However, this part of the body is very often neglected during clinical assessment of the illness in favour of the articulations of the upper limbs, knees and hips. Nevertheless, it appears evident that a detailed clinical examination of the forefoot, ankle and heel could be useful in order to get closer to a rheumatological diagnosis. Thanks to blood tests and conventional imaging, completed with a scanner or MRI if needed, every doctor should be in a position to detect the beginning of rheumatoid arthritis in the foot or ankle. As well as its diagnostic contribution, this book broaches the theme of the most recent medicinal treatments, that of orthoses, made-to-measure shoes (the prescription of which practitioners and the rheumatologists often hand over to the orthopaedic surgeons), and finally that of local injections. These elements constitute the main treatment of the rheumatoid foot. Taking care of the ill subject does not necessarily stop there and a surgeon may need to operate on the forefoot, which is more common, or on the mid-foot or the ankle.

International experts have given their opinion on (i) the kinematics of the ankle; (ii) the basics of replacing the ankle joint; and (iii) the current status of several modern ankle-prosthesis designs. A better understanding of the ankle kinematics, alignment techniques and biological stabilisation and the results of newer designs have changed the former attitude worldwide. First, because I know the extent of their personal experience, from which they have selected the best for this book. Second, because it seemed to me that their detailed analysis of the numerous works on the subject, works which have been published all over the world and which provide diverse physiopathologic interpretations, would provide a comprehensive study meeting a real need. Algodystrophy, to adopt the term used by the authors, merits rheumatolog ists' careful attention. It is indeed a frequent condition and, as is now well known, occurs in the most varied etiologic circumstances; it is not solely posttraumatic, a notion on which diagnosis had long been based. This variable etiology suggests the complexity of algodystrophy's pathogenic mechanism.
