L'objectif principal de cette étude était de rechercher si les patients sous anticoagulants oraux directs (AOD) sont moins nombreux que les patients sous antivitamines K (AVK) à savoir qu'ils prennent un anticoagulant. Matériel et méthodes. Une étude ancillaire a été réalisée lors de la phase 1 de l'étude CACAO (Comparaison des accidents et de leurs circonstances sous anticoagulants oraux). L'étude CACAO-1 est une étude descriptive transversale nationale, réalisée auprès de patients ayant un traitement anticoagulant oral et suivis en cabinet de médecine générale. Etait inclus tout patient majeur sous traitement anticoagulant oral se présentant, quel que soit le motif de consultation, chez un médecin généraliste investigateur Pour tout patient éligible, une question standardisée a été posée par le médecin généraliste avant même l'inclusion, selon la formulation suivante : « J'ai une question à vous poser pour une étude, je la pose à tous mes patients. Pouvez-vous me dire si parmi vos traitements, il y a un anticoagulant ? ». Résultats. 7 079 patients ont été inclus et analysés, dont 5 642 avaient un AVK (79.7%) et 1 437 un AOD (20.3%). 131 patients sous AVK (2.3%, IC95% [1.9-2.7]) et 70 patients sous AOD (4.9%, IC95% [3.8-6.0]) ont répondu « Non » à la question standardisée. La non connaissance du traitement anticoagulant était augmentée avec l'âge et diminuée avec l'autonomie du patient et son observance perçue par le médecin. Conclusion. Ces résultats mettent en relief les enjeux de l'éducation thérapeutique dans le suivi des patients sous AOD, et parmi eux, plus particulièrement les patients âgés.

Ils sont indispensables et sauvent de nombreuses vies, les pathologies qu'ils préviennent mettant souvent en jeu le pronostic vital. En 2013, environ 3 millions de personnes ont reçu au moins un médicament anticoagulant. Leur utilisation, qui concerne une population de plus en plus large, est associée à un risque d'accidents hémorragiques dont la prévention et la prise en charge constituent un enjeu majeur de santé publique. L'éducation thérapeutique faite par les médecins et le personnel paramédical est essentielle pour lutter contre la iatrogénie. L'objectif principal de la thèse était d'évaluer les perceptions des patients du traitement anticoagulant pouvant influencer l'utilisation du traitement. L'objectif secondaire était de concevoir un outil électronique d'aide au suivi à partir des données recueillies chez les patients. Matériel et méthode : Nous avons mené une étude qualitative sur 6 mois. Les patients ont répondu à des entretiens individuels comportant 5 questions réponses ouvertes. Un échantillonnage a été réalisé afin de recueillir des perceptions différentes des patients sous anticoagulants en termes d'âge, de sexe, de traitement prescris, de suivi médical. Analyse des données en codage ouvert avec triangulation de l'analyse. Résultats : Quinze entretiens individuels ont été réalisés. Tous les patients interrogés connaissaient leur traitement anticoagulant et l'indication de leur prescription. Ils avaient eu pour la plupart des séances d'éducation thérapeutique : préventions dans la vie quotidienne, les interférences, les adaptations posologiques en fonction des résultats biologiques. Les patients les plus jeunes étaient motivés par la création d'un outil d'aide au suivi avec pour principales attentes : un rappel de la prise du traitement, une communication directe avec le médecin, un rappel des interférences. Des propositions sont faites pour tenter de définir le contenu de cet outil. Conclusion : Les patients interrogés dans cette étude avaient des connaissances générales assez bonnes sur leur traitement anticoagulant. De nombreuses améliorations pourraient être apportées dans leur suivi grâce à la conception et l'utilisation d'un outil électronique permettant de gérer les situations difficiles.

Depuis 2008, il existe une alternative thérapeutique : les anticoagulants oraux directs (AOD) dont les indications ne cessent de s'élargir au cours des années. L'éducation thérapeutique est un élément clé pour la prise en charge des malades sous anticoagulants oraux du fait de leur haut risque iatrogénique. L'objectif de cette étude est de comparer les connaissances des patients sous anticoagulants oraux au long cours selon leur traitement : AVK versus AOD. Les objectifs secondaires sont d'évaluer les modalités de l'information thérapeutique, le ressenti et l'observance des patients selon la classe pharmacologique : AVK ou AOD. Méthodes : C'est une étude épidémiologique descriptive réalisée auprès des patients avec l'aide des médecins généralistes de Seine Maritime, des angiologues de la clinique Mathilde à Rouen, des médecins du service de médecine interne au Centre Hospitalier Universitaire (CHU) de Rouen, entre le 1er mars et le 31 mai 2017. Chaque patient était interrogé par unquestionnaire de 8 questions. Le critère de jugement principal était l'analyse comparative des moyennes des notes obtenues sur la connaissance du traitement entre les patients sous AVK et ceux sous AOD. Résultats : Cent vingt sept patients ont été inclus : 72 dans le groupe AVK et 52 dans le groupe AOD. La connaissance thérapeutique globale était satisfaisante avec une note moyenne respectivement de 5,69/8 dans le groupe AVK contre 5,48/8 dans groupe AOD sans différence significative. La surveillance biologique était significativement mieux connue dans le groupe AVK. Les facteurs influençant l'activité anticoagulante n'étaient pas suffisamment connus dans les deux groupes (régime alimentaire, interactions médicamenteuses). Les patients ayant reçu un livret d'information avaient significativement une meilleure connaissance du traitement anticoagulant que ceux ne l'ayant pas reçu (6,00/8 versus 5,26/ 8). Les patients sous AOD trouvaient le traitement significativement plus satisfaisant que ceux du groupe AVK. Conclusion : Notre étude suggère que les patients connaissent insuffisamment les facteurs influençant l'activité anticoagulante du traitement anticoagulant oral quelle qu'en soit la classe thérapeutique. Au vu du risque iatrogénique clairement identifié et insuffisamment connu, de nouvelles mesures d'éducation thérapeutique plus homogènes pourraient être proposées.

Book excerpt: The major cause of death in the Western world is some form of vascular disease; and principal among these forms is atherosclerotic heart disease (ASHD). Although much is known about the etiology and treatment of ASHD, there is, as yet, no specific means of prognosis of an impending coronary episode. There are, however, several indications of susceptibility to coronary disease, generally known as risk factors, the foremost of which is hyperlipidemia. Hyperlipidemia is more commonly designated as hypercholesteremia or triglyceridemia, depending upon which moiety is elevated, but since lipids are transported in the blood as members of a lipoprotein complex, the most descriptive general term would be hyperlipoproteinemia. This volume represents an effort to elucidate the origins and metabolic behavior of lipoproteins and their components, to describe aspects of the morphology, biochemistry and experimental induction of ASHD, and to describe modalities of treatment. The contributions to this book include descriptions of cholesterol synthesis and metabolism, as well as the metabolism of bile acids, the principal products of cholesterol metabolism. There are also chapters on the mechanisms of hyperlipidemia and on lipoprotein metabolism. The induction of experimental atherosclerosis and the aortic structural changes caused by this disease are discussed.
