The book demonstrates that having one or more uninsured members in a family can have adverse consequences for everyone in the household and that the financial, physical, and emotional well-being of all members of a family may be adversely affected if any family member lacks coverage. It concludes with the finding that uninsured children have worse access to and use fewer health care services than children with insurance, including important preventive services that can have beneficial long-term effects.

Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

Book excerpt: As the nation expands the availability of health insurance to more Americans, it is essential to consider the large number of individuals who are already eligible for public health insurance but do not have it. In New York State 36 percent of uninsured adults -- or a total of 820,000 people -- are currently eligible for one of New York's public coverage programs (Medicaid or Family Health Plus) but unenrolled. New York State has committed itself to finding and enrolling all uninsured New Yorkers who are currently eligible for coverage, and there have been numerous studies of this population. However, these studies have looked either at the population as a whole or at families and children specifically. While there are good reasons to focus on children and families, little is known about childless adults who are eligible for public health insurance but not enrolled in it. This is a qualitative study, conducted in New York City, designed to understand what the barriers are for this population in particular and to identify strategies and policy changes to address these barriers. The findings will be particularly useful now that eligibility for public programs has been expanded and incentives to increase participation have been created through the health care reform act recently signed into law.
