Book excerpt: This Public Expenditure Review (PER) is the result of collaboration among UNICEF, the World Bank Group, and Lesotho's Ministries of Health and Finance. Senior economists from the Clinton Health Access Initiative provided invaluable support in analyzing health expenditure data. The PER covers a five-year period, from fiscal years (FY) 2011/12 to 2015/16 (in Lesotho, the fiscal year runs from April 1 through March 30). The report describes and analyzes expenditure patterns in Lesotho's public health sector using multiple data sources. Focusing on the Ministry of Health and Social Welfare's (MoH) expenditure, the report identifies opportunities to improve the efficiency, equity, and effectiveness of financial resource use. To present a clear view of health expenditure, the report disaggregates total expenditure in the health system and describes its allocation across cost centers, districts (10 of them in Lesotho), levels of care, and health facilities (hospitals and primary health centers). Specific focus is on allocation of expenditure by economic classifications, and across outsourced health service providers.

Although education spending is one of the highest in the world as a percentage of GDP (13.5 percent of GDP) and universal access to primary education enshrined in law, poor educational outcomes persist across the country. Lesotho is also not on track to achieve the two education-related MDGs, with both primary enrollment (82 percent) and primary completion (65 percent) rates lagging far behind the goal of 100 percent by the end 2015. The report is organized as follows. The first chapter offers a detailed assessment of the overall sectoral budgeting and expenditure patterns in the education sector over a five-year period, from fiscal years (FY) 2011-12 to 2015-16, using multiple data sources. The second chapter estimates the cost of expanding secondary education to achieve the government's goal of universal compulsory lower basic education by 2020 and makes recommendations on how to better utilize the funding for the sector taking into account the fiscal constraints at macro level.

Lesotho is facing a tough macro-fiscal outlook due to a sharp decline in Southern African Customs Union (SACU) revenues. This situation necessitates a significant adjustment in the current fiscal stance to ensure longer-term fiscal sustainability. However, the adjustment should be tailored to minimize any adverse growth and poverty impacts. Thus, this PER is intended to support the government's efforts to adjust its policies to better address Lesotho's current macro-fiscal circumstances. Lesotho is one of the poorest and most unequal countries in the world, despite a relatively good growth performance over the past 15 years. Lesotho's per capita gross national income is about 1550 US dollars. Lesotho's poverty rate is 59 percent (1.90 US dollars purchasing power parity [PPP] per day), its Gini coefficient is 0.541, and about 59 percent of the population now lives below the international poverty line of 1.90 dollar/day. Both poverty and extreme poverty disproportionately affect the rural population, and the bottom 40 percent of Lesotho's population experienced a decline in consumption each year between 2002 and 2011. This compares to increases, albeit meager, for the remaining 60 percent of the population over the same period. Lesotho's gross domestic product (GDP) grew at an annual average rate of 4 percent between 2000 and 2016, whereas its GDP per capita grew at an average rate of 2.8 percent during the same period. Despite the high level of government spending, Lesotho faces challenges in addressing inclusive growth and providing access to quality services for the poor, while also operating in a highly fragile environment. After political turmoil, the new government with a fragile coalition of 7 parties was established in June 2017. The government is facing a significant challenge to improving access to and the quality of public services. It is also seeking to invigorate the domestic private sector to diversify the growth sources of its economy. The level of unemployment is very high, with a low employment-to working-age population ratio, which limits prospects for social mobility and poverty reduction.

Book excerpt: This Public Expenditure Review (PER) for health in Zimbabwe is an analytical report developed by the World Bank Health, Nutrition and Population Task Team working with the World Bank's Macroeconomics and Fiscal Management Global Practice team, Zimbabwe's Ministry of Finance and Economic Development (MOFED) and Ministry of Health and Child Care (MOHCC), with input from development partners. The aims of the PER are to provide objective evidence to inform the allocation of resources to enable equitable, efficient, and sustainable health care provision and to highlight questions about health sector performance for policy makers and key stakeholders. The findings also aim to present Zimbabwe's health sector spending in an international and regional perspective, highlighting key issues for improving health outcomes. The PER is based on analysis of primary and secondary data as well as an extensive review of existing literature. The research was complemented by interviews with key stakeholders, and data collection from major development partners and domestic health funders, including private companies. Micro-level data from household and health facility surveys in May-August 2014 (primarily for the impact evaluation of the Results-Based Financing (RBF) program supported by the World Bank) complemented these scarce data.

Malnutrition is intrinsically connected to human capital—undernutrition contributes to nearly half of child mortality, and stunting reduces productivity and earnings in adulthood. What is being spent and by whom and on what? To address these questions, nutrition public expenditure reviews (NPERs) determine the level of a country’s overall nutrition public spending and assess whether its expenditure profile will enable the country to realize its nutrition goals and objectives. When done well, NPERs go beyond simply quantifying how much is spent on nutrition; they measure how well money is being spent to achieve nutrition outcomes and identify specific recommendations for improvement. A Guiding Framework for Nutrition Public Expenditure Reviews presents the key elements of an NPER and offers guidance, practical steps, and examples for carrying out an NPER. The book draws upon good practices from past NPERs as well as common practices and expertise from public expenditure reviews in other sectors. This handbook is intended for practitioners who are tasked with carrying out NPERs. Other target audiences include country nutrition policy makers, development partner officials, government technical staff, and nutrition advocates. The book presents data and analytical challenges faced by previous NPER teams and lays out the kinds of analyses that past NPERs have been able to carry out and those that they were unable to perform because of data or capacity constraints. It concludes with further work needed at the global and country levels to create the conditions necessary to conduct more comprehensive NPERs.
