Book excerpt: Concerns have been raised about whether workers exposed to engineered nanoparticles are at increased risk of adverse health effects. The current body of evidence about the possible health risks of occupational exposure to engineered nanoparticles is quite small. While there is increasing evidence to indicate that exposure to some engineered nanoparticles can cause adverse health effects in laboratory animals, no health studies of workers exposed to the few engineered nanoparticles tested in animals have been published. The purpose of this document from the National Institute for Occupational Safety and Health (NIOSH) is to provide interim guidance about whether specific medical screening, including performing medical tests on asymptomatic workers, is appropriate for these workers. Medical screening is only one part of what should be considered a complete safety and health management program. An ideal safety and health management program follows a hierarchy of controls and involves various occupational health surveillance measures. Since specific medical screening of asymptomatic workers exposed to engineered nanoparticles has not been extensively discussed in the scientific literature, this document makes recommendations based upon what is known until more rigorous research can be performed. Currently there is insufficient scientific and medical evidence to recommend the specific medical screening of workers potentially exposed to engineered nanoparticles. Nonetheless, this lack of evidence does not preclude specific medical screening by employers interested in taking precautions beyond existing industrial hygiene measures. If nanoparticles are composed of a chemical or bulk material for which medical screening recommendations exist, these same screening recommendations would be applicable for workers exposed to engineered nanoparticles as well. As research into the hazards of engineered nanoparticles continues, vigilant reassessment of available data is critical to determine whether specific medical screening is warranted for workers. In the interim, the following recommendations are provided for workplaces where workers may be exposed to engineered nanoparticles in the course of their work: Take prudent measure to control exposures to engineered nanoparticles; Conduct hazard surveillance as the basis for implementing controls; Continue use of established medical surveillance approaches. Concerned individuals from government, industry, labor, and academia, together with occupational health professionals and medical personnel, have raised questions about whether workers exposed to engineered nanoparticles should be provided some type of medical surveillance. The purpose of this document is to provide interim guidance concerning specific medical screening for these workers—that is, medical tests for asymptomatic workers—until additional research either supports or negates the need for this type of screening. The type and degree of screening recommended here is in addition to any medical surveillance taking place as part of existing occupational health surveillance efforts.

Further, because many pregnant women are not tested for HIV and therefore do not receive treatment, the number of children born with HIV is still unacceptably high. What can we do to eliminate this tragic and costly inheritance? In response to a congressional request, this book evaluates the extent to which state efforts have been effective in reducing the perinatal transmission of HIV. The committee recommends that testing HIV be a routine part of prenatal care, and that health care providers notify women that HIV testing is part of the usual array of prenatal tests and that they have an opportunity to refuse the HIV test. This approach could help both reduce the number of pediatric AIDS cases and improve treatment for mothers with AIDS. Reducing the Odds will be of special interest to federal, state, and local health policymakers, prenatal care providers, maternal and child health specialists, public health practitioners, and advocates for HIV/AIDS patients. Book excerpt: The Former Hanford Worker Medical Monitoring Program, directed by the Occupational and Environmental Medicine Program at the University of Washington, served former production and other non-construction workers who were potentially exposed to workplace hazards while working for the USDOE or its contractors at Hanford. The USDOE Former Workers Program arose from Congressional action in the Defense Authorization of 1993 (Public Law 102). Section 3162 stated that, "The Secretary shall establish and carry out a program for the identification and ongoing medical evaluation of current and former Department of Energy employees who are subject to significant health risks as a result of exposure of such employees to hazardous or radioactive substances during such employment." (This also covers former employees of USDOE contractors and subcontractors.) The key objective has been to provide these former workers with medical evaluations in order to determine whether workers have experienced significant risk due to workplace exposure to hazards. Exposures to asbestos, beryllium, and noise can produce specific medical conditions: asbestosis, berylliosis, and noise-induced hearing loss (NIHL). Each of these conditions can be identified by specific, non-invasive screening tests, which are widely available. Treatments are also available for individuals affected by these conditions. This project involved two phases. Phase I involved a needs and risk assessment, characterizing the nature and extent of workplace health hazards which may have increased the risk for long-term health effects. We categorized jobs and tasks by likelihood of exposures to specific workplace health hazards; and located and established contact with former Hanford workers. Phase II involved implementation of medical monitoring programs for former workers whose individual work history indicated significant risk for adverse health effects. We identified 118,000 former workers, employed from 1943 to 1997. After excluding current workers, construction workers, and deceased workers, the total estimated number of former workers eligible for screening was 72,611. By September, 2006, 53,010 workers had been contacted, 20,298 responded, 2,835 were eligible and authorized, and 2,773 workers were ultimately screened. The cohort was 80% male, 85% white, and had a mean age of 63 years (range 24-96 years) at the time of first exam. Participants completed an occupational health history survey prior to the medical exam. Former Hanford workers were considered eligible for an exam if they reported exposure to asbestos, beryllium, or noise, or if a review of their Hanford work history indicated possible or probable exposure to one of these three hazards. We also invited any former Hanford worker who requested an exam to participate, regardless of documentation of exposure. The screening exam included a problem-focused physical exam, along with screening tests for one or more of three specific medical conditions: asbestosis (chest X-ray and spirometry), berylliosis (chest X-ray, spirometry, and beryllium-induced lymphocyte proliferation test), and NIHL (audiometry). We assisted ill workers in filing appropriate workers' compensation claims, and facilitated appropriate follow-up medical care. This program has made an important contribution to the health of former DOE contractor workers at the Hanford defense nuclear site.
