Les principaux progrès sont notamment l'apparition des appareils d'imagerie Cone Beam et la chirurgie assistée par ordinateur. En alliant les principes généraux de l'implantologie et les nouvelles images issues de la reconstruction volumique à faisceau conique, le praticien peut à ce stade décider d'avoir recours à la chirurgie assistée par ordinateur pour poser les implants. Celle-ci peut être de trois types. Elle est passive, grâce à la navigation chirurgicale qui permet au chirurgien de suivre en temps réel la position de ses instruments localisés par repérage spatial infrarouge. Elle peut être également semi-active, grâce à la réalisation assistée par ordinateur de gouttières chirurgicales contraignant les gestes dans la direction prédeéterminée. Et enfin elle peut être active, grâce à des appareils autonomes programmés pour réaliser une partie du geste opératoire. L'assistance active ne sera pas développée. Ces techniques d'assistanat utilisent des logiciels de planification pour un diagnostic précis, une préparation et planification adaptées aux multiples doléances (risques anatomiques, dents adjacentes et antagonistes et position de la future prothèse implanto-portée) et enfin un geste opératoire le plus conforme possible à la planification. Il sont dépendants ou intégrés aux appareils Cone Beam. Une présentation de plusieurs cas cliniques permet de se rendre compte de l'utilité de toutes ces nouveautés, en fonction de la situation clinique et du projet prothétique. Cette thèse a pour but de développer toutes ces nouvelles technologies informatiques apparues ces vingt dernières années.

Book excerpt: Depuis les premiers travaux du Pr Brånemark, à qui l'on doit la découverte de l'exceptionnelle affinité du titane pour l'os vivant, l'implantologie a connu des avancées techniques et technologiques considérables. L'étude et la mise en oeuvre chirurgicale du positionnement d'un implant au sein de l'os alvéolaire se sont notamment précisées grâce à l'évolution de l'imagerie médicale et des logiciels d'exploitation à disposition du chirurgien. Cependant, le risque d'erreur humaine en per-opératoire est toujours présent. La robotique médicale a fait ses preuves en médecine, en augmentant la précision et la sécurité d'un acte chirurgical, ce qui a rendu possible des chirurgies anciennement considérées comme trop risquées (dans le domaine de la neurochirurgie notamment). En 2017, des robots ont été conçu pour guider le chirurgien-dentiste dans l'ostéotomie et la pose de l'implant. Ces robots permettent la réalisation d'un acte chirurgical reproduisant parfaitement la planification implantaire virtuelle réalisée sur les logiciels informatiques. Nous allons décrire les étapes de développement de la robotique en implantologie ainsi que les nouvelles technologies qui ont abouti à la fabrication des robots de dernière génération. L'objectif de cette thèse est de comprendre le fonctionnement et les bénéfices apportés par le guidage robotique lors d'une chirurgie implantaire.
