Available in PDF, EPUB and Kindle. Book excerpt: Abstract Background: The impending exit of the aging nursing workforce, especially nurse leaders, poses a great concern to healthcare executives. Organizations must take action to lessen the impact of nurse leader turnover. Interventions: The author developed a mentoring plan and a curriculum for leader development, based on the Essentials of Nurse Manager Orientation adopted from the American Organization for Nursing Leadership (AONL) and American Association of Critical Care Nurses (AACN), for assistant nurse managers (ANM) who are new to the role. The curriculum was implemented for two cohorts of nurse leaders within the first two years of their hire date. Each participant was assigned a mentor, who they met with regularly and served as a resource. Measures: The AONL/AACN Nurse Manager Inventory Tool self-assessment of competency and a 23-question demographic and satisfaction survey were completed by the participants before and after the learning development and mentoring sessions. Results: The participants reported a mean raw score improvement of 1.20, a 59% improvement, for all competency areas of the Nurse Manager Inventory Tool four to six months after the first learning session and mentoring activity. The results showed an increase in the ANM satisfaction with the on-boarding process from a 6.3 to a 6.7 score and an increase in overall ANM satisfaction with their jobs from 6.4 to a 6.5 score on a 7-point Likert scale. Conclusion: Succession planning is a deliberate strategy taken by organizations to develop the workforce to ensure smooth transition and stability in leadership to support the frontline staff in ensuring safe and quality patient care. Creation and implementation of a succession plan that aligns with the organization’s strategic plan can improve both retention and organizational stability. This applied doctoral project presents case study in which three deliverables for succession planning, grounded in servant leadership, were produced for nursing leadership within a federal healthcare facility. A succession plan was built from the ground up through informant interviews, working group input, and feedback to determine essential leadership skills and tools to quantify needs for nursing leadership positions. A targeted presentation to key stakeholders on the resulting plan was delivered. Finally, using feedback from the stakeholder presentation, a succession planning toolkit was produced for use by other facilities.

This academic medical center was increasing bed-capacity and needed to hire 12 additional nurse managers (NM) to meet these new requirements. Promoting assistant nurse managers (ANM) to the NM role saves time and money, supports leadership and cultural continuity, and demonstrates a strong organizational commitment to internal human capital. Context. Formal nurse manager succession planning (NMSP) programs have been shown to increase nurse manager competency and retention rates, while also helping to identify and develop new generations of nurse leaders. To mitigate the recent loss of ANMs and strengthen the remaining leadership bench strength, the organization is developing, implementing, and evaluating a formal NMSP program. Interventions. The Doctor of Nursing Practice (DNP) project included the development, implementation, and evaluation of a formal NMSP program for the nurse manager role, with the aim of decreasing turnover and filling NM positions with internal ANM candidates. Measures. Comprehensive NMSP project evaluation and outcome measures were improved ANM retention and internal promotions to the nurse manager role, as well as improved candidate perceptions of leadership and management skill competency post leadership development program using a valid and reliable tool. Results. Comprehensive NMSP program evaluation showed increased ANM retention and internal promotions to the nurse manager role, as well as improved candidate perceptions of leadership and management skill competency post leadership development program. Conclusion. The increased retention and internal promotion outcomes of this DNP-led evidence-based project suggest that formal and deliberate succession planning, along with individualized developmental plans has positive implications for preparing future NMs.

The literature suggests a lack of existing standardized succession plans for these leaders. Senior leaders are often faced with the complex issue of how to develop and validate the competencies required for these leadership roles (Dyess, Sherman, Pratt, & Chiang- Hanisko, 2016). Therefore, in the absence of a succession plan, creating a leadership development program, which provides a pipeline of new executive leaders, would alleviate this gap. The motivating force is an effort to promote current talent and provide a higher level of competence. The American Organization of Nurse Executives0́9 (2015) nurse executive competencies were used to identify behavioral attributes necessary for successful leadership performance. Evidence-based simulation was chosen as the modality to educate and evaluate potential leaders, allowing for a leadership development program designed to strengthen and enhance clinical judgment while providing cost savings to the organization (Bleich, 2015). The evidence-based intervention for this project was composed of two parts. Part one was development of three simulation scenarios that were validated and beta tested by DNP students. Part two was the development of a toolkit for the California Simulation Alliance leadership repository, including a scenario on gravitas and a process of implementation and evaluation of the success of participants. Creating a leadership development program for succession planning using simulation is valuable for learning, practice, and proficiency for the individual; improving metrics; and stabilizing the culture and cost savings to the organization.

Aging nurse leaders are leaving the profession and thus are creating gaps which need to be filled (Stichler, 2008). The unavailability of respective schools and negative perceptions of nurses regarding the healthcare sector leads to the lack of interest in the nursing profession (Nelsey, Brownie, 2012). Factors contributing to burn-out in nursing departments affect the intentions of nurses to leave and lower satisfaction towards their profession. The proposed succession plan will act as a resource to identify features that affect staff morale, clinical competence and staff commitment to their jobs. The current project also addresses the nurse0́9s interest in future leadership roles within the organization. This project allows the implementation and development of strategies to ensure successful succession of nurses in healthcare. The Plan-Do-Study-Act (PDSA) evaluation model is applied to this project to evaluate iterative analysis of change to improve succession methods in healthcare. This project has identified de-motivation, inexperienced staff, lack of corporation or teamwork and increased workload as possible problems that offer obstacles in implementing the succession plan.
