AOD) ont fait leur apparition dans l'arsenal thérapeutique du médecin visant à remplacer les antivitamines K (AVK) et ont vu leur prescription rapidement augmenter. Cependant les médecins généralistes semblaient réticents à les prescrire. L'objectif de cette étude était de recueillir leur opinion, leur ressenti vis-à-vis des AOD afin d'identifier les causes de cette réticence. Nous avons réalisé une étude qualitative par focus groups auprès de 19 médecins généralistes des départements du Rhône, de la Drôme et de l'Ardèche entre novembre 2016 et Mars 2017. Les médecins généralistes sont principalement amenés à renouveler les traitements par AOD initiés par un confrère spécialiste. Cette tâche n'est pas toujours aisée du fait de difficultés techniques de communication. Les principaux freins à la prescription d'AOD par les médecins généralistes sont : la crainte du risque hémorragique majorée par l'absence d'antidote disponible pour chaque AOD, le manque de recul, la balance bénéfice risque jugée défavorable, les réticences des patients, des autorités sanitaires et de la presse médicale indépendante. Plusieurs facteurs facilitant la prescription d'AOD ont été identifiés : la facilité d'emploi, la posologie fixe et l'absence de surveillance biologique apportant du confort pour le patient et le médecin. Les médecins généralistes sont plus enclins à prescrire les AOD quand la décision a été partagée avec le confrère spécialiste et avec le patient. Une meilleure collaboration entre les professionnels de santé passant par des formations communes et une amélioration des moyens de communication permettrait une prescription plus sereine de ces molécules.
