Book excerpt: La 4e de couverture indique : "Les cancers des voies aéro-digestives représentent une part importante des cancers en France en 2014 (5ème type de cancer). Leur étiologie repose essentiellement sur une intoxication éthylo-tabagique associée à d’autres facteurs (exposition chimique, environnement, port d’une prothèse mal adaptée,...). Ils sont traités principalement par radiothérapie. Le traitement consiste en l’utilisation d’une dose suffisante de rayonnement ionisant, afin de détruire la tumeur. Le rayonnement atteint également les zones voisines de la tumeur et entraîne des effets secondaires généraux (aplasie médullaire, asthénie, perte des poils et cheveux,...) mais également des effets sur la sphère cervico-faciale (radiomucite, hyposialie, caries post-radiques, ostéoradionécrose,...). Les caries post-radiques sont une des conséquences du traitement anti-cancéreux. Elles résultent d’une modification de la qualité (pH, ions thyocianate, anti-corps) et de la quantité de salive produite par le patient. La réduction de l’ouverture buccale, le manque d’hygiène bucco-dentaire et la modification du ratio déminéralisation/reminéralisation favorise leur apparition, généralement plusieurs mois après la radiothérapie. La fluoroprophylaxie est le moyen le plus efficace dans la prévention des caries post-radiques. Cette mesure doit être appliquée dès le début du traitement et poursuivie à vie pour limiter les effets de la radiothérapie. Le rôle du chirurgien-dentiste est de dépister les lésions pré-cancéreuses (kératoses tabagiques, lichen plan,...) et cancéreuses lors des rendez-vous au cabinet du patient mais il joue un rôle aussi dans la prévention et le traitement des lésions causées par la radiothérapie. Il a été démontré qu’un suivi régulier par le praticien et l’assiduité du patient à maintenir une hygiène bucco-dentaire correcte permettaient la limitation des caries post-radiques et la réapparition de lésions cancéreuses."
