These results were obtained by loss of heterozygosity (LOM) studies in breast tumors. Metastasis and poor outcome are the cancer phenotypes associated with lip' 5.5. Since breast cancer patients usually do not die as a result of the primary tumor, but from the progression to a metastatic phenotype, we have embarked on a study to identify% the gene(s) responsible on lip15.5. We have constructed a detailed physical map across the region, participated in sequencing the region, generated a transcript map across the region and have begun to access the genes in this region as candidates for tumor suppressor genes. The candidate region has been reduced to 400 kb and contains about 4-6 potential genes. We have cloned the region and the molecular characterization has begun. Interestingly, the 11p15.5 .5 region has been shown to be located in an imprinted domain. We have evidence to suggest preferential 11pl 5.5 maternal loss in breast tumors suggesting that breast cancer in this region has an imprinting component. We have cloned the homologous imprinted region from the mouse and are testing for imprinted expression in fetal and adult tissues including mammary gland.

It has been proven to detect breast cancer at an earlier stage and to reduce the number of women dying from the disease. However, it has a number of limitations. These current limitations in early breast cancer detection technology are driving a surge of new technological developments, from modifications of x-ray mammography such as computer programs that can indicate suspicious areas, to newer methods of detection such as magnetic resonance imaging (MRI) or biochemical tests on breast fluids. To explore the merits and drawbacks of these new breast cancer detection techniques, the Institute of Medicine of the National Academy of Sciences convened a committee of experts. During its year of operation, the committee examined the peer-reviewed literature, consulted with other experts in the field, and held two public workshops. In addition to identifying promising new technologies for early detection, the committee explored potential barriers that might prevent the development of new detection methods and their common usage. Such barriers could include lack of funding from agencies that support research and lack of investment in the commercial sector; complicated, inconsistent, or unpredictable federal regulations; inadequate insurance reimbursement; and limited access to or unacceptability of breast cancer detection technology for women and their doctors. Based on the findings of their study, the committee prepared a report entitled Mammography and Beyond: Developing Technology for Early Detection of Breast Cancer, which was published in the spring of 2001. This is a non-technical summary of that report.

The spread of a primary tumor to colonize neighboring and distant organs is the relentless endgame that defines the neoplastic process. Patients who have been diagnosed with cancer are treated to prevent both the recurrence of the tumor at the site of origin and metastasis that would re-stage them as advanced stage IV cancer. Historically and still with some types of cancer, stage IV is perceived by patients as “terminal.” Fortunately, recent molecular therapies have extended the lives of patients with advanced cancer and reassuringly people living with metastatic disease increasingly visit our clinics. What is the path forward? Given that the consilience of science and medicine is a dynamic art from which therapies arise, it would be misguided to consider any single work adequate at capturing the horizon for research. So with humility we constructed this text as primer for scientists. It begins with a broad introduction to the clinical management of common cancers. This is intended to serve as a foundation for investigators to consider when developing basic science hypotheses. Unquestionably, medical and surgical care of cancer patients reveals biology and dictates how novel therapeutics will ultimately be evaluated in clinical trials. The second section of this text offers provocative and evolving insights that underscore the breadth of science involved in the elucidation of cancer metastasis biology.
