Book excerpt: La situation sanitaire dans le monde est caractérisée par la rareté de l'accès aux soins dans les pays en voie de développement, la croissance non maîtrisée des coûts de santé dans les pays développés. La santé étant l'un des éléments-clés du développement, il s'est instauré une coopération sanitaire internationale, qui revêt aujourd'hui deux aspects. La coopération institutionnelle s'articule autour des organisations inter-gouvernementales universelles et régionales et d'organisations non gouvernementales, telle la Croix-Rouge internationale qui contribuent chacune selon son domaine de compétence à la protection de la santé, ou qui collaborent à la réalisation d'un projet commun. Il s'agit d'une coopération à grande échelle dirigée et coordonnée par l'organisation mondiale de la santé dans le cadre du système des Nations Unies. L'action de l'OMS dans le monde - lutte contre les grandes maladies, recherche médicale, assistance technique - est sous-tendue par un programme stratégique, "la santé pour tous en l'an 2000", dont la pierre de touche est l'accès aux soins de santé primaires. La coopération interétatique, le plus souvent bilatérale consiste surtout en relations commerciales: transfert de technologie médicale, formation du personnel,... Plus rarement, elle prend la forme d'une aide des pays développés aux pays en voie de développement. La coopération sanitaire internationale s'insère dans le droit sanitaire international, ensemble de normes qui s'imposent aux états. Celles-ci sont de deux ordres. La Croix-rouge internationale a élaboré un droit humanitaire visant à la protection des personnes en temps de guerre à travers les conventions de Genève. L'organisation mondiale de la santé adopte des règlements ayant force obligatoire pour les états ainsi que des résolutions dans le cadre des compétences que lui a conférées sa constitution.
