Book excerpt: La réhabilitation prothétique fixée d'un maxillaire édenté représente un véritable challenge pour le chirurgien dentiste qui devra gérer les déficit osseux. L'utilisation d'implants zygomatiques représente une des solutions thérapeutiques possibles. Néanmoins, la fosse infra temporale, la cavité orbitaire et le sinus maxillaire sont des obstacles anatomiques à considérer dans ce type de chirurgie. D'une part, les données bibliographiques plaident en faveur des implants zygomatiques dont l'utilisation permet de se prémunir des risques associés aux chirurgies pré implantaires avec prélèvement osseux. De plus, une revue de la littérature actuelle montre un de succès implantaire de l'ordre de 97,2%, pour les 2400 implants zygomatiques suivis sur une période allant de 6 mois à 12 ans. D'autre part, notre étude expérimentale ex-vivo, qui est la seule à ce jour à comparer des manipulations réalisées à main levée et en chirurgie guidée, montre des résultats très satisfaisants en terme de stabilité implantaire (90% parmi l'ensemble des 20 implants posés sur 5 pièces anatomiques formolées). L'utilisation d'un guide chirurgical ) appui muqueux, associé à une nouvelle méthode de calcul des déviations entre la planification et la réelle position implantaire, montre une diminution significative du risque de lésion par rapport aux chirurgies réalisées à main levée. Les déviations maximales mesurées au cours de notre étude sont en deçà de celles obtenues dans les études similaires. Au vue des données dont nous disposons, l'implant à ancrage zygomatique semble être une technique fiable et reproductible, qu'il faudra toutefois mettre à l'épreuve du temps.

Book excerpt: Les implants zygomatiques étaient initialement indiqués lors de délabrements maxillo-faciaux, consécutifs de pathologies tumorales ou de séquelles des fentes labio-alvéolo-palatines. Leurs indications se sont ensuite étendues aux patients édentés totaux ou partiels présentant une atrophie sévère du maxillaire. L'implantologie zygomatique constitue une alternative aux techniques chirurgicales d'augmentation osseuse. Elle permette d'aboutir à des réhabilitations prothétiques fixes implanto-portées en réduisant significativement les durées de traitement et la morbidité des prélèvements osseux (calvaria, iliaque). Néanmoins, il s'agit d'une technique difficile avec un risque non négligeable de complications lié en partie à la complexité des structures anatomiques adjacentes. Les complications peropératoires majeures sont la perforation du plancher orbitaire et l'effraction au sein de la fosse infra-temporale. D'autres complications surviennent en post-opératoires, entre autres les sinusites, les fistules oro-antrales, ou encore les échecs implantaires et prothétiques. Une modification du protocole chirurgical initial, l'utilisation d'une nouvelle méthode d'évaluation des profils anatomiques, et enfin l'utilisation des nouvelles technologies ont permis une gestion plus rigoureuse des risques potentiels de complications. Le développement de la chirurgie guidée et de la navigation chirurgicale contribuera de façon certaine à une réduction significative des complications per et post opératoires dans cette implantologie de recours. La connaissance approfondie de l'anatomie cranio-faciale et l'expérience chirurgicale reste néanmoins des éléments garants du succès de cette thérapeutique.

La perte d'éléments dentaires est à l'origine d'une résorption progressive et centripète de l'os maxillaire. En cas d'atrophie sévère de ce dernier, plusieurs solutions pourront être avancées, telles les greffes osseuses ou le sinus lift. Cependant, ces techniques ne sont pas toujours envisageables et nécessitent souvent des temps d'attente longs et une temporisation difficile. En fonction de la demande et des besoins du patient, les implants zygomatiques constituent une alternative thérapeutique efficace et sûre, permettant une temporisation fixe tout en évitant les greffes osseuses. En plus d'une connaissance anatomique fondamentale, cette chirurgie nécessite des compétences et habitudes de travail, afin d'allier précision et reproductibilité. Cette thèse détaille dans un premier temps les différentes techniques chirurgicales permettant la mise en place d'implants zygomatiques, puis porte sur une étude rétrospective incluant 86 patients, dont le but est de comparer l'approche ZAGA avec la technique extra-maxillaire. Cette étude évalue les critères anatomiques, cliniques et chirurgicaux permettant de déterminer le succès implantaire, la survenue de complications et la satisfaction des patients pour chacune des cohortes. Finalement, elle ouvre une discussion, notamment sur les complications associées aux différentes techniques et sur les limites de l'étude.
